Employee tripped on a stone and landed on back and got cuts to arm. Employee went to the ER and got looked at and got 10 stitches on the arm. Employee was released.

Employee was on the screen tower using a hose to clear a plug up of hard pack on conveyor when his foot slipped on wet surface causing him to lose his balance, but not fall. His left knee was twisted as a result. He was treated for a ligament injury and was given a brace, crutches and work restrictions.

A frozen chunk had plugged up a conveyor on the crusher. The employee used a pinch bar in an attempt to reposition the chunk. The hook at the end of the pinch bar slipped off of the frozen chunk which caused the straight end of the bar to hit the employee in the mouth, chipping two teeth.

Employee was dismounting his loader when he twisted his knee. He was using three points of contact however the step was slippery. The employee was evaluated, treated for a strain to his right lower leg and released to modified duty.

Employee states he was looking for a piece of steel on the steel rack. The area was covered in snow, he plowed and shoveled an area around the rack. When leaving the area, the employee states he caught his right foot on a pallet and fell on his right knee. He fell onto a piece of steel causing a laceration to the right knee.
