KIMBALLTON PLANT #1 Metal/Non-Metal

The injured employee was pushing a large wheelbarrow loaded with scale on the cooler floor, up the inclined drag line ramp. About halfway up the ramp, they heard a loud pop in their left ankle. They stopped and tried to walk, but was unable to put any weight on it.

Two Maintenance Technicians were unloading 8 X 8 X 48 long Treated lumber wood blocks for the track pedestrian crossing. They were sliding blocks out of the back of a pickup, when one of the blocks slipped out of the grip of the injured EE. The block fell onto their right thumb. The injured EE went to the ER and diagnosed with a hairline fracture of the first digit of the thumb.

Injured employee was struck by a 6 foot long aluminum bar being used by another employee, while trying to unplug the old side silo screen.

The employee started work at 7:00 PM on May 31 and was walking back to change house after the shift ended at 7:00 AM on Jun 1. EE stepped on a RR track, then tripped. EE's foot caught on the track and EE's hand broke the fall. EE's finger caught on a bent spike. EE was wearing all of the PPE except gloves. EE fractured right ring finger and lost a fingernail.

Employee was sitting on the ground underneath Kiln 1, and was using a cable stripping tool to remove the insulation from a new electrical cable that was being installed for Kiln 3. The employee was pulling the tool towards them, when the tool slipped and the blade struck them in the left knee. Employee received a laceration on the knee.

Employee was putting crib block behind the global pump tire, and EE pinned finger between the crib block and trailer.

The injured employee was helping a another Miner smash empty 55-gallon drums, and putting them in the loader bucket. EE lifted an empty drum which weighed approximately 20 lbs, and twisted body to put it in the bucket. EE then felt a sharp pain go up EE's back.

Employee was troubleshooting the Kiln 3 lube system around 9:30 AM. The temp was 81 deg F, with a heat index of 95 deg F. After returning to the shop to work on the pressure switch, EE asked for help because EE was shaking too bad to work. EE sat down to cool off but began to cramp from possible dehydration. EE was diagnosed at the ER with heat exhaustion and dehydration.

The employee was slicing conveyor belt and was cutting the belt with a utility knife. As EE was cutting the belt inside the belt frame, the top of the knife struck the chain of a come-along and caused the knife to fly out of EE's left hand. The blade of the knife struck EE on the inside of the right forearm, causing a deep puncture wound that required stitches.

Employee was walking up the walkway adjacent to the coal belt, tripped on something and fell onto the walkway. Stated they did not know what they tripped on, as there were no hazards present and the walkway was clear. EE braced the fall with right hand, but left hand contacted the grating, causing swelling to left hand and a fractured left pinky finger.

Employee was performing an alignment on a lime kiln when they came in contact with Lime dust which resulted in lime burn to right arm and neck. Employee reported this incident on 4/1/20.

EE was pulling out from dumping a load of stone in a stock area. The Shift Supervisor pulled ahead of EE and in the exchange, EE forgot the bed was still up. EE pulled out and caught the raised bed in the top, throwing EE into the steering wheel. EE sustained a laceration to the forehead over EE's left eye.

Employee was attempting to empty out the Guzzler vacuum truck, but the cylinder latch on the right hand side would not release. Employee attempted to free it by using a mattock to hit up on the latch, but it did not release. When doing so they caught the left index finger between the mattock handle and a bolt on the back of the truck.

Injured employee and another mechanic were moving a coupling shaft to the back of a flatbed truck when the injured employee's right pinkie finger was caught by the grid coupling which cut through the glove and finger. Total weight of the coupling was about 100-110 pounds. The coupling teeth cut through the glove and into the right pinkie finger.

Employee fell from a vertical ladder at the #2 coal mill on the Cooler floor, and struck head, causing a concussion. Employee does not remember what happened and there were no witnesses.

Temporary employee was underground, operating the #16 haul truck. EE was backing under the loader when EE received first bucket load of 5th trip of the shift. The load shook the truck more than normal and EE felt neck tighten and pain shot through it.

