While attempting to close roll top barge lids employees boot caught on edge of barge causing EE to lose balance and stumble, where EE twisted EE's right ankle

Employee was climbing into the roofbolter. EE grabbed the handle with the left hand and pulled themselves up when a sharp pain developed between the left elbow and left shoulder. EE put on restricted duty.

Employee was connecting a hose to the top of a pneumatic railcar prior to loading. While moving the vent hose employee lost balance and fell forward. Employee reached out to catch themselves with a hand and felt a pain in shoulder. Pain did not subside over time and was diagnosed via MRI that was confirmed by an orthopedic doctor and was given a cortisone injection on 3/16/23.

Employee was retrieving a 5 gallon bucket of stone from an automatic sampler and while attempting to pick it up, felt a pain in EE's back. Employee initially reported it but did not want immediate medical attention. Showing no improvement employee returned to Occ. Med on 9/20/22 and has been subscribed further treatment for physical therapy.

Employee was suffering from pain in EE's right inner elbow while working. Employee was sent to local Occupational Medicine office for evaluation. A nerve conduction test was ordered and was confirmed by an Orthopedic surgeon on 1/20/22

Employee has pain on both hands / wrists. Believed to be repetitive motion illness. Doctor confirmed on 10/15/2020 Carpal Tunnel in both wrists. Surgery may be scheduled at a later date. No restrictions / no lost time at this time.

Repetitive Motion illness associated with the use of hand tools. Nerve Conduction testing was positive for Carpal Tunnel. Confirmed by Orthopedic Surgeon on 7/2/2020. Employee has chosen not to complete surgery at this time, but may at a later date.

Employee was complaining of pain in both hands. Believes it to be from repetitive motion. The employee had nerve conduction testing and it was confirmed on 3/12/2020 by Orthopedic Specialist that it was carpal tunnel repetitive motion illness. Employee is NOT restricted or missed time, but there will be surgery scheduled at a later date which will include restricted work.

Employee was climbing a fixed metal ladder approximately 8' to reach an access platform. On about the 3rd or 4th rung the employee felt a "pop" in left knee. There was no pain, but the next day it felt sore and swollen. After about a week without improvement the employee sought medical attention. An MRI was able to confirm on 4/5/19 that there was a torn meniscus.

Employee has pain in both hands. Believes it is repetitive motion related. The employee was sent for evaluation and further testing. On 2/11/19 an orthopedic specialist concluded that surgery would be necessary and put the employee on restricted work at that time pending scheduling of surgery.

While attempting to remove a belt scraper, the opposite end slid out of the housing, causing the scraper shaft to tilt on the end the employee was working on. The employees thumb was smashed between the scraper shaft and the housing for the conveyor head pulley.

Ee was changing a return idler on a conveyor belt and felt a pain in back. Employee was sent to Medical for evaluation and an X-ray. The X-ray was negative. After further issues with pain, employee returned to medical and MRI. The employee was sent to orthopedic specialist on 10/31/18 and was diagnosed with a damaged disc, further medical treatment was ordered.

Cleaning chute for Hydrator #1, the employee was wearing a PAPR (respirator hood). The employee believed employee was done cleaning and had opened the hood shield to clean fogged glasses, when material fell in the chute getting hydrated lime in eyes.

Employee stated was clearing the pots on the Anfo Rig when EE heard an air leak. While using hand to find the leak, a hose blew off causing Anfo to strike EE's hand.

Employee was gouging anchor in the pre-heater. The air coming out of the gouger hit a small pocket of material dust, which caused the dust to rise up under EE's welding hood and safety glasses. Employee was wearing all of the proper PPE for the task.

The employee was operating a man lift when ee struck a protruding roof bolt pinching finger between the roof bolt and the hand rail.

Employee experiencing pain in shoulder, suspects it to be repetitive motion illness.

The employee was uprighting a manlift basket, lost footing, fell and struck left ear on the side of the basket.

