INJURED PARTY WAS DRAGGING A TRAILOR WITH THE UNIT SCOOP WHEN IT CAME LOOSE HE STOPPED THE SCOOP AND PUT IT IN NEUTRAL BUT DID NOT TURN THE SCOOP OFF. AS HE WAS GETTING OUT OF THE HE NOTICED T HE SCOOP WAS MOVING, AS HE TURNED AROUND HE SEENTHAT THE SCOOP WAS JUST ABOUT THE HOT THE COAL RIB. INJURED PARTY WAS IN WASHER PLANT FIXING SKIRT RUBBER ON A BELT. AS HE WAS STRIKING THE CHISEL PUNCH WITH A RUBBER MALLOT HE MISSED THE PUNCH &STRUCK HIS RIGHT INDEX FINGER.
INJURED PARTY WAS STRIKING A PIECE OF DRAG CHAINWITH A PIECE OF COAL ROLL WHEN A PIECE OF THE COAL ROLL SPLINTERED OFF STRIKING HIM ON THE LEFT FOREARM.

LEE PERSONNEL CARRIER DOWN THIE MAIN EAST SLOPE WHEN ONE OF THE BRAKE PADS & PAD BRACKETS BROKE COMPLETELY OFF THE BRAKE ASSEMBLY, CAUSING PLUNGER TO COME OUT LEAKING THE BRAKE FLUID, THEREFORE LOSS OF BRAKES. AFTER THIS, INJURED PUT DIESEL RIDE IN REVERSE, WHICH CAUSED ENGINE TO DIE. AT THIS TIME, INJURED PARTIES HIT ARCHES ON MAIN EAST SLOPE.
INJURED PARTIES WERE DRIVING A PERSONNEL CARRIERDOWN THE MAIN EAST SLOPE WHEN ONE OF THE BRAKE PADS BROKE CAUSING THE BRAKES TO FAIL. AFTER THIS HAPPENED THE INJURED PARTIES PUT THE DESSEL RIDE IN REVERSE WHICH CAUSED THE ENGINE TO DIE. AT THIS TIME THE INJURED PARTIES HIT THE ARCHES ON THE MAIN EAST SLOPE.
INJURED PARTY WAS TRYING TO DRAG CABLE OUT OF ROADWAY WHEN HIS FEET SLIPPED OUT FROM UNDER HIM AND HE FELL BACKWARDS.
INJURED PARTY BENT DOWN TO PICK UP THE REMOTE CONTROL BOX ON THE MINER WHEN HE SAID HE FELT A SHARP PAIN IN HIS LOWER BACK.
INJURED PARTY REPORTED WHILE WALKING ACROSS THE SECTION HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL AND PULL SOMETHING IN HIS LOWER BACK.
INJURED PARTY IS A PUMPMAN BY OCCUPATION AND HE WAS LOADING A PUMP INTO THE BACK OF A RIDE WHEN THE PUMP ROLLED OVER AND SMASHED HIS LEFT RING FINGER.
INJURED PARTY HAD BEEN LOADING ROUND PROPS INTO A PROP TRAILOR WHEN ONE OF THE PROPS ROLLED OVER AND CAUGHT HIS MIDDLE FINGER ON LEFT HAND.
INJURED PARTY WAS TALKING BATTERY SCOOP OFF CHARGE ON THE #1 UNIT. WHEN HE BENT OVER AND STARTED PULLING THE PLUG OUT OF THE SCOOP BATTERY HE FELT A PAIN IN HIS LOWER BACK.
INJURED PARTY WAS CLEANING UP A SETUP WITH THE S7S SCOOP WHEN HE RAN OVER SOMETHING IN THE ROAD AND IT THREW HIS LEFT SHOULDER AND HEAD INTO THE ROOF.

THE FALL WAS 30' LONG, 18' WIDE, 8' HIGH. THE SECTION HAD ALREADY MOVED OUT OF THIS AREA. INVESTIGATED BY MSHA 5-1-2000.

THE ROOF FALL WAS 100' LONG, 18' WIDE AND 8' HIGH. MSHA PERSONNEL WERE CALLED AND IT WAS INVESTIGATED.