The employee attempted to adjust a strap on a hook as the winch was taking up slack in the line. The line became taut and the employee's fingertip was pinched momentarily between the strap and hook saddle.

Injury occurred 09/17/18, but did not become reportable until 11/30/18 (dental care reqd). EE (injured) and another mechanic completed welding in the screenhouse. Co-worker accidentally dropped a bag of welding rods onto EE's face, causing a broken upper tooth. Dental care in late Nov was reqd.

EE (injured Miner) and another Miner were hanging curtains in the 8WFWSWM area. EE was holding the curtain while the other Miner was using a hand held drill to drill a small hole in the curtain, so they could pin it to the wall. The drill caught the curtain and ee glove, spinning the glove and hand up in the curtain. ee right thumb was fractured.

Employee was chipping concrete with jackhammer and sledge hammer; lime dust was also present; dust got in employee's eye causing to scratch right eye.

Employee was walking on an uneven concrete surface on the cooler floor (east side). EE was unable to see the uneven surface because it was covered with lime accumulation. EE twisted left ankle, then fell on the right knee, causing strains to both. The injured employee is a Lhoist North America accounting auditor, and not an employee of the Kimballton mine.

A Miner was lifting a 12" dia X 20 ft long piece of steel pipe from a dumpster on the 13th level of the mine. The pipe hung on the edge of the dumpster and broke loose, striking the injured employee on the left shoulder. Injured employee was standing too close to the lift zone.

A miner was loading the heading with explosives, when EE started smelling ammonia and got a headache. As EE started loading the lifters, EE began to feel sick. Two other miners in the area noticed the ammonia like smell in the area. Initial indications are that ANFO may have reacted with waste lime kiln dust in the same work area.

While setting limits on the Kiln 1 skip traveler, the injured employee noticed that the slack cable safety switch wire had been pulled out of the switch body, while they were repairing the high speed shaft and skip cable. Employee asked for assistance from Electrical Supervisor. Employee improperly used a knife to cut tie-wrap that was used to hoist material up.

Employee was walking on the top of the #1 fuel bin on a sloped surface which had residue of fuel and lime. Employee slipped on the residue and slid into the corner of a hatch door. He received a puncture wound to the left side of his torso.

Employee was using a hand grinder to grind welds on the screen of the Old Side Silo, when he stated he felt something in his right eye. He was wearing safety glasses and a face shield. Employee did not report the injury on Friday 7/24 as required, and reported it to supervision Sunday 7/26.

Employee was using a steel bar to unplug the stone feeder hopper in the 36" mine belt tunnel on the surface (below the stone pocket). A rock fell down inside the hopper and struck the steel bar, causing the bar to strike the employee in the face just above the nose.

Employee completed loading lime into a railcar. He was in the process of closing the doors on the top of the railcar. The door hung and he overexerted his back as he attempted to close the door. Injury occurred Aug 30. Employee waited to visit the Dr. on Sep 1. No Dr.'s report was provided to EHS Manager until Sept 18th. Injury reclassified as MSHA reportable Sept 18th.

Employee was exiting the southwest door of the kiln firing floor to get some vacuum hose. He slipped on some hardened slick lime (from recent rainfall) just outside of the doorway, and fell on his right wrist.

Employee was climbing on a stepladder propped against the #1 belt structure on the #1 belt slope. He was standing on the 2nd rung from the bottom approximately 18 inches off the ground, when the right foot of the ladder sunk in the mud. The ladder spun off the belt structure and the Miner fell to the ground. He received contusions and abrasions to his right shoulder and arm.

Employee had been raking scale out of Kiln 1 on August 3rd using a steel rake. He developed a blister on his right palm from repetitive raking, and was wearing leather gloves at the time. The employee worked on August 4th, and the blister became infected and red streaks were running up his right arm. He went to the emergency room and was diagnosed with cellulitis.

Employee was welding tyre pads on the Kiln 1 Pier 2, when he experienced heat exhaustion. Employee was working in a high heat environment and began cramping and feeling nauseous.

