Bulldozer operator was preparing a drill bench on top of an approx. 50' highwall. Operator was pushing consolidated material over the edge of the highwall. The bulldozer traveled over the edge of the highwall and fell to the pit bottom. Operator was found unresponsive. 1st aid and CPR were performed until the coroner arrived and pronounced the victim dead at 1404 hours.

Blaster's Helper was driving/operating Blasting Agent Bulk Truck when he missed a gear descending a 14% grade ramp. Driver was unable to stop or slow truck resulting in truck crashing into highwall. Employee suffered contusion to head and upper body.
Employee was cutting a pump hose and cut his right leg.
Blaster was using a man shovel to shovel drill cutting into bore hole when he felt sharp pain from back to both legs. Results was a strained lower back.

Employee was pulling a pump that was submerged underwater. He pulled his elbow and shoulder. He wore a brace but the injury got worse. He went to the doctor on 7/29/13. He reported the injury to the office on 7/30/2013

The employee was in a truck with a crane attached. He was lifting a pump with the crane. The crane fell and jerked the truck up and slammed it down. The employee hurt his back with the impact of the truck when it fell.
EE was working on a dozer with an air wrench. A bolt came loose pinching his middle finger on this right hand.
Employee was setting out pumps. He slipped on a rock and fell.
Employee was getting off of a bulldozer. He twisted his ankle on February 25, 2012. The ankle got worse by Monday February 27. He left work and went to the doctor.

Coal truck driver for Black Gold Trucking had got- ten loaded with coal and was leaving out of the coal yard. He got almost to the gate when he died with a heart attack. The truck rolled back down a slight grade and turned over on its left side. Damage to truck 7000.00 approx & 3000.00 approx. to the dump trailer.

EE WAS HELPING LOAD MATERIAL BEING MOVED FROM THE MANCHESTER MINE TO THE NEW MINE IN TUSCALOOSA COUNTY AL. AN EYE WHICH HAD BEEN WELDED ONTO A PIECE OF STEEL BROKE OFF WHILE BEING LOADED ONTO TRUCK. THIS ALLOWED THE SHEET OF METAL TO SLIDE DOWN HIS STOMACH (SCRATCHING & CUTTING) & HIT HIS LEFT THIGH BRUISING & CUTTING IT
EE WAS FUELING HIS DOZER AFTER COMPLETING HIS SHIFT. WHEN HE FINISHED, HE STEPPED OFF DOZER INTO A HOLE AND TURNED HIS RIGHT ANKLE OVER. HE WENT TO THE EMERGENCY ROOM AT WALKER BAPTIST FOR T REATMENT. WAS TOLD TO PUT ICE PACK ON IT AND ELEVATE FOOT. MEDICATION FOR PAIN WAS PRESCRIBED. HE WAS OFF WORK THE FOLLOWING DYA, CAME BACK 4-20-00, WAS CALLED BACK TO DR'S OFFICE ON 4-24-0

EE AND EE WERE MOVING PUMPS AND DISCHARGE HOSES IN THE PIT AREA DUE TO EXTREME WATER LEVELS. WHEN THEY BEGAN PULLING THE DISCHARGE HOSES, EE GRABBEDHIS LOWER RIGHT SIDE. HE THOUGHT HE HAD ON LY PULLED A MUSCLE AT THE TIME AND DID NOT GO TO DR FOR SEVERAL DAYS. DR DID NOT DECIDE ON SURGERY FOR THE HERNIA, WHICH WAS A RESULT OF PULL ON THE MUSCLES, UNTIL AFTER 1ST OF YEAR. HE LOST
AFTER EATING SUPPER, HE WAS GETTING BACK INTO HIS LOADER AND BUMPED HIS HEAD ON THE CATWALK OF THE ROCK TRUCK PARKED NEXT TO HIS LOADER AND RECEIVED A CUT ON TOP OF HIS HEAD REQUIRING TWO STAP LES TO CLOSE OUT. HE FAILED TO REALIZE THE ROCK TRUCK WAS PARKED AS CLOSE TO HIS LOADER AS IT WAS.
EE CLIMBED TO TOP OF POWER TRUCK WHICH IS EQUIPPED WITH HANDRAILS. HE HAD TO CROSS OVER 3 BARS WHICH OPENED /CROSSED CHUTE ON POWEDER BIN. WHENHE CROSSED OVER THE BARS ONTHE POWDER BIN, HE SLIPPED AND FELL TO THE GROUND

AS HE WAS STEPPING DOWN ONTO TOP OF PUSHARM ON DOZER, HIS RIGHT FOOT SL IPPED OUT FROM UNDER HIM. THE RIGHT SIDE OF HIS RIGHT LEG STRUCK THE PUSHARM AS HE WAS FALLING. HIS BODY THEN FELL ACROSS THE PUSHARM BEFORE HITTING GROUND.