INJURED PARTY WAS LOADING 5 GAL OIL CANS OFF A WOODEN PALLET ONTO AN OIL TRAILER WHEN THE 5 GAL CAN OF OIL FELL OFF THE PALLET SUDDENLY CAUSING HIM TO PULL A MUSCLE IN HIS RIGHT SHOULDER TRYIN G TO HOLD THE CAN.

INJURED PARTY WAS PICKING UP A WOODEN PALLET ON THE 2-26-98, TURNED TO THROW IT IN FRONT END LOADER, FELT A BURNING SENSATION IN LOWER BACK. THOUGHT IT WAS A PULLED MUSCLE. BECAME REPORTABLE 3 -16-98 TO MSHA.
INJURED PARTY WAS SQUATTNG BESIDE MINER IN THE #8 ENTRY IN THE NEW RET ROOM SET UP WHEN A PIECE OF ROCK FELL OUT BETWEEN PINS STRIKING INJURED PARTY ON RIGHT HEEL. ROCK WAS ABOUT 3"OR 4" THIC K 2'LONG 1' WIDE.
A ROOF FALL WAS DISCOVERED IN THE 1 SOUTH PANEL THE 2ND EAST IN THE 7 ENTERY IN THE PILLAR LINE BETWEEN 7 & 00 AND 7&50 THE FALL WAS ABOUT 27' LONG 18' WIDE AND 26" HIGH. 30" FULLY GROUTED ROO F BOLTS WERE USED IN THIS AREA.
INJURED PARTY WAS INSTALLING ROOF BOLT S IN THE 8 ENTRY OF THE 1 SOUTH PANEL ON THE 1 UNIT, WHEN A PIECE OF SLATE FELL OUT OF ROOF STRIKING THE INJURED PARTY ON THE LEFT HIP AND LEFT ANKLE. TH E PIECE OF SLATE WAS ABOUT 4' WIDE 4' LONG AND 4" THICK.
EE WAS INSTALLING ROOF BOLTS IN THE #1 ENTRY IN THE SOUTH PANEL WHEN A PIECE OF GLOB FELL OUT OF ROOF STRIKING EE ON LEFT HIP AND LEG.
A FALL WAS DISCOVERED BY MINE FOREMAN ON WEEKLY WALK AT X-CUT #31 ON THE 2ND EAST, IN THE #5 ENTRY NEUTRAL RETURN OVER THROUGH #6 ENTRY RETURN INTO #7 ENTRY RETURN AT SPAD #14+85.
A FALL OCCURRED IN THE3RD EAST PANEL OFF THE 1ST SOUTH SUBMAINS IN THE 4 ENTRY AT SPAD 4 & 50 AT AROUND 500 PM THE FALL FELL ON THE FEEDER AND ON SHUTTLE CAR 1 WITH SERIAL 16478 AND LITTLE FEL L ON 3 CAR SERIAL ET15612. ALL PIECE OF EQUIPMENT WERE GOTTEN GROUTED ROOF BOLTS WERE USED IN THE AREA.
FALL WAS DISCOVERED BY 3RD SHIFT MINE FOREMAN AT 500 AM BY 3RD SHIFT MINE FOREMAN. IT WAS IN THE 2 ENTRY IN THE NEUTRAL IN THE 3RD EAST ENTRIES AT SPAD 2 & 50. FALL WAS 20' LONG, 20' WIDE AND 40" HIGH.

ON THE 11-22-97 AT 9AM INJURED PARTY WAS REPLACING A CHUTE OFF THE RAW COAL BREAKER IN THE WASH PLANT. WHEN IT CAM ELOOSE CAUISNG INJURED PARTY TOO SLIP DN FALL OFF A I-BEAM. HE WAS STANDIN G ON. HE INJURED HIS LOWER BACK AND HURT HIS RIGHT LEG BELOW THE KNEE. INJURED PARTY DECIDED NOT TO GO TO THE DR TIL 11-26-97.