Injured employee was checking the Kiln #3 fire box for scale. As he was closing the steel door, he caught his left pinky finger between the door handle and the latch.

Employee working on the drilled face on the 15E SW tunnel foot wall. Employee was getting in the Getman blasting rig basket, when the face popped and a small rock slab fell on him while in the basket. Employee was not performing any physical work on the wall, but was simply preparing to go up in the basket to perform work.

Employee was working on uneven and sloped surface in the underground mine at the 7W to 6W level pipeline raise hole. Employee was moving pipe back into position, when he slipped and fell. Employee sprained his right knee and had minor contusion and laceration. Employee will be on modified work duty until released by a Dr.

Employee was cleaning the tail pulley area underneath the apron conveyor at the Old Side Silo. He was kneeling to access the area with a broom. When he finished cleaning, he stood up and felt his knee pop. Employee diagnosed with a strained knee.

Employee was using a sledgehammer to dislodge hydrate from the K1 hydrate feed bin, and strained his left shoulder.

Employee was helping other employees unload 150 each 50 lbs bags of Anfo explosives on the 2nd level of the mine. Employee overexerted his left shoulder and was diagnosed with a strain of the left shoulder and was given restricted duty.

EE was drilling a starter hole in the duct work. The drill left a piece of metal shaving. EE had gloves on, but when he tried to pull the duct work, a piece of the metal cut his finger.

Employee was driving the feeder shaft of the 10E Crusher in, when his left middle finger was caught between the guard and hammer handle.

EE was working at the plant on 1-6-2011. The EE did not complain of any lime in his eye while he was at work. EE left work at his scheduled quitting time, eye seemed irritated, called the plant and was sent to see the eye doctor, where a piece of lime was removed. EE returned to work.

Employee was stationing the wedge connector for the cable and another emplyee was swinging a 2 pound hammer. The hammer glanced off the wedge and struck the fingers of injuried employee.

Employee was preparing to descend into pit for #1 feed elevator. The access panel covering the opening was removed. Employee tripped and fell, striking the right side of his rib cage on pit opening. X-rays revealed a fracture of the 4th rib.

On February 16, 2010, Injury (Lhoist NA) was visiting the Kimballton underground mine. While walking underground with other experienced mine employee(s), He twisted his ankle navigating around a muck pile. Upon taking him to the hospital, x-rays revealed a broken bone. He was at our facility to get familiar with our operation.

Employee was injured while placing a reach-extending pipe onto the end of the rock breaker at the crusher. The pipe slipped off, catching his thumb between the pipe and a concrete wall. The extension is used to reach up into the truck dump ore pass and dislodge hung rocks. Went to the ER no broken bones, received 18 stiches.

Employees was scaling when a rock fell striking the scaling bar causing the employees hand to get caught between the bar and the top rail of the scaling basket.

While working on replacing bearings felt a pull in lower back but continued to work. While coming down steps started to feel a shooting pain in his lower back.

Employee was removing a belt roller to free up netal that was wedged in a chute and the roller fillped over smashing the middle and ring finger of the left hand.

Employee pulled on a concrete form to strip it from the concrete. He did not report that he was in pain at the time. He worked his shift and returned to work the next day and worked. He was in obvious discomfort and was required to seek medical evaluation. The doctor said that he had a strained or damaged rotator cuff injury and laid the employee off for 2 weeks.

Installing fan on #9 loader and felt a pull/pain in groin. Went to doctor a couple of days after incident due to pain. Had no hernia but doctor put him on light duty for 7 days.

Working on the coal crusher and a sledge hammer lying on an elevated surface fell, contacting his left hand. Went emergency room for x-rays but nothing was broken. Doctor placed him light duty for 10 days.

Moving gantry crane to support loader bucket and employee states he felt his knee pop. This is the same knee he had surgury on in 2008. It was thought this was an aggrevation of an existing condition. Went to the doctor on February 5, 2009 and doctor said it was a new incident.

