Pulling a cable along the framework of conveyor and stepped in hole in grating. Associate fell and impacted knee, back and arm on grating.
A metal cover fell down and struck employee on the right side of the face/jaw as ee held the ladder for another employee to perform the work.

While walking with his supervisor, the injured miner slipped on a rock, caught himself mid-fall, and felt a sharp pain in his lower back.

Employee was swinging a sledge hammer when his left hand small finger came in contact with a channel brace which resulted in the employee having to have three stitches and his nail removed.
Employee was operating Komatsu PC 300 with a hydraulic hammer, a small stone came thru screen over windshield and shattered windshield, and piece of glass lodged in employee's index finger on his left hand.
The employee stated he was climbing down the steps off the John Deere haul truck. When his left foot touched the ground, his right boot caught on the bottom step causing him to fall and bump his right knee. The employee refused medical attention when he reported the incident at 16:45 P.M.

EE was dismounting a Volvo A-40D Haul Truck using the steps that are mounted on the cab section. As he was climbing down the stairs, one of his feet slipped on the last step (2' above ground). The slippage of his foot caused him to jump to the ground where he landed on his feet in an awkward position. He fractured his back in two places as a result.
EE was climbing a ladder to access the bed of a service vehicle to acquire tools. EE grabbed the top section of the ladder where it is secured to the service truck by a bolt on each side. EE'S middle finger on his right hand became pinched at the pivot point and was wounded. EE received 10 stitches on his middle finger.

EMPLOYEE WAS EXITING HAUL TRUCK WHEN HE STEPPED ONTO UNEVEN GROUND AND TWISTED HIS ANKLE. EMPLOYEE WAS PLACED IN AN "AIR CAST" FOR 2-3 WEEKS.
