EE was changing a steering cylinder on a haul truck. The cylinder slipped off the ear it fastens to and mashed the EE's left middle and ring fingers between the ear and cylinder lacerating the middle finger requiring stitches, thus making this accident reportable to MSHA.

EE was helping to change a return roller, claims they hurt a shoulder stating they didn't know they hurt it but that was the only task that could've hurt it. After some discussion we have decided to file a worker's comp claim until further investigation. EE has missed work because of this making it a lost time accident and reportable to MSHA.

EE was performing maintenance in the middle deck of a screen. EE slid under the screen tube to access problem area and raised up hitting the left side of their head on the screen tubes protective cover. The EE lacerated their head and received two staples to close the laceration making this incident reportable to MSHA.

EE was unstopping feeder in surge tunnel. Rock came out of feeder and pinched EE's thumb between a feeder support and the rock, causing a laceration just behind the cuticle on the thumb. The laceration required 3 stitches.

EE was climbing on back of welding truck, slipped and hit left knee. It swelled up, EE went to doctor, and doctor said it was just bruised. On follow-up appt on June 14th, Dr. said possible meniscus tear, gave cortisone injection, and recommended physical therapy, but did not modify duty. Reporting based on Doctor's actions at June 14th appt.

EE was assisting installing impeller in a GIW pump. The impeller was sitting upright between the forks on the forklift. The impeller shifted a little forward on the forks, and the EE got in front of the forks to reposition the impeller. The impeller fell forward off the forks striking the EE in the right ankle and foot.

The EE's were pulling a shaft through a pipe for a gate at the plant with a come-along. They had a pad eye welded to the end of the shaft. The EE looked in the pipe to measure distant left. The pad eye pulled a chunk out of the shaft and hit the EE in the safety glasses. The safety glasses pressed hard enough to cut the right side of the EE's face and require six stitches.

The EE was helping to change a return roller on the No. 1 belt. Another EE was holding one side, and the roller slipped out of EE's hands. The roller came down and hit the injured EE's hand, causing a fracture on the right hand.

EE was tightening bolts on pipe flange on pump in the pit. EE had wrench in left hand and ratchet in right. Left hand slipped off wrench and right hand pulling pinched middle left finger between pipe and wrench. Laceration to left middle finger requiring 6 stitches.

While installing skirt boards on #9 conveyor, the EE decided to lift up on the tongue of a trailer and try to move it instead of using a truck/trailer hitch or forklift. The employee strained a muscle in back and has missed time from work due to the accident, thus making the incident reportable.

EE was working on the scalping tower installing new chutes when a neighbor began firing an automatic weapon in the direction of the screening tower from a neighboring property. One of the bullets hit the EE in the right, upper thigh. The EE was taken to the hospital, treated and released for THE injuries. EE missed two days of work due to the incident.

EEs were lowering jaw die into chute for wear purposes when the die got hung on the walls. They cut one side of the die off, the die fell, and the chain hit the EE in the right knee. The EE has a fractured patella on the right leg, causing the injury to be reportable.

EE reported for work, clocked in and was in breakroom. EE was putting work boots on and felt a pain in back and had to leave to go to the doctor. The EE missed the next day, making the incident reportable, but there was no accident involved in this incident. The EE came back the 2nd day with no issues.

EE was helping to change pulley on No.1 belt. Bearing on the pulley was stuck. The EE thought that they could free the bearing up with their hands. When they freed the bearing up, the pulley and bearing came free and pinched EE's fingers between conveyor frame. EE had to get Stitches in fingers, making accident reportable.

EE was fueling haul truck and the nozzle slipped out of hand. EE jumped back to avoid getting splashed, hit the safety chain for the diesel containment, and fell over the chain and into the containment. EE hit leg on something causing a bruise on right leg. The doctor kept ee out of work for two days to be cautious, causing the accident to be reportable.