Employee was loading shells in the kiln gun and laid his left hand on the gun (on or near the re-coil spring) and once the gun was fired the spring pinched his hand. Went to the emergency room to have it looked at. No stitches or broken bones.

Employee was cleaning spill behind mills & tripped over cover {unsecured} in floor. Employee went to catch himself & fell to his knees & felt strain in center of lower back.

While loading a shot, the employees right index finger was struck by a rock and needed to be surgically removed at the second knuckle.

Employee was stepping over a screwline when his foot got caught on the screw causing him to fall. When he went to catch himself he sprained his wrist. X-rays revealed a small chip in his wrist.

Employee was climbing down off #2 belt when his right foot slipped on rail and got hung on energency pull cord. Once off balance employee went to put his left foot down on ground causing his left knee to twist. X-rays revealed two fractures to his upper tibia.

Employees was tramming the Fletcher 2-boom jumbo drill when it disengaged causing it to free wheel. Operator reached for park brake and pulled/strained right shoulder.

Employee was changing out fan blades on a ventilation fan and stepped down from an elevation of approx 1 foot when his foot slipped and twisted his right knee.

Employee was using a knife to cut a piece of belting; the knife slipped and cut the top of his thumb requiring 7 stitches.

EE was bagging lime when some lime got under his shirt sleeve & he didn't wash off & worked all day and burned his wrist.

Employee was scaling the roof of a recently blasted area when a rock fell from the roof grazing the left side of his face and striking his right forearm causing a laceration that required 5 stiches.

A metal plate was cut with a torch leaving jagged edges. The employee was attempting to help move the metal when his wrist came into contact with one of the jagged edges cutting it.

Apparently at approx 1:00 am on November 5, 2006 employee was stepping down on a railcar to close the hatches; he had one foot on the ladder and one on the top of the car. When he put his weight on the foot placed on the top of the railcar, it gave way causing a severe sprain.

Employee used a compressor to climb down from a belt line. When employee stepped on the down dip of the axle it created a seesaw effect and employee fell off spraining his ankle.

Employee was on stomach working on a Fletcher Scaler trying to re-connect a #20 hose. Employee felt left rib pain and could not finish the job. Physician?s diagnosis was contusion to left chest wall.

Employee was cleaning with a bobcat loader and some heated dust blew out of the hopper and burnt his arm, face and neck.

Employee was getting out of haul truck and mis-judged where the handrail was causing him to come down hard on his foot, which caused a hairline fracture.

WAS COMING OFF A COAL CAR. WHEN THE CAR STOPPED, IT JERKED THE CABLE HITTING HIS LEG AND CUTTING IT.

Employee was check scaling when a small rock fell and struck the employee.

Unknown conditions. Was found by a foreman (Jeff Morgan), as he was traveling in the mine. No one was working in the area when the fall occurred.

WAS UNSTOPPING THE OXYGEN ANALYSER ON THE BACK OF #1 KILN WHEN A PUFF OF STEAM AND HOT GAS CAME BACK IN THE PIPE AND BURNT HIS ARM.

HE WAS CLEANING OUT BALL MACHINE WHEN MATERIAL FELL FROM THE SIDE OF THE MACHINE AND STRUCK HIS HARD HAT.

EE WAS TRAVELING UP IN A SCALING RIG WHEN THE BAR HIT THE SIDE OF THE RIB AND A ROCK DISLODGED, HITTING THE SCALING BAR AND GLANCING OFF HIS ARM.

EE WAS TAKING A STONE SAMPLE WITH PLASTIC BUCKETWHEN THE BUCKET HIT HIS FINGER & BENT HIS FINGER

WAS CLIMBING UP ONTO CAT HAUL TRK WHEN HIS FOOT SLIPPED & HE FELL APPROX 5' TO THE GROUND

WAS REMOVING FITTINGS FROM BURST HYUDRAULIC HOSE ON HOSE MACHINE, GLOVE GOT CAUGHT ON HOSE AND TWISTED HAND.