Operator of scissor style man-lift was performing cleaning operations in kiln area. Employee pinched hand between handrail of man-lift and a protruding 4" trolley beam for hoists.

Cleaning out a slide gate on Lime Kiln Dust bin, when dust from above came loose and fell onto employee

Employee was welding inside a steel chute and a hot piece of welding slag burnt through EE's pant leg and rolled down in EE's left boot and burned the top of EE's left foot in two places.

While operating the Brokk in the kiln a hose came loose. While ee was repairing the hose the tear out crew was removing the debris from the kiln causing vibration. A couple of brick from overhead fell striking ee on the hardhat bounced and hit EE's finger that was resting on the Brokk.

Wet and slippery conditions on floor, caused a slip. Employee fell and hurt back

Slipped while exiting forklift, fell on right shoulder. Dislocated shoulder

Employee was removing a u-bolt from inside the cooler. Lime fell from overhead and got into his eyes. Employee was hard hat and safety glasses. Eye doctor issued prescription drops.

Employee was removing a cooler grates and smashed his pinky finger. Finger tip was broken and required stitches.

Employee was working inside a cooler at the lime operation. Lime from overhead got into his eye (employee was wearing hard hat and safety glasses). After flushing the eye he was taken to eye dr who prescribed prescription antibiotic.

Attempting to remove plugged material from chute.

While attempting to remove a flare used for lighting the gas lance for kiln start-up the flare broke causing chemical burns to three fingers.

Unplanned ignition within the kiln. This was immediately reported to MSHA at 0345 on 10/23/14. MSHA mine inspector was dispatched.

Quicklime Dust (Calcium Oxide) got into left eye while working on a screw conveyor.

Pulling Stone Pins from Preheater stone chutes and left elbow pain flaired up from previous repetitive motion illness. Employee saw Occupational Medicine on 5/5/14 received initial medical review. 5/20/14 due to recurring pain employee returned to Occupational Medicine and received a prescription for an anti-inflammatory, making this an MSHA reportable incident.

Employee was demoing an old bullnose setion inside preheater. As the piece was cut it fell, and one of the anchors caught his left knee and gouged/cut him. Employee thought that bullnose section still had one gusset attached when he torched the bullnose piece.

While pulling cable through conduit a small piece of fibrous metal went past safety glasses and irritated eye.

Material was hung and not flowing from bin 4139 down to the rotary valve 4141 in the hydrator building. Employee was hammering on chute with hammer and inspection door opened allowing hydrate to flow out and strike employee in face. Lime in the eyes, received prescription.

electrician was installing conduit on a unistrut rack when a piece of conduit started to fall, the electrician went to his knees to catch conduit landing on his left knee, it has been sore since

Closing/latching #1 barge lid on the Memco Barge 5186. Latch was struck and would not close. While putting pressure on latch, it gave way smashing finger between latch and lid of barge.

Employee had a previous injury resulting in pins/plate in his leg. Employee was moving sheets on the ground in preparation for tugger and injured his leg from the lifting of the material.

While removing belt scraper on belt conveyor BC-315 had the opposite side of shaft pull free. This caused entire scraper to fall pinching left index finger.

Employee was standing on barge lids on Bunge 323 barge while attempting to move lids with tow boat. Two lids that are supposed to be interlocked and not able to open had a mechanical failure and came apart, causing employee to lose balance and fall onto loaded material.

While working on dust collector, DC450 a gust of wind stirred up dust laying on a beam and blew it toward EE. The result was quicklime particles in the eye. Eye was flushed by Optometrist and antibiotic eye drops prescribed.

EE was inspecting the drive belts and conveyor belt for tension. Conveyor is located in a storage building (250'longX130'wideX68'high). Conveyors runs length of building and has catwalks on both sides. Conveyor is located 50' above ground level. Air temp outside est close to 100 the temp inside building @ conveyor level est 115 +. Was walking on catwalk and passed out.

As climbing down ladder slipped on ladder rung. Caught himself with right hand felt strain in right elbow and shoulder.