EE removed heater core from Skidsteer loader. Due to location, EE had hard hat off to remove. EE took the heater core to parts washer and laid it on top of washer, before putting hard hat on. EE bent over to pick up strap and heater core fell and hit top of head. Laceration required 3 staples.

During a safety Meeting, the EE stooped down against the wall. When EE stood up, they fainted and fell to the floor, causing a laceration above right eye, requiring stitches. The doctors kept EE in hospital to resolve cause of fainting. They concluded that it was EE's blood pressure medicine that caused EE to faint, and changed the medication.

The EE was attempting to change a leaking hose on a CAT 988 Loader. The line was under the cab, so the employee was in an awkward position getting to the bolts. While trying to loosen the bolts, the EE felt a pain in abdomen. EE did not go to the doctor until 6/13/16, where the doctor diagnosed a hernia, placed EE on restrictions, thereby making the accident reportable.

The EE is the Pit Loader operator. EE was digging in a difficult shot and hit EE's right elbow several times throughout the day. EE went to the doctor on 2/11/16. Doctor wrote EE out-of-work until 2/18/16. On 2/16/16, EE went to a specialist, and they said it was a deep bruise. Doctor gave EE medicine, and the swelling and pain went away. EE returned on 2/18/16.

On Saturday, 12/5/15, EE complained of pain to his right hand and swelling. He was unsure as to cause. The doctors found metal in his hand on 12/13/15 from where he had cleaned out hydraulic tank on 12/4/15, which caused infection and hospitalization, thereby making it a reportable accident. Until 12/13/15, the doctors thought it could be a spider bite or boil under the skin.

EE was attempting to dislodge a rock from a tail pulley with a pry bar. The pry bar slipped and the EE hit his right thumb on a brace of the conveyor frame. The frame cut his hand behind his right thumb, requiring six stitches.

EE was swinging a 10 lb. sledge hammer to tighten a lock nut on a crusher. After about 5-6 swings, he felt a sharp pain in his right lower back that began to radiate to his right leg. He went to the doctor and he had pinched his sciatic nerve. The doctor prescribed steroids and wrote him out of work for the next two days, thereby making the accident reportable.

The EE was stepping down out of the scale area (one step above the rest of the office). She stepped down with her right foot, and as her foot touch the floor, her ankle turned over and she heard a "pop." She was taken to Orthopaedist where it was X-Rayed and a fracture to the right Fibula was determined.

The EE was replacing poly screens. He was attempting to remove some of the pins. He swung the hammer, hit a piece of angle iron, the hammer bounced over and hit his left index finger causing a fracture. The EE's finger was bandaged with a splint, but is still able to perform his normal job (control room operator) with no restrictions.

On 3/5/2014, the EE was attempting to pull two 5-gallon buckets apart when he felt a sharp pain in his lower back, hip, side and leg. In the last couple of days, the EE felt a bulge in his right lower abdomen, with some pain. He was sent to the doctor and diagnosed with a hernia and placed on restrictions on 4/24/2014, thereby making it reportable.

EE was hooking up a welding lead on the welding truck. He looked down to step off of the concrete where his truck was parked, and then looked up as he was stepping. As he looked up, he ran into a piece of angle iron that was placed on the truck that they planned to use on the job they were working on. The angle iron hit him on the left side of his face, requiring 8 stitches.

EE was using pry bar to wedge gate into a diverter box. When the gate fell into place, the employee lost balance and caught left hand between gate and pry bar, fracturing his 3rd and 4th fingers on left hand.

EE was helping raise up guard rails on scales at the load out. One EE picked up their end of pipe and when the other EE picked up their end, it slipped out of their hands. The pipe fell and struck the EE in the right knee, requiring 8 stitches.

EE was helping place bracing on screen tower. The EE was holding the bracing against the screen tower, when the bracing came loose, swung out and struck his left hand.

While cutting a plug out of cylinder for an excavator, the knife slipped off the plug, cutting the left hand of the employee.

