Employee was swinging a sledge hammer to try and break a gear box off of a shaft.
Employee was checking motor on piece of equipment when the ladder they were standing on kicked out from underneath when they were climbing down ladder. Employee stopped themselves from falling by reaching with right arm.

Employee was installing a jib arm on top of old rotex elevators. Employee was lifting jib arm from waist level to overhead. Employee was using team lift. While lifting employee felt pain in abdomen. Employee is scheduled for surgery on March 5, 2024 to repair hernia. Employee has been working since accident on light duty.
Employee was doing regular job duties, specifically splitting samples and began to feel pain in the right elbow. Employee didn't lose any work time until 5/8/2024. That is the date that EE had MRI and the doctor took employee out of work.

Contractor was using utility knife to cut insulation off a wire, contractor was wearing leather gloves. The knife slipped and cut left index finger.

Employee was changing belts on cooler blower #3 when the screw driver that EE was using slipped off sheave causing EE's finger to get pinched between belt and sheave.
Employee was driving a chisel into a belt pulley to remove said pulley from an air compressor head. Employee swung at and missed end of chisel striking left hand.

Opened hatch door to clean plugged cyclone. Climbed down and cleaned them. Forgot to close hatch door. Came back seconds later and fell through the hole. Note: EE has not lost a day of work yet.
Climbing down old main loading elevator and bumped right elbow. Employee has not lost any work from incident.

Employee was working near dryer burner chamber when he heard cries from inside burner chamber for help. In his attempt to exit the manlift he was working from to assist fellow workers, he fell 4' onto catwalk injuring his left knee.
OUR BRICKLAYER WAS IN A 70'FT. SAND DRYER, ALONG WITH OTHER TRADESMEN FROM OTHER COMPANIES, WHO STARTED THEIR WORK PRIOR TO OUR ARRIVAL. WHILE PERFORMING REPAIRS TO THE FIREBRICK. IN THE COURSE OF WELDING ANCHOR PINS FOR THE BRICKS, AFTER INSTALLING THE THIRD ANCHOR, AN APPARENT NATURAL GAS POCKET IGNITED, BURNING OUR BRICKLAYERS ARM, BACK AND TOP OF HIS HEAD.
