Employee was utilizing a prybar to assist with removal of motor for replacement when the prybar slipped and struck employee on the right side of the hardhat/head on the right ear causing a laceration to the upper portion of the right ear. The hardhat took the brunt of the force from the blow of the prybar.

Injured employee was opening hatch lids on top of trucks when they strained their back. Employee was working with case management for remedies when they decided to be seen by a doctor which then elevated case to reportable.
Grinder angle was incorrect, causing it to bind up on a narrow area on angle iron. Grinder bounced at employee causing lacerations on chin and neck.
Employee jumped from loader ladder into soft berm and rolled EE's left ankle. Requested to see doctor on 3/26/2024 and was given restrictions.

EE was using an pneumatic air hammer to chip lime scale away from the truck side loading scale. The EE reached out over head while lying on the ground and over extended, causing an acute pain in the right shoulder and numbness in the right hand.

We were in man lift boomed out over elevator. A Lhoist employee entered and began operating elevator. The elevator made contact with the man lift boom that I, and my employee, were in. My employee received a broken nose and slight fracture in right knee.
Winter storm caused the water pipe to freeze that is located on the exterior wall. EE used a propane pear torch burner to thaw the metal water pipe. EE returned the burner and upon EE return, noticed smoke coming from the lab. Extinguishers were used to put the fire out. It was around 38 minutes that all signs of fire was not present in the wall. No injuries or entrapments.

EE slipped and fell on snow and ice and was place on restricted duty following the incident. EE landed on their RT side including hip and elbow, however when medical treatment was sought it involved EE's right foot.

Employee was air lancing around a lime silo when lime came in contact with eyes. EE immediately flushed eyes for 10 minutes at an eye wash station, they used 2 bottles of saline solution. Eyes felt better however they wanted medical treatment. Employee was sent to urgent care this morning. EE received 3 prescriptions, one of which was tylenol with codeine.
EE was stung by a bee while working on Kiln Level 2 at LNA Marble Falls superstructure project. The EE was transported to Urgent Care. From there the EE was being transported to a Medical Center, when the EE went into anaphylactic shock. The EMS had EE transported by helicopter to a Medical Center in Austin Tx. for treatment.

EE was lifting a railcar vibrator into place when EE felt a pop on the inside of the elbow. The EE experienced soreness, but did not report the injury to management until December 15th. It was at that time, the employee sought medical treatment and was placed on restricted work duties.

I pulled on the handle of a sand blasting pot to check the sand volume. The pot was completely empty and tipped back. I attempted to step back and tripped, pulling the pot down with me. The handle of the sand pot came down across my left ankle and caused a fracture.
An employee was lifting an end of a screw flight, when employee felt pain in lower back. The employee was placed on restricted duty by a medical provider.

Working on the drive of a screw conveyor. One employee was jogging, when another grabbed the belt and tried to manually jog. His thumb was caught by the belt and his right thumb was amputated.

Employee was standing on ladder when ladder slipped out from under him and his tools hit him in the lip. The ladder possibly slipped from dust material under legs of ladder.
Employee was laying brick and went to hit the brick with a brick hammer to lodge it into place. Instead of hitting the brick he hit the metal shim beside it and the hammer slipped off the shim and hit his finger.
Multiple welders in same area; redness in left eye with moderate discomfort. Employee notified supervisor of discomfort at 9:30 AM 05/13/2009; received medication from doctor. Noticable relief within a couple hours, will return to doctor for check-up 05/15/2009; employee was released with no limitations or restrictions 05/13/2009

Employee was cleaning up lime & dirt accumulated on the ground. He was wearing shaded safety glasses & pulled them down to see better. Lime & dirt blew into the employee's face & eyes. Employee stated this caused irritation to both eyes even after flushing with water.

ATTEMPTING TO CLOSE THE OUTER DOOR, THE UPPER DOOR AND LOWER DOORS CLOSED TOGETHER WITH THE EMPLOYEE RIGHT HAND IN BE TWEEN THEM CAUSING BRUISING AND A SLIGHT FRACTURE TO THE MIDDLE FINGER. EMPLOYEE WAS REMOVING BOLTS USING A HAND WRENCH AND STRAINED WRIST BUT DID NOT REALIZE THAT THERE WAS ANY DAMAGE UNTIL IT BEGAN TO SWELL AFTER CONTINUED USE FOR SEVERAL DAYS.