EMPLOYEE WAS TAKING A SAMPLE AND FELT SOMETHING POP IN HER RIGHT SHOULDER

EE WAS WASHING OFF A TAIL PULLEY WHEN A ROCK FELLOFF THE BELT, STRIKING THE INDEX FINGER ON HIS LEFT HAND. HE WAS GIVEN A SHOT TO NUMB THE FINGER WHEN THE DR PUSHED THE NAIL BACK INTO PLACE. NO BROKEN BONES OR STITCHES

EE WAS TAKING BOLTS OUT OF A SCALPING SCREEN. EE WAS PULLING SLACK OUT OF HOSE WHEN THE TORCH FLIPPED OVER, BURNING HIS RIGHT HAND.

EE WAS INSTALLING RETURN ROLLER ON BELT, PUSHING UPWARD WITH LEFT SHOULDER & FELT A POP IN SHOULDER. WENT TO DR & GIVEN SOMETHING FOR PAIN.EE RET'D TO DR 12/27 WHEN PAIN RETURNED. THE ACCIDENT BECAME REPORTABLE AT THAT TIME.

EE WAS CLIMBING OF A LOADER AND STEPPED ON A ROCK, CAUSING PAIN IN HIS BACK

EE WAS USING A COME-A-LONG (CHAIN HOIST) TO MOVE A FENDER INTO POSITION WHEN CHAIN HOIST SLIPPED AND STRUCK HIM IN THE HEAD, REQUIRING STITCHES. NOTE: NO LOST TIME - EE'S MOTHER DIED ON THE 17 TH AND HE WAS OFF THE 18TH & 19TH.

EE WAS ONE OF 4 EE'S LIFTING AN EXHAUST FAN WHEN ANOTHER EE LET GO SHIFTING THE WEIGHT CAUSING THE INJURY TO EE AND HE FELL TO HIS KNEES. NOTE:EE DID NOT SEEK MEDICAL AID UNTIL FEB. 20TH.

EE WAS WELDING ON A BUCKET. HE WAS IN A SITTING POSITION AND REACHED BEHIND AND PICKED UP A PIECE OF STEEL. HE FELT A POP IN HIS BACK.

I WAS REPAIRING SHOP LIGHTS. I WAS ON A MANLIFT ABOUT 13'-15'HIGH. I NEEDED TO RAISE THE LIFT ABOUT A FOOT TO GET TO A JUNCTION BOX. WHEN I STARTED TO RAISE THE LIFT IF SUDDENLY TILTED ON ONE END AND I FELL TO THE FLOOR WITH THE LIFT. I RECEIVED FACIAL CUTS & BRUISES, 2 BROKEN RIBS, A BRUISED LIVER, A BRUISED HIP, A BRUISED RT. CALF MUSCLE, AND MY LT. HAND WAS BRUISED.

EE DIAGNOSED WITH CARPAL TUNNEL - 5-7-02.

EE WAS WASHING OUT UNDER THE PLANT WHEN HE PASSED OUT AND FELL, STRIKING HIS HEAD. HE RECEIVED A CUT OVER HIS RIGHT EYE REQUIRING 3 STITCHES.

EE WAS STEAMCLEANING A PICKUP TRUCK WHEN HE ACCIDENTALLY SPRAYED STEAM DOWN HIS LEFT BOOT. NOTE: EE DID NOT SEEK MEDICAL AID UNTIL THE NEXT WEEK WHEN IT WAS DETERMINED HE SUFFERED 2ND AND POSS IBLY 3RD DEGREE BURNS.

EE WAS WELDING IN THE BACK OF A EUCLID TRUCK AT THE SHOP. HE BECAME PALE, SHAKEY, AND SLUMPED OVER. HE WAS TAKEN TO A HOSPITAL.

EE WAS WASHING OUT UNDER A CONVEYOR WHILE KNEELING. WHEN HE STOOD UP HE FELT A PAIN IN HIS BACK.EE DID NOT SEEK MEDICAL ATTENTION BUT DID NOT COME BACK TO WORK ON JAN. 1 (NOT A HOLIDAY FOR US) .