USING A PRY BAR TO CLOSE DOOR ON CONRAIL RAIL COAL CAR, BAR SLIPPED & STRUCK EE ON FOREHEAD

CUTTING CABLE WITH CABLE CUTTER, AND SLEDGE HAMMER. ANOTHER EMPLOYEE WAS HITTING THE CABLE AND DID NOT CUT THE CABLE THE FIRST TIME CAUSING HIM TO CUT ON ANOTHER SECTION WHEN HE HIT THE CABLE CUTTER THE 2ND TIME CAUSING IT TO SLIVER SMALL PIECES HITTING INJURED IN LEFT TEMPLE AREA.

USING A BAR TO UNLODGE MATERIAL IN FEED ELEVATOR. BAR HAD A SHARP EDGE ON IT. THUMB CAME IN CONTACT WITH IT AND LACERATED THE THUMB APPROX 3/4".

MECHANIC WAS TIGHTENING OIL PLUG IN TRUCK AFTER CHANGING THE OIL. THE WRENCH SLIPPED OFF THE PLUG AND HIS ARM CAME IN CONTACT WITH THE DRAIN PAN EDGE.

LIME BURN TO EYES-CLEANING OUT VENTURI IN HYDRATE DEPT.

LACERATION REQUIRING 2 SUTURES TO RING FINGER OF LEFT HAND-LACERATION WAS CAUSED WHEN METAL PLATE SLIPPED IN HIS HAND.

EE WAS OPERATING A 988 LOADER ON 11-7 SHIFT, THE LOADER WAS TRAVELING TO BALL MACHINE AREA WHEN IT WENT INTO A LOW POINT IN THE ROAD. WHEN THE MACHINE SLOPED TO THE SIDE LEFT HE RESPONDED BY P UTTING HIS ARM OUT TO SUPPORT HIMSELF THE DOOR TOF THE LOADER CLOSED AND CAUGHT HIS FINGER.

WHILE REMOVING METAL BALLS FROM BALL MACHINE A PIECE OF EITHER THE BALL OR THE CHISEL STRUCK EE IN LEFT FOREARM. AT THE TIME THERE WAS MINIMAL BLEEDILNG & ONLY THOUGHT TO BE A LACERATION. ON M AY 12, EE REPORTED TO SUPERVISOR THAT HE THOUGHT THERE MIGHT BE SOME METAL IN HIS ARM BECAUSE IT WASN'T GETTING BETTER.

SCALING IN GE LIFT WHEN A SMALL ROCK FELL OUT OFCROSS CUT AND HIT EE'S HARD HAT.

SMALL ROCK HIT RIGHT FOREARM CAUSING LACERATION 3/4" LONG. EE WAS SCALING IN 15 EAST #1 TUNNEL #8 WINDOW. FOUR SUTURES.

EE WAS USING SHORT PRY BAR TO FREE JAMMED STONE FROM CHUTE FEED-O-WEIGHT- BAR MASHED FINGER BETWEEN SAFETY BARS OF NO. 2 ELEVATOR PIT.

BELT GUARD WAS PROPPED UP WHEN EE PASSED BY - KNOCKING GUARD OVER ON HIS LEFT FOOT.,

EE PLACED HIS RIGHT HAND ON THE DOOR FRAME OF A 988 CAT END LOADER & WITH HIS LEFT HAND PULLED THE DOOR TO - MASHING THE RIGHT THUMB.

WASTE HAUL TRUCK WAS BACKED INTO BERM AND ON THE THIRD PASS THE BERM WAS KNOCKED DOWN WITH THE TRUCK DRIVING BACKWARDS OVER THE EDGE OF NUMBER 2 MINE LEVEL. THE TRUCK SLID BACKWARDS IN THE MAT ERIAL COMING TO REST BETWEEN THE 6TH AND 7TH LEVEL RESTING ON THE PASSSENGER SIDE AT A 45' INCLINE SUPPORTED BY THE WASTE MATERIAL AND PILLAR.

DRYING RELLING BELT WITH AN AIR HOSE AT GUARD ONDRIVE, DRIVE ROLL CAUGHT AIR LINE JERKING HAND AGAINST GUARD PINCHING LEFT LITTLE FINGER.