A FALL WAS DISCOVERED BY THE DAYSHIFT MIME FOREMAN AROUND 8AM AT THE MAIN EAST RETURN SEALS. THE FALL WAS IN FRONT OF THE #2 SEAL. THE FALL WAS 18'W, 25'L, & HEIGHT WAS UNDETERMINED. FEDERAL & STATE AUTHORITIES WERE NOTIFIED IMMEDIATELY.
WE HAD A ROOF FALL AT 7:45 AM IN THE EAST PANEL OF THE 6TH NORTH PANEL IN THE LAST OPEN CUT. THE FALL WAS OUTBY THE #3 ENTRY ABOUT 30' AND FELL THROUGH THE INTERSECTION OF #6 ENTRY INTO THE IN TERSECTION OF THE #5 ENTRY ABOUT 10'. THE FALL WAS 80' LONG X 18' WIDE X ABOUT 12' TO 40" HIGH. WE HAD JUST MOVED INTO THIS AREA, WE HAD NOT MINED ANY COAL YET.

A FALL WAS DISCOVERED BY THE 3RD SHIFT PRESHIFT EXAMINER AT APP. 10:30 PM. THE FALL WAS DISCOVERED IN THE FACE OF THE #4 ENTRY IN THE 6TH NORTH PANEL OFF THE 2ND MAIN EAST. THE FALL WAS FOUND TO BE 60' LONG, 18' WIDE AND 3 1/2' TO 4' HIGH. THE FALL FELL IN THE #4 ENTRY INTO THE INTERSECTION OF 11+00 SPAD AND THROUGH FOR ABOUT 20'. FALL WAS INVESTIGATED BY M.S.H.A.
INJURED PARTY HAD WENT TO SUPPLY TRAILER TO LOAD UP ROOF BOLTER. HE VENT OVER TO PICK UP SOME PIN PLATES THAT WERE HUNG. HE JERKED AT THEM TRYING TO GET THEM LOOSE WHEN HE FELT HIS BACK PULL.
A ROOF FALL WAS DISCOVERED IN THE 1ST SET OF INTAKE ROOMS OFF THE 5TH NORTH PANEL IN THE 2ND X-CUT BACK FROM THE LAST OPEN. THE FALL WAS 3' TO 4' H, 19' WIDE & 70' LONG WE HAD ALREADY; MINED & HAD MOVED OUT OF THIS AREA.
INJURED PARTY HAD WENT DOWN INTO PIT AREA OF MINE AND PICKED UP A SHEARING JACK FOR A CONTINUOUS MINER. HE BROUGHT IT BACK UP THE HILL AND WAS IN THE PROCESS OF LAYING IT DOWN BEHIND THE SHOP WHEN THE JACK SLIPPED AND CAUGHT HIS MIDDLE FINGER ON HIS RIGHT HAND ON THE END CUTTING IT.
INJURED PARTY WAS LIFTING X-BAR BY HIMSELF TO PUT A PIECE OF WOOD UNDER IT WHEN HE FELT A PULL IN HIS RIGHT HIP. WORKED REST OF SHIFT.
A FALL WAS FOUND ON #1 UNIT, IN THE #6 ENTRY AT THE INTERSECTION OF SPAD #2+50 IN THE 4TH NORTH PANEL OFF THE 2ND MAIN ENTRY. THE LENGTH WAS ABOUT 60', 19', & 2-1/2' HIGH. THERE WAS A SLIP IN THIS AREA. 42" BOLTS HAD BEEN PUT IN THIS AREA 7& IT WAS TIMBERED.
EE WAS PULLING ON A WASH DOWN HOSE WHICH IS USED ON 3RD SHIFT TO WASH EQUIPMENT WHEN HE PULLED HARD AT HOSE & FELT SHOULDER PULL.
A FALL OCCURRED IN THE #1 ENTRY OF THE 3RD NORTH PANEL OFF THE 2ND MAIN EAST AT SPAD #14+00. IT WAS A CONTINUATION OF A FALL THAT FELL THE 1-2-97. THE FALL WAS 180' LONG, 19' WIDE & 2-4' HIGH.

INJURED PARTY HURT LOWER BACK WHILE SHOVELING ON THE 2ND EAST PIGG AND BACK BELT HEADER. INJURED PARTY TWISTED TRUNK WHILE THROWING A SHOVELFULLOF COAL ON BELT. INJURED WENT AHEAD AND CAME TO WORK THE 12-6-96 AND WORKED THE WHOLE SHIFT. DIDNOT GO TO DR. UNTIL 12-8-96. REPORTED TO ME AT 12-9-96 AT 2:30 PM. BY INJURED PARTY.