Incident report was filled out on 3/8/11 with complaints of pain in elbow from repetitive motion. 6/8/11 employee sought further medical attention with orthopedic specialist and injection of cortizone into elbow to relieve discomfort.

This incident was called into the MSHA 24 hour hotline at 12:20am on 2/13/2011. There was an unplanned ignition of lubricant in the Kiln #1 Bull Gear housing. The fire was first noted at 10:15pm. It was extinguished at 10:35pm but continued to reflash until 10:50pm. MSHA inspector of the Rolla, MO field office was onsite conducting the investigation on 2/16/2011

Walking down slight ramp at bottom of stairs in office building. Slipped on floor and fell.

EE smashed finger between ash rake for kiln and kiln hood door. He received 17 stitches around the right index finger tip.

Twisted knee while working on dust collector DC-450

There was a loss of power at the plant. When power was restored operators in plant noticed that a power pole from the local Electric Utilities Substation to Tower Rock Stone was on fire. This power line goes across our property. Local Fire Dept and power company were called.

At approx. 11:50 two miners working near rail loading facility noticed a private car enter fac. and proceeded to travel between our rail and Burlington Northern main line. Car became stuck on the tracks. Driver got out of the car and off the tracks just before train struck his car.

Pumping water out of permanent barge dock. Pump hose was inside barge with lid/hatch open. Stepped in hole with left leg. Over the next couple of days, experienced pain in right leg from shock/strain. Followed up w/occupational medicine on 2/24/09. MRI ordered, surgery 3/11/09.

While inspecting bull mill pinion shaft the floor grating that he was standing on slipped and shifted causing the employee to stike his back and side on a hand rail.

A fire started inside the gear drive for kiln #2. It was put out with CO2 fire extinguishers but continued to flare up. Fire was out at 1:40 pm but flared up at times after that. MSHA was notified via phone. An MSHA inspector arrived onsite between 3:30-4:00 pm.

After changing a thermocouple on kiln #2 transfer chute and was leaving preheater area, missed 1st step of 8 form transfer chute platform. Hit heel on 2nd or 3rd step, then fell down stairs. Struck arm, missed 1st scheduled shift of work on 6/17/08.

Elbow pain due to repetive motion from water lancing. Restricted work while going through physical therapy and further evaluation.

He slipped and fell forward on a walkway that was very slick from sleet and freezing rain that was falling. He landed on right knee, right elbow and right wrist. Restricted work: must wear splint until medical follow-up.

Stiffnes/soreness in right elbow. Repetitive motion illness as per doctor. Surgery performed on 4/13/07. No restricted work prior to surgery. Estimated 6 - 10 weeks restricted work after surgery.

While performing pms on dc-420 the employee hit his head on the ladder cage preparing to climb the ladder. The employee injured his neck/back.

Lead person/Team Leader for Utility/Clean-up Crew was shoveling and other labor. Both elbows (primarily right) hurt. Employee is being seen by an Orthopedic Specialsit.

While opening lids on a barge the employee attempted to get off the Combing lid and slipped. He landed on the barge deck on his left shoulder. Diagnosed with a strain to the left A-C Joint/ligament.

EE was operating a water lance w/8000 psi, when his left hand slipped off the handle in front of the wand. Cuts on fingers of left hand required 26 stitches to close.

A conveyor belt fire was discovered and extinguished on the morning of 12/29/04. Due to problems with water and freezing conditions, it took approximately 1 to 1 1/2 hours to extinguish. Exact cause is being investigated.

EMPLOYEE WAS LOWERING A SIX INCH VACUUM HOSE THAT CINTAINED KILN FEED ROCK WHEN HE FELT HIS SHOULDER POP. TOO MUCH WEIGHT IN HOSE.

EMPLOYEE WAS PULLING WATER LANCE BY THE WAND AND THE TRIGGER CAUGHT ON AN ACCESS DOOR AND ACTIVATED THE LANCE.. THE LANCE ABRAIDED THE LEG AND CAUSED A CUT WHICH NEEDED THREE STITCHES TO CLOSE . THE EE RETURNED TO FULL DUTY, AND DID NOT MISSWORK.