EE WAS STANDING ON A SLICK METAL SURFACE PULLINGON A ROPE - ROPE BROKE & EE FELL BACKWARDS ON BUTTOCKS CAUSING MUSCLE TEAR AROUND ???? LOW BACK GLUTEUS STRAIN.

BRAKES ON RAIL CAR WERE NOT IN WORKING ORDER TO HOLD/STOP LOADED CAR.

EMPLOYEE STATES THAT HE WAS STEPPING OFF HAUL TRUCK LADDER AND TWISTED HIS BACK.

EMPLOYEE STEPPED OFF TRUCK ONTO LOOSE GRAVEL--SLIPPED--TWISTING ANKLE WHEN FALLING.

EMPLOYEE WAS USING A BAND SAW TO CUT A BROOM STICK WHEN HE GOT A GLOVE HE WAS WEARING CAUGHT IN THE SAW BLADE THE SAW CUT INTO THE TOP OF HIS INDEX & MIDDLE FINGER REQUIRING SUTURES TO CLOSE

EMPLOYEE WAS WALKING ACROSS RAILS AND TRIPPED, GRABBING A RAIL CAR CATWALK, HIS RIGHT INDEX FINGER WENT THROUGH A SERRATED HOLE IN THE RUNG OF THE CATWALK/FRACTURING THE BONE AND TEARING THE S KIN OF THE FINGER AS HE FELL.

MOVING A HYDRAULIC CYLINDER FROM A PALLET & STRAINED SHUOLDER & RT FOREMOST FINGER OF LEFT HAND

WORKING IN 14 EAST X 1 MAIN WHEN A ROCK FELL FROM UNSCALED TOP CAUSING A LACERATION OF APPORXIMATELY 2" ON RIGHT FOREARM.

EMPLOYEE STEPPED FROM CAB OF 75' SCALING RIG AND FELL FROM LADDER. HIS FOOT WENT UNDER THE OUTRIGGER CO-WORKER WAS LOWERING.

EMPLOYEE WAS REMOVING BRICK WITH A JACKHAMMER WHEN THE JACKHAMMER SLID FROM AN ELEVATED POSITION WITH THE CHISEL EDGE STRIKING HIS LEFT FOOT

INJURED PLACED HIMSELF IN A HAZARDOUS SITUATION BY STADDLING THE ROCK BOX OF THE CRUSHER WHILE WORKING COME-A-LONG TO DRAG JAW DIE OFF OF CRUSHER RAIL,WHEN DIE DISLODGED,BEING SUSPENEDED BY HO IST,IT SWUNG,STRIKING HIM ON THE RIGHT LEG.

HEATED LIME DROPPED FROM BUCKET OF LOADER INTP PUDDLE OF WATER CAUSING STEAM BURNS TO LOADER OPERATOR.

SCALING BAR STRUCK EMPLOYEE IN HEAD WHILE PRYING ROCK LOOSE FROM ROOF, KNOCKING HARD HAT OFF AND CUTTING 1 1/4" GASH IN HEAD.

TWO EMPLOYEES WEREE SCALING A HEADING (FACE) IN 11 E #1 MAIN. INJURED WAS ACCIDENTLY STRUCK BY ANOTHER EMPLOYEE WITH THE SCALING BAR BEHIND THE RIGHT EAR ON THE MASTOID PROCESS CAUSING SWELLI NG.

EMPLOYEE WAS HOLDING A WIPER BUSHING WITH PIPE WRENCH WHILE ANOTHER EMPLOYEE WAS ATTEMPTING TO FREE BY STRIKING WITH HAMMER. HAMMER HANDLE STRUCK EMPLOYEE'S RIGHT LITTLE FINGER PINCHING IT ON TRUCK CAB. LACERATION TO RIGHT LITTLE FINGER REQUIRING FIVE STITCHES.

WET SLIPPERY SURFACE CAUSED FALL- BRACED SELF BY CATCHING RAIL AND SPRAINED THUMB.
