Employee was emptying the belt magnet when a roof bolt was raised up by the magnet striking the employee on the hand.
Contract worker and co-workers were getting slack in order to make a splice in the decline belt. The belt grip came off and flew striking employee in back/rib cage area. Cause of this injury was body positioning and awareness. Compliance with rules/regs, PPE and mining systems not a factor. Miner proficiency may be a factor.

Employee was washing down the 3rd floor of the prep plant with a wash down hose. Employee stepped on a slick spot causing employee to strain right knee. ***Employee did not start missing work until 11/15/2017. ***The WC Claim was deemed compensable on 11/30/2017.

Employee was walking across refuge truck parking lot. Ground was uneven, snow and ice covered. Employee slipped and fell. He landed on a shovel handle that he was carrying.

Employee was trying to flip/roll 6" rubber pump line that was full of water. As he was doing this motion he felt a burning in his rt bicep/shoulder. Employee continued to work until a later date when he went and seen a Dr. who sent him to a specialist that told him he would need to have the tendon repaired. EE worked until day of surgery 8-29-13 then RTW on 8-30-13

EE positioned a cap wedge between the flight conveyor chain & the drive sprocket on the vessel & then instructed the plant operator to jog it. The chain broke causing the flight to reverse direction and pinch EE's left arm between the flight and the chute. EE was transported to CAMC for x-rays but they were unable to test due to swelling. EE is currently off work awaiting MRI.
The truck was driving up hill loaded when he began to lose power he applied the brakes and steered into the berm turning over on the driver's side.

Contractor was attempting to clean windshield on his coal truck. Contractor had his left foot on drivers side tire and right foot on driver side steps. Contractor slipped and fell backward onto another truck parked beside him. Total fall was approximately 4 feet. Slip, fall, strain.
While adjusting the 1/1/2" to 0" belt, an employee walked up an inclined 3" water line "steel". The water line broke causing the employee to fall. The employee was taken for x-rays and observation. He was released to return to his next scheduled shift which is 1/31/2011.

While drilling a hole through a piece of angel iron, with a hand drill, the drill bit broke and struck EE's left hand.

Employee suffered small cut to right eye. When slamp he was working on slipped from his hand hitting a metal band and causing a piece of metal to break off and enter eye. Not reported to this office until 7/23/07.
Employee suffered a laceration to his mouth when the pipe he was cutting swung loose and hit him in the face.