EMPLOYEE SIPPED ON ICE AND CATWALK OF EUCLID HAUL TRUCK. HE GRABBED THE HANDRAIL AND PULLED MUSCLE IN HIS BACK. NOTE: EMPLOYEE HAD PHYSICAL THERAPY SEVERAL WEEKS LATER. THIS WAS NOT KNOWN UNTIL RECORDS WERE CHECKED WHEN THE EMPLOYEE SUFFERED ANOTHER ACCIDENT THIS YEAR.

EE SLIPPED ON TO THE ACCESS LADDER ON A HAUL TRUCK AND FELL TO THE GROUND INJURING HIS BACK. EE WAS UNABLE TO WORK ON 12-6 AND BECAME LOST TIME AND A MEDICAL CASE.

EMPLOYEE WAS REMOVING A TOOTH FROM A LOADER BUCKET. AS HE HIT THE SHANK WITH A HAMMER, A SPLINTER OF METAL CAME OFF THE TOOTH, STRIKING EMPLOYEEIN LEFT LEG. (NOTE: STITCHES WERE ADDED IN A FOL LOW-UP EXAM THE NEXT WEEK).

EE WORKING ON SCREEN ON THE SCALPING SCREEN: HE UNHOOKED HIS SAFETY LINE TO REPOSITION HIMSELF, WHEN HE SLIPPED, SLID OFF THE SCREEN & FELL APPROX 5 FT INTO ANOTHER CHUTE

EE WAS TAKING A LOAD OF ABC TO THE THE PIT FLOORWITH A CAT 980C LOADER WHEN THE BRAKES FAILED. EE JUMPED OUT AND LANDED ON HIS LEFT LEG BREAKING THE BONE(FEMUR). THE LOADER ROLLED ABOUT 40 F EET AND STOPPED AT A BERM. THE BUCKET WAS NOT LOWERED AND THE PARK/EMERGENCY BRAKE WAS NOT ACTIVATED.

EE WAS WORKING ON A SHORT HEAD CRUSHER LOOSENING A VALVE. WHEN IT CAME LOOSE IT DROPPED HITTING THE RING FINGER OF HIS RIGHT HAND. CUT REQUIRED 4 STITCHES. X-RAYS SHOWED NO BROKEN BONES.

2 EMPLOYEE WERE LIFTING THE ROOFF OF A SMALL SHED (100 LBS) WHEN IT COLLAPSED CAUSING A TWISTING MOTION TO ONE EE. HE FELT A PAIN IN HIS RIGHT GROIN. IT GRADUALLY WORSENED. HE SAW DR. 1/10/00. DETERMINED TO HAVE A HERNIA. NOTE: EE RETURNED TO HIS REGULAR JOB BUT IS SCHEDULED TO HAVE SURGERY LATER THIS MONTH.

EE WAS TRANSFERRING OIL FROM A BARREL INTO A BUCKET WEHN HE BARREL SLIPPED CAUSING HIM TO STRAINHIS LEFT ARM FROM THE ELBOW TOTHE SHOULDER

EMPLOYEE WAS USING A SLEDGE HAMMER TO KNOCK A PIN INTO A 245 SHOVEL. A PIECE OF METAL BROKE OFF AND ENTERED CALF OF LEFT LEG.

THE EE JAMMED HIS BACK WHILE OPERATING A 980C CAT LOADER. NOTE' THE EE TOLD HIS SUPERVISOR HIS BACK HURT AND WENT TO HIS DR THE NEXT DAY. IT WAS NOT UNTIL 8-10-98 THE EE GAVE NOTICE OF AN INJU RY ON THE JOB.

AS EE WAS MOUNTING THE VOLVO LOADER HIS RIGHT FOOT SLIPPED OFF THE BOTTOM STEP ACUSING HIM TO DROP BACK TO THE GROUND ON HIS RIGHT FOOT.