WAS REAMING OUT HOLES IN A STEEL PLATE, AND THE DRILL BIT CAUGHT IN THE HOLE AND SPUN THE PLATE. THE PLATE STRUCK THE TOP OF THE EMPLOYEE'S LEFT HAND CUTTING THE HAND. THE CUT REQUIRED 5 STITC HES TO CLOSE.

FELT PAIN IN GROIN AREA AFTER USING A PRY BAR TO CLOSE A BARGE LID. WENT TO MD TO HAVE A CHECK FOR PULLED MUSCLE OR HERNIA.

EE FELT PAIN IN LOWER BACK AFTER INSTALLING A RETURN IDLER ON A 36" CONVEYOR BELT. EE THOUGHT THAT HE HURT HIS BACK WHEN LIFTING THE BELT TO INSTALL THE IDLER. EE WAS GIVEN PAINKILLER AND PUT ON LIGHT DUTY FOR ~1 WEEK. MY

WHILE DRILLING A 1/4" HOE THROUGH A PIPE THE DRILL BIT BROKE DOWNWARD FORCE ON THE DRILL CAUSED THE RIGHT HAND TO COME DOWN ON THE DRILL BIT CUTTING THE THUMB ON THE RIGHT HAND. 3 STITCHES WERE REQUIRED TO CLOSE THE WOUND.

WHILE RAKING GRATES, EE PINCHED INDEX FINGER ON LEFT HAND BETWEEN RAKE HANDLE & DUST DUMP HOPPER. THIS TORE THE FLESH OVER THE MIDDLE KNUCKLE. WOUND REQUIRED SALINE WASH & 3 STITCHES TO CLOSE.

HE WAS WORKING ON BOBCAT LOADER. PIECE OF ANGLE WAS BEING USED AS A STOP FOR BUCKET ARM. PART WAS MISSING. AS PRESSURE WAS APPLIED DOWNWARD, ANGLE POPPED OUT OF PLACE HITTING HIS HAND.

WAS CARRYING BOX INTO OFFICE, BRACED BOX AGAINSTWALL, OPENED THE DOOR, CAUGHT THE DOOR WITH HER ARM TO KEEP IT FROM CLOSING & THE DOOR LATCH CUTHER ARM.

WHILE MAKING A REGULAR, INSPECTION. INSPECTOR HAEE SLIPPED AND FELL WHILE WALKING AND COMING IN CONTACT WITH A PATCH OF ICE. EE STATED THAT HE COULDN'T SEE ICE DUE TO BUILDUP ON MATERIAL C OVERING INTO.

RAKING GRATES EMPLOYEE SMASHED HIS INDEX FINGER AGAINST THE HOOD WHILE TRYING TO REACH UP INTO THE UPPER LEVEL OF THE GRATES.

THE ILLNES WAS REPORTED TO THIS OFFICE ON 4-28. THE EMPLOYEE STATED SHE FIRST NOTICED THE PAIN ON 3-27 ON HER LEFT SIDE. SHE STATES SHE HAD SO MUCH PAIN ON HER LEFT SIDE SHE FOUND IT VERY DI FFICULT TO GRASP ITEMS. THE PAIN LESSENED WITH LESS USE. SHE WENT TO THE DR FOR TREATMENT, THINKING SHE HAD PULLED A MUSCLE. THE DR INFORMED HER SHE HAD CARPAL TUNNEL IN HER RIGHT HAND CAUSI

EMPLOYEE WAS WALKING ON KILN FIRING FLOOR WHEN LIME DUST BLEW INTO HIS EYE. SAFETY GLASSES WERE BEING WORN AT THE TIME.

EE WAS RAKING COAL OFF OF BELT BY FLOP GATE WHEN A BIG CHUNK BROKE LOOSE HITTING FLOP GATE CAUSING FLOP GATE TO KICK BACK HITTING EE IN CHIN.

