EMPLOYEE COMPAINED OF BCK PAIN. SENT EMPLOYEE TO HOSPITAL TO BE EXAMINED, DOCTOR FOUND SOME INFLAMATION IN LOWER BACK.
EMPLOYEE PARKED WELDING TRUCK ON SLIGHT INCLINE, HE EXITED TRUCK TO GUIDE OTHER DRIVER INTO AREA. TRUCK ROLLED BACKWARDES KNOCKING EMPLOYEE DOWN.
EMPLOYEE WAS REPLACING TIRE ON ROLL CRUSHER. TIRE WAS CHAINED AND LOWERED INTO POSITION BY CRANE CHAIN WAS DISCONNECTED FROM TIRE AND HANGING FREE FROM HOOK OF CRANE. CHAIN SOMEHOW DISENGAGED ITSELF FROM CRANE HOOK AND FELL ONTO EMPLOYEE'S KNEE.
