EE was walking around a screening tower and said EE tripped over EE's own feet falling to the ground. EE reported this two weeks later stating EE's knee had swelling and discomfort. EE was treated, has a follow up and restrictions until then making this MSHA reportable. Ee was terminated on 09/24/2025 as a result of a separate company safety infraction.

EE was climbing into a manlift and got tangled up in the twin leg lanyard causing EE to fall and sprained right ankle in the process. EE was treated and written out of work the following day making this MSHA reportable.

EE was using a pry bar to unclog rip rap that was hung in a chute. When the rock came loose it fell onto the EE's hand smashing their left index finger. The EE was treated and received 3 sutures in the finger making this reportable to MSHA.

EE was attempting to put a sheave on a pulley shaft that had been cut, the EE cut EE's left middle finger on a bur on the shaft. EE was taken to the urgent care where EE received stitches in EE's finger, making this reportable to MSHA.
EE was loading a hydraulic cylinder and was holding by the head cap, when EE dropped the cylinder to the floor the barrel collapsed mashing EE's fingers in between the eye and head cap. This resulted in broken fingers on both hands. EE has splints on 3 different fingers making this accident reportable.
EE was walking across flat ground after a rain event and slipped in mud and fell landing on EE's left shoulder. EE was still hurting and went to get checked out and was released to light duty until on 3/20/23 until EE has surgery. EE's job duty has changed, and EE will miss time for recovery making this accident reportable.

EE was in the manlift helping to repair a guard. The manlift basket got caught on a piece of metal and then jolted loose. When it came loose, the EE fell against the handrail, hit lower back and pinched EE's sciatic nerve. The doctor placed EE on light duty.
EE was assisting forklift to turnover cheekplate. The forklift operator got the ckeekplate picked up on one side to about 85 degrees. The EE began to push on the cheekplate. The forks lost tension under the plate, popped up, knocked the EE's hand loose from the plate, and the plate fell back on the EE's left, lower leg causing a fracture of the two bones.

EE was lifting a bucket of ABC with EE's right hand and placed it on a hand truck. EE felt a sharp pain in EE's sternum. This was on Friday. On Monday, it was not any better, so EE went to the doctor and was diagnosed with a muscle strain and placed on light duty, thus making the incident reportable.
The EE was working on Tower No. 1. The chute over the scalping screen stopped up and spilled rock out of the chute. The EE saw the material begin to fall, and a 6" rock hit EE in the back causing a Contusion. Doctor put EE on restricted duty for one week, thus causing the accident to be reportable.
Employee was positioning HDPE pipe in fusion machine. 300' length of 18" pipe was suspended from a trackhoe by a chain & being aligned by a rough terrain forklift. Employee attempted to make a final adjustment in the pipe alignment & placed hand between the pipes. At that time the pipe shifted in the chain & entrapped the employee's fingers. Gloves were being worn.

Standing on the trucks ladder, slipped, hit chest on the ladder, contusion right front wall of thorax
The EE was moving sand with the 988 loader when the joystick locked into the autoraise position with a full bucket of sand. Due to the incline of the pile, this raised the counter weight and rear wheels. The operator kicked the loader out and this slammed the bucket to the ground. This threw the operator into the ROPS and caused a 3" gash in head and a concussion.
EE was putting a boom/stick pin in the long reach trackhoe. EE bent over to line up the hole and when EE raised back up, EE felt something "snap" in the lower part of EE's back. Ended up just being spasms resulting in restricted duty causing the incident to be reportable.

The EE was working on a haul truck from a step ladder. EE stepped from the ladder with left foot and left the right foot on the rung of the ladder. This twisted EE's left knee and EE heard a pop. EE was unable to put any weight on it. EE was diagnosed with a sprained knee and was put on light duty until further checkups.
EE was working on a skid steer loader. Upon exiting the loader, stepped down to the concrete floor and left knee twisted, causing fall to the floor. The EE went to the doctor and was placed on light duty for a sprain. Employee has a follow-up appointment on 3/26/18, where they should be released to full duty.

