While lowering the anvil into the Remco, EE was holding the anvil and a co-worker was pulling with a strap to move it into place. It wasn't lined up, so they were trying to maneuver into place. At some point the cable of the crane got some slack and then released smashing EE's left pinky finger between the anvil and the rotor.

A belt ripped on an 8x20 screen causing mud to fall through and build up on the tail Polly stopping the conveyor. LOTO was performed. EE started chipping mud off the Polly using a 30-inch pry bar to knock the mud away. EE used EE's right hand to brace self against the frame of the belt and used EE's left to chip. As EE was chipping, EE missed the Polly and hit EE's right thumb.

Employee was adjusting the jaw wedge plates and couldn't get the nut loose with cordless impact. Employee decided to use an open ended wrench and got the nut to come loose, as it broke free elbow struck the corner of the jaw and cut elbow open. Employee received stitches and a prescription for an antibiotic.

EE had been experiencing pain in shoulder after shoveling and lifting.

Climbing into loader by pulling himself with his arms. Injury date 4/24/08, seen by Dr. 5/7/08, went recordable 6/5/08 with an injection of cortizone. No time off from work or work restrictions.

CREATING STOCKPILES WITH THE DOZER. UPON PUTTING THE DOZER IN REVERSE, IT STALLED CAUSING IT TO ROLL FORWARD AND OVER THE 6-8 FOOT EDGE THAT WAS THERE. EMPLOYEE JERKED HARD BY THE SEATBELT HE WAS WEARING.

USING A CUTTING TORCH IN A 9 TO 10 INCH OPENING UNDER JAW BELLY BELT TO CUT A BOLT, EMPLOYEE MADE THE CUT & WAS MOVING THE TORCH TO SHUT IT OFF. TORCH HEAD CAUGHT ON THE FRAME & SPUN AROUND TH E FLAME, CROSSING HIS ARM.

EMPLOYEE WAS INSTALLING V-BELT ON DRIVE MOTOR TO CRUSHER. SLIPPED ON MOTOR LANDING, AS HE CAUGHT HIS BALANCE HE SLIGHTLY STRAINED HIS LOWER BACK.

EE W/DIABETIC CONDITION STATES HE FELT HIS BLOODSUGAR GETTING LOW & TRIED TO CORRECT WITH CANDY WHEN HE LOST CONSCIOUSNESS FELL & STRUCK HIS HEAD CAUSING LACERATION REQUIRING 8 STITCHES

EMPLOYEE PUSHING, LIFTING BELTING INTO LOADER BUCKET STRAINED RIGHT BICEP.

EE NOTICE SLIGHT PAIN IN HER LEG WHILE CHECKING OIL IN THE MILL AT BEGINNING OF THE SHIFT. WHEN CREW BEGAN BAG PRODUCTION (FILLING BAGS & STACKING THEM), EE NOTICED THE PAIN INCREASED. AT 1000 , EE CAME INTO THE OFFICE TO REPORT PAIN TOO INTENSE TO CONTINUE WORK. EE SAID SHE DOESN'T KNOW OF ANY INJURY THAT WOULD CAUSE THIS.

EE WAS MOUNTING THE 950 LOADER WHILE AT THE C4 HOPPER STEPPED ONTO A ROCK WITH HIS RIGHT FOOT & HEARD SOMETHING POP IN HIS RIGHT ANKLE-ANKLE SPRAIN EE STATED HE HAS PREVIOUS SPORTS INJURIES TO THIS ANKLE WAITING FOR DR REPORT BEFORE FILING.

HE NOTICED ENGINE COOLANT TEMPERATURE WARMER THAN USUAL. BROUGHT TRUCK TO SHOP AREA. PARKED TRUCK WITH ENGINE IDLING. REMOVED RADIATOR CAP AND EXCESS PRESSURE TO COOLANT SYSTEM DISCHARGED HOT OIL AND COOLANT OUT OF RADIATOR CAP HOLE ONTO HIS ARMS CAUSING 2ND DEGREE BURN FROM WRIST TO ELBOW AND UPPER ARM ON TOP.

ON 9/16/96 EE COMPLAINED OF SORENESS IN RIGHT SHULDER WHILE WALKING SOWN STEPS EE SLIPPED & LOST HIS BALANCE CATCHING HIMSELF WITH HIS RIGHT HAND ON HAND RAIL. [FIRST AID TREATMENT] EE SOUGHT SURGICAL TREATMENT 10/22/96. [MEDICAL TREATMENT]EE WILL RETURN TO WORK 11/11/96.

WHILE DROPPING LOADED (GRAVITY) RAILROAD CAR, THE CAR IMPACTED WITH A PARKED CAR CAUSING EMPLOYEE TO BRUISE RIBS.