SMASHED LEFT INDEX FINGER BETWEEN BOLT STUB ON LIFT TOP BARGE AND LIFTING CLEVIS. EE WAS CLIMBING UP TO THE ROOF OF THE BARGE VIA THE ACCESS LADDER. HE REACHED UP AND GRABBED THE LADDER, AS HE PUT HIS WEIGHT ON THE CLEVIS AS HE WAS STEPING TO THE TOP OF THE BARGE, THE CLEVISPIVITED-SMASHING HIS FINGER BETWEEN IT AND THE BOLT.

EMPLOYEE STEPPED IN HOLE NEXT TO RAILROAD RAIL. HOLE HAD PREVIOUSLY BEEN COVERED BUT COVER WAS MISSING.

WHILE RAKING THE COOLER GRATES, A BURST OF AIR FROM THE KILN CAUSED A DUST PARTICLE TO GET INTO EE S EYE. PARTICLE HAD TO BE REMOVED FROM EYE BY DR. NO DAMAGE TO EYE.

WHILE PRYING ON DUST COLLECTOR BAG AND CAGE WITH CROWBAR, THE CROW BAR SLIPPED OFF AND IMPACTED EE IN THE STOMACH. RESULT WAS SMALL CUT NEXT TO NAVEL AND BRUISED BOWELL.

EMPLOYEE HIT ACESS DOOR TO BALL MILL WITH HEAD. RESULTED IN CUT BESIDE RIGHT EYE. REQUIRED 5 STITCHES.

WHILE CHECKING KILN COLLER GRATES FOR BUILDUP, DUST WAS BLOWN FROM THE KILN TOWARD THE INJURED EE. SAFETY GLASSES DID NOT ADEQUATELY PROTECT THE EE'S EYE. A DUST PARTICLE EMBEDDED IN LEFT EYE OF EE.

EE WAS REMOVING INSPECTION DOOR TO CHECK FOR PLUGGED CHUTE. WHEN INSPECTION DOOR WAS REMOVED, METAL CHUTE PLATE ABOVE DOOR ALSO SLID DOWN. BOLTSHAD BEEN LEFT OUT OF CHUTE PLATE. EE JUMPED OUT OF WAY RESULTING IN POSSIBLE GROIN MUSCLE PULL.

CLC LOADING TRUCKS IN VERY WINDY WEATHER AND LIME DUST CONTINUED TO BLOW OVER EMPLOYEES. HE WAS WEARING SAFETY GLASSES AND MONO-GOGGLES.

EXPLOSION IN BALL MILL POSSIBLY CAUSED BY HOT EMBER DISLODGED WHILE AIR LANCING TRANSFER CHUTE BUTTERFLY VALVE, LOW BALL IN CHARGE IN MILL, AMBIENT AIR BEING ADDED TO MILL DURING SHUTDOWN. ALS O EXPLOSION IN COAL BAG HOUSE. EQUIP DAMAGE TO AIR DUCT EXP. JOINTS, COAL EXHAUSTER FAN HOUSING,AND BAGHOUSE.

EMPLOYEE WAS WALKING UP STAIRWAY TO TOP OF CHAT BUILDING WHEN WIND JUST BLEW DEBRIS INTO HIS EYE.

EE WAS ASSISTING OTHER EMPLOYEES TO ALIGN A SECTION OF GRIP STRUT INTO PLACE, WHEN SECTION WENT INTO PLACE IT CAUGHT HIS FINGER BETWEEN THE GRIP STRUT AND THE ANKLE IN WHICH IT BELONGED.

EMPLOYEE ALLEGES THAT WHILE HELPING SET A PIECE OF HANDRAIL ON AN INCOMPLETE SECTION OF CONVEYOR THE WIND CAUGHT THE HANDRAIL CAUSING IT TO SPIN. WHEN HE REACHED TO CATCH THE LOAD, HE LOST HIS BALANCE FALL BACKWARD OFF THE OPEN END OF THE CATWALK. HE FELL APPROXIMATELY 17'.

