The employee alleges that when employee got off employee's heavy equipment, employee stepped on a stone from the internal road, causing pain in right knee.

Employee was cleaning a continuous corridor to a vibrator screen, when an approximately 2-inch stone struck employee's right shoulder, causing a bruise.

While the employee was cutting a strap of employee's truck with a knife to replace an hydraulic hose, the thumb of employee's left hand was cut.

Employee alleges that EE was tightening the oil tube bolts on the 320B excavator bucket. When EE was adjusting one of the Allen screws, the tool came loose causing the ring finger of EE's left hand to hit the metal, causing swelling in the finger.

The employee alleges that while filling the truck's tank with diesel the hose of the air pump that draws diesel from the tank came loose, spreading diesel down the ladder, railings, and floor of the truck. When the employee tried to get down, EE slipped hitting EE's groin, thigh and knee on the right side of the body.

The employee received a small cut in left hand while making a movement of a tire of the backhoe loader. Employee received 5 stitches at a diagnostic and treatment center. Employee will return to work on August 9, 2019, as notified.

The employee indicates a painful back and apparent pinched nerve, allegedly from operating the excavator 320. The employee was referred to Fondo del Seguro de Estado by a private doctor.

The employee was accommodating the Plant Conveyor Toughing of conveyor #6 (with an approximate weight of 10 to 15 lbs.). When he forcefully pushed to accommodate it into place, he felt discomfort in his neck and lower back.

The employee was opening the hatch on the back of the conveyor cover, pricked the thumb of right hand.

The employee was walking in the office area and slipped, ee injured lower back and left hip.

While the employee was taking out the starter of the truck, he made an undue force, causing him to injure his lower back.

Employee of the 600 excavator was referred "Fondo Del Seguro De Estado" for back pain.

The employee going up the stairs of the conveyor belt, the belt slipped and pricking the middle finger of the right hand.

The employee was cleaning with shovel under conveyor, when he felt pain in his back and hip & couldn't move.

EE was said that he was holding a nail with a wrench then he started hammering the wrench it slipped and hit between his left thumb and index finger. It caused a laceration and his hand to swell up.

The employee was giving maintenance to the conveyor; he was standing in the hallway holding the interior part of the handrail of the conveyor. He got dizzy, falling on hallway, hitting his right leg and affecting the muscle of the thigh.

The employee was repairing the primary feeder chain. The chain was twisted. When he tried to straighten the chain, it was loose on the bottom. When it stretched, it moved trapping the little finger of the right hand against the sproket.

Angle iron fell on ground, bounced on rock acting as a see saw, hit person on chin causing cut at chin.

EE WAS WORKING AT MILL #3 FRIDAY 10:30 AM. WHILE HELPING OTHER EE (CONTRACTOR) TO MOVE A MOTOR HIS RT HAND WAS CAUGHT AND SQUEEZED WITH CONSEQUENCES STATED BELOW, BROKEN FINGERS,10 STITCHES. A T TIME HE WAS WEARING SAFETY GLOVES. WAS REFERRED TO SAN CARLOS HOSPITAL, TREATED BY DR (ORTHOPEDICS)

WHILE REPAIRING TIRE OF U-32 [WATER TRUCK], WHILE TAKING BOLTS OF TIRE THE SAME FELL ON HIS RIGHT LEG INJURING HIMSELF.

EE WAS CLEANING #1 MILL AREA. HE LOST HIS BALANCE AND FELL DOWN KNOCKING HIS HEAD IN A METAL BANISTER. THE WOUND NEEDS 7 SURGICAL STITCHES.

SINCE HE IS GIVING MAINTENANCE IN MILL 1 TRANSMISSION CASE HIT HIS HEART FINGER, LEFT HAND, RECEIVING A LITTLE INJURY.

EE OPERATE THE TRACK DRILL MACHINE, WHEN HE GO TO GLIDE & FALL DOWN. HE WOUND HIS LEFT LEG & FOOT.

HE WENT FOR THE FOOT STEP AND TURNED HIS FOOT.

EMPLOYEE WAS OPERATING AN EXCAVATOR (JD-690). HE WAS GETTING OFF THE EQUIPMENT, FELL TO GROUND, TWISTING LEFT ANKLE.

EE MAINTENANCE MAN WAS TRYING TO START THE MOTOR OF THE DIESEL TANKS WHEN HE YANKED THE STARTED CORD HE HIT HIS RT ELBOW ON ANOTHER TANK

HE WAS WORKING NORMALLY AND GAVE A BACKACHE.

HE WAS INSTALLING AN I BEAM ANDHURT HIS RIGHT HAND. WRONG LIFTING.