PLACING CHEEK PLATES IN JAW CRUSHER AND ONE PLATE HE WAS HOLDING SLIPPED AND CUT LEFT PALM.

EE WAS STANDING IN SHOP DURING LUNCH BREAK TALKING TO WELDER WHO WAS WELDING ON 245 CAT. SHOVEL TRACKS. HE WAS RESTING HIS RIGHT FOOT ON TOP OF TRACK APPROX. 4" HIGH. AS HE WAS SWITCHING FEET TO REST ON TRACK, HE FELL AND DISLOCATED HIS RIGGHT KNEE.

EE PICKED UP THE BIT END OF A BROKEN DRILL STEEL AND THEN COMPLAINED OF A BURNING IN HIS STOMACH.

WHILE HELPING DRILLER PLACE STEEL IN RACK, EMPL CAUGHT THUMB IN ROD CHANGER RACK AS THE CDRILLER WAS MOVING STEEL WITH THE ROD CHANGER.

FORKS ON FORK LIFT HUNG WHILE LOWERING. EE WAS EXAMINING PROBLEM WHEN THE FORKS DROPPED AND INJURED HIS FINGER. HE HAD PLACED HIS HAND IN AN AREA PLATES CAME TOGETHER.

CUT HAND WHEN KNIFE SLIPPED WHILE CUTTING TAPE OFF CONNECTIONS ON A FEEDER MOTOR AT THE JAW CRUSHER.

EE WAS OPERATING A FRONT END LOADER, PLACING STEMMING AT A 5 HOT SITE. EE PLACED HIS HAND ON THE OUTSIDE OF CAB TO LEAN OUT TO INSPECT TERRAIN. AS THE LOADER MAILED INTO UNEVEN GROUND THE DOO R SLAMMED SHUT ON EE'S LEFT THUMB.

EMPLOYEE WAS ATTEMPTING TO BEND A FLANGE BY STRIKING IT WITH A HAND SLEDGE HAMMER THE METAL BOUNCED UP & STRUCK THE EMPLOYEE ON THE RT HAND

INJURED EMPLOYEE WAS HOLDING A TRACK LINK FOR A 245 HOE WHILE ANOTHER EMPLOYEE WAS DRIVING THE CONNECTION PIN. A SLIVER OF STEEL BROKE FROM THE PIN, STRIKING INJURED IN THE THROAT, SEVERING TH E JUGULAR VEIN AND CAUSING SEVERE LACERATION. WILL BE LOST TIME.

EMPLOYEE WAS HEATING TRACK RAILS WITH A TORCH ANOTHER EMPLOYEE WAS DRIVING TRACK PIN INTO PLACE WITH SLEDGE HAMMER A PIECE OF METAL BROKE OFF THE PIN & HIT EMPLOYEE IN LEFT UPPER ARM IT WAS A NEW PIN

EMPLOYEE WAS CUTTING COLLECT OUT OF A 988 LOADER BUCKET WHEN TRAPPED GREAST FLARED UP BLOWING GREASE UNDER GOGGLES & INTO EYES ALSO MINOR BURNS IN FACE AREA NOTE REPORTED UPON KNOWLEDGE OF PRE SCRIPTION MEDICATION PURCHASED

EMPLOYEE WAS HELPING UNPLUG A CHUTE AND GOT DUST IN HIS EYE.HE DOES NOT KNOW EXACTLY WHEN IT HAPPENED.NOTE:NOTHING WAS IMBEDDED IN THE EYE.EMPLOYEE DID NOT FILL A PRESCRIPTION UNTIL MARCH AND DID NOT IMMEDIATELY NOTIFY THE COMPANY.REPORTED UPON KNOWLEDGE OF PRESCRIPTION.

EMPLOYEE WAS WELDING AND RECEIVED WHAT HE THOUGHT WAS AN EYE BURN. LATER VISIT TO DOCTOR UNCOVERED PIECE OF SLAG IN HIS EYE. REPORTED THIS DATE WHEN FOUND OUT PESCRIPTION MEDICATION PRESCRIBED AND TAKEN.