EE WAS CLIMBING OUT OF A 6 X 16 FORM, WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL APPROX 3 FEET ONTO A PIECE OF #5 REBAR, PUNCTURING HIS RT SCROTUM AND ENTERING HIS GROIN APPROX. 2-4 INCHES.

EMP WAS THROWING AWAY A METAL CAN THAT HAD A HOLE PUNCHED IN SIDE WITH RAGGED EDGE ON IT, WHICH CUT EMP INSIDE LEFT FOREARM.

WHILE PREPARING A WELD FOR PAIN EE GOT A SLIVER OF METAL IN HIS LEFT EYE.

EE WAS PUTTING AWAY A WATER FILTER WHEN HE MISSED THE HOOK ON WHICH INT WAS STORED, HE TRIED TO CATCHIT AND CUT HIS HAND ON A HOSE CLAMP.

WHILE UNLOADING A TRUCK, EMPLOYEE SLIPPED. TRYING TO CATCH HIMSELF WITH HIS RIGHT HAND HIS HAND CAUGHT A METAL CLIP TEARING A LARGE PLACE IN HIS HAND.

EE WAS NAILING CHAMPER TO INSIDE OF A SMALL FORM, WHEN HE CAUGHT HIS HAND ON A NAIL ON THE OPPOSITE SIDE OF THE FORM.

ON JULY 10TH EE REPORTED TO WORK WITH COMPLAINT OF LOWER ABDOMINAL AND BACK PAIN MADE DR. APPR FOR 7-12. EE SUGGESTED SEEING A SURGEON POSSIBLE HERNIA SEEN DR WAS DIAGNOSED ABDOMINAL STRAIN ON 7-20. EE STILL HAVING PAIN EVEN WITH LIGHT DUTYTOOK TO ER ROOM DR PRESCRIBED PAIN MED AND LIGHT DUTY.

EMPLOYEE WAS HELPING OTHER EMPLOYEES TO UNLOAD A TRUCK WHEN HE WAS STUNG BY A BEE. EE IS ALLERGIC TO BEE STINGS.

EMPLOYEE ALLEDGES HE WAS BITE ON HIS UPPER LEFT THIGH ON 6/27. BY THE MORNING OF THE 28TH THE AREA OF THE BITE WAS RED AND SWOLLEN. DOCTOR DETERMINED IT WAS A NON-POISONOUS BITE OF SOME KIND.

EE WAS CUTTING OFF EXCESS FLASHING ON HOPPERS. WHEN HE MOVED, HIS LEFT FOREARM WAS CAUGHT BY A PIECE OF FLASHING.

EMPLOYEE CAME TO WORK ON 6/27 WITH AN APPARENT INSECT BITE ON HIS FACE. HE STATED THAT THE PREVIOUS DAY HE HAD A SPIDER IN HIS HOOD AND THOUGHT IT MAY HAVE BIT HIM. AFTER DR. EXAMININATION IT WAS DETERMINED THAT IT WAS IN FACT A SPIDER BITE

EE ALLEGES THAT WHILE MOVING A PIECE OF REBAR, AT WHICH TIME HE WAS STANDING ON A REBAR MAT, HE LOST HIS FOOTING, SHIFTING ALL THE WEIGHT ONTO HIS LEFT SIDE, CAUSING A MUSCLE IN HIS BACK TO PU LL.

INJURED WAS REMOVING THE FLASH GUARD FROM A DISCONNECT THAT HAD BEEN ENERGERIZED BY THE UTILITY COMPANY TO CHECK THE VOLTAGE AS HE WAS LOOSENINGTHE 2ND OF 4 RETAINING SCREWS. THE DISCONNECT EXPLODED IN HIS FACE.

EE WAS LIFTING A CROSS TIE WITH THE ASSISTANCE OF ANOTHER EMPLOYEE WHEN HE PULLED A MUSCLE IN HIS BACK