The EE was operating a R-65 Haul truck and smelled rotten eggs, so rolled up the windows. A little later the battery exploded and ruptured EE's right ear drum. Upon further investigation, the alternator was over charging the battery which caused the explosion. The EE was restricted for five days.
The EE was cutting a section off of a conveyor belt using a razor knife. The knife slipped, cutting the EE on the Left Wrist. No Stitches were required, but on 9/16/16, the EE went back to the doctor for a follow up and they prescribed antibiotics for fear of infection, thus making the accident reportable.
EE was attempting to put a pin back into a Euclid R-90 Haul Truck. Employee was using air tools. During the process, got something in left eye. The next day employee went to the doctor and they had to remove a piece of metal from eye. Employee was wearing safety glasses at the time of the accident.
There was a trespasser on the border of our property who jumped off an elevated surface, head first into the Swannanoa River. The trespasser hit head on the river bottom and was taken away by paramedics. We do not know the extent of injuries due to privacy laws. EMS told us that injured was coherent and answering questions on way to hospital, so we did not immediately report.

EE was welding in a chute. He finished with one welding rod and attempted to throw it onto a shelf close to him. When he threw it, it bounced off a plate and came back into his left shoe burning his leg. Accident occurred on 7/11/13, but EE did not feel need to go to doctor until 7/23/13, which is when we got diagnosis of 3rd degree burns, making it reportable.

The contract employee was notching a board that he was replacing on the office wall. As he was cutting the board, the knife slipped off of the board and hit his right thumb. The employee went to urgent care and the laceration required 5 stitches, thereby making the accident reportable. Name of contractor is Osada Construction
EE was helping to install new guard on a conveyor drive. While in the seated position, the EE felt a pain in his groin area. He stood up and felt a knot in his right groin area. Went to the doctor and had a hernia. The doctor restricted activity for 10 days to no lifting over 10 lbs.

EE was changing a hydraulic hose on a rock breaker. He was pulling with one wrench and pushing with another. When he broke the connection loose, the wrench came back and hit him in the mouth. He bruised his lip and chipped his front tooth

EE was cleaning up on Scalping tower when a rock came off screen and struck EE on Back. Doctor said the EE's back was bruised. Came back to work. On 4/21/09, EE went back to doctor for checkup and doctor scheduled physical therapy for bruise. Started physical therapy (ten sessions) on 4/27/09, and accident became reportable on 4/27/2009.

EE claims that he finished his shift, got down off of his haul truck and was walking to the shop to put his keys away when he must have stepped on a round rock and twisted his ankle. Reported it to supervisor two days later. Went to the doctor two days after that and doctor took out of work due to sprained ankle.

The EE was guiding a euclid onto a low boy, when he slipped and fell on the trailer rail. At the time the EE said he was fine. Since, he has come back and said that he has had problems with his back. After going to the doctor, the doctor said he needed surgery. The accident became reportable on March 10, 2008 after having surgery and becoming lost time.
The employee was cleaning out a clogged chute leading off the primary crusher. Once unclogged, the employee walked away from the chute and rock fell from the top of the chute, striking the employee's left hand.

Employee was closing the tailgate on a company pick up truck when he caught his right hand index finger between the right side of the gate and side of the bed.
EE WAS CUTTING A PLASTIC TIE WITH A POCKET KNIFE. THE KNIFE SLIPPED WHEN THE TIE WAS CUT THROUGH, CUTTING EE'S RIGHT INDEX FINGER.
EE was unloading a d-7 dozer at the quarry. He was reconnecting the angle arm on the blade prior to unloaidng and felt a burning in his lower back.

EMPLOYEE WAS DRIVING DOWN A RAMP FROM A BENCH, THE ENGINGE QUIT RESULTING NO STEERING, EMPLOYEE JUMPED OUT WHILE TRUCK WENT THROUGH BERM AND TURNED OVER ON ITS SIDE OFF A 20 FT EMBANKMENT. EM PLOYEE SUFFERED CUTS AND BRUISES FROM CONTACT WITH GROUND AFTER JUMPING FROM TRUCK.
VICTIM WAS REMOVING MAIN BEARING CAP FROM A 8V71 DETROIT ENGINE WHEN IT SUDDENLY CAME LOOSE CAUSING THE VICTIM TO SCRAPER HIS FINGER ACROSS A BURRED EDGE OF THE ENGINE BLOCK - THIS PROCESS IS DONE BY HAND-THIS TOOK 7 STITCHES IN RIGHT INDEX FINGER.

EMPLOYEE WAS HELPING TO DISASSEMBLE A FRONTEND LOADER FOR TRANSPORTATION. THIS REQUIRED BLOCKING THE MACHINE WITH CROSSTIES FOR SUPPORT AND PARTIAL DISASSEMBLY (REMOVAL OF SOCKET AND TIRES). B ACK WAS ALLEGEDLY STRAINED DURING THE AFTERNOON'S ACTIVITIES TO ACCOMPLISH THE TASK.