EE WAS LIFTING HEAVY ROCKS OUT OF A CHOKED CRUSHER WHEN HE FELT A PAIN IN THE GROIN AREA, CAUSING HIM TO DROP THE ROCK ON ANOTHER EE HAND TWO INJURIES.

ROCK FELL ON EE S HAND WHEN DROPPED BY A SECOND EE SUFFERING A HERNIA REQUIRED 6 STITCHES ON ONE FINGER.

WHILE USING A SLEDGE HAMMER AND A HAMMER WRENCH TO TIGHTEN BOLTS ON JAW CRUSHER A CHIP OF THE HAMMER WRENCH CAME OFF HITTING EE IN UPPER LEFT THIGH OBJECT WAS EMBEDDED.

SMALL METAL PARTICAL BLEW IN EMPLS EYE WHILE WELDING OBJECT WAS EMBEDDED REMOVED WITH MAGNET.

EMPL WAS WELDING WHEN PIECE OF HOT SLAG BURNT HOLE IN PANTS LEG & FELL INTO BOOT. EMPL WAS WEARING LIFE LINE AT TIME & HAD DIFFICULTY GETTING BOOT OFF. NOTE: EMPL SUFFERED UNRELATED CHEST PAIN S NEXT DAY & MISSED 1 WEEK OF WORK. LOST TIME NOT DUE TO BURN INJURY.

EMPLOYEE WAS CLEANING OVER LOADER CONVEYOR. AS HE STEPPED OFF HE LOST HIS FOOTING AND FELL. EMPLOYEE DID NOT SEEK MEDICAL AID UNTIL 1-15-90.

WALKING IN THE CATWALK OF 61 B CRANE WHEN A WOODEN BOARD GAVE WAY CAUSING L LEG AND KNEE TO FALL THROUGH OPENING EMP FILLED PAIN PRESCRIPT

DRILLING IN LARGE ROCK WHEN THE DRILL STEEL STUCK WHILE ATTEMPT TO REMOVE STEEL HAMMER TWISTED THROWING EMP TO GROUND. ACC REPORTED AT THIS DATE WHEN EMP FILLED MED PRECRIPT

WHILE LOADING TRUCKS WITH THE SHOVEL SEVERAL LARGE ROCKS BROKE LOOSE ROLLING DOWN THE PILE.THE OPERATOR CAUGHT ONE WITH THE SHOVEL BUCKET.A SECOND ROCK ROLLED INTO THE CAB KNOCKING IT TO THE P IT FLOOR.THE FIRST ROCK PREVENTED THE OPERATOR FROM BACKING AWAY FROM THE SECOND ROCK.

EMPL STEPPED DOWN FROM 988 TIRE TO A FRAME LADDER & FELT SHARP PAIN IN BACK. REINJURY TO PREVIOUS BACK PROBLEM LAST YEAR. THIS ACCIDENT REPORTED LATE. THERE WAS SOME QUESTION EMPL WAS HURT SKY ING DAY BEFORE NOT ABLE TO CONFIRM.

EMPLOYEE WAS DRIVING A HAUL TRUCK WHEN THE RIGHT FRONT "A" FRAME BROKE, THROWING HIM AGAINST THE STEERING WHEEL.

EMPL CUTTING CLIP OFF CABLE SPOOL.CABLE WAS WOUND UNDE PRESSURE AND WHEN CABLE WAS CUT,IT UNWOUND SUDDENLY STRIKING EMPL ABOVE RIGHT EYE.

EMPLOYEE WAS HAMMERING OLD TEETH OF A BUCKET.THE HAMMER GLANCED OFF CAUSING EMPLOYEE TO TWIST AROUND.

EMPLOYEE WAS PULLING ON A STUCK GATE WHENHE FELT A PULLING SENSATION & PAIN IN HIS LEFT LOWER ABDOMAN
