Fall occurred in the track entry at spad #112. Fall was 20'L x 20'W x 8'H. Intersection was bolted with 4' resin bolts. Fall was approx 3400' from face. Men were evacuated and area has been timbered. off. A cleanup plan has been posted and is being followed. There were no injuries.

Member was removing drill steel ouf of hole after drilling. Rock fell and caught right hand between drill steel and rib. Received 12 stitches.
There was a gob approximately 4" deep on the offside of the belt drive. EE changed directions while walking and twisted his knee.
EMPLOYEE WAS PULLING SLACK ON THE #2 SHUTTLE CAR FOR A POWER MOVE WHEN HE FELT A POP & PAIN IN HIS LEFT CALF.

A roof fall was discovered in return entry of #7 on 10-27-05 at 11:00 am the fall measured app 30' x 20' x 7' high. no personal injuries or machinery was involved. Prelimary investigation revailed that the fall was above the anchoraye point.
Size of fall was 60' long 20 wide 8' high. Fell in intake at spad #124 2000' from face. Ventilation was affected. Area will be rehabilitated. Passage of miners was not affected. No injuries. MSHA was notified immediately as well as state officials.
LOCATION IS SPAD #113, SECTION 001 APPROX 2000' FROM ACTIVE FACE IN THE BELT ENTRY. VENTILATION OR PASSAGE OF MINERS WAS NOT AFFECTED. ALL AGENCIES WERE NOTIFIED.
FALL LOCATION IS SPAD #0143, #5 ENTRY APPROX 1500 FT FROM ACTIVE FACE. FALL DID NOT AFFECT VENTILATION OR PASSAGE OF MINERS. SIZE OF THE FALL IS APPROX. 25' LONG, 20' WIDTH, 8' HEIGHT. MSHA INSPECTOR INVESTIGATED FALL ALONG WITH Co.
LOCATION OF FALL IS APPROX 2000' FROM FACE IN #5 ENTRY OUTBY SPAD #101. ALL APPROACHES HAVE BEEN CRIBBED OFF. VENTILATION OR PASSAGE WAS NOT AFFECTED.
Employee was unloading top structure from car on track at #8 brk. Member slipped and fell striking his head on bottom with structure landing on his chest. Complained of shortness of breath.

Employee was carrying buckets filled with rock dust. He bent over to pick up buckets and strained his lower back.
Roof fall on #2 belt, 002 section, #4 entry from spad # 151 to # 186 for a distance of app 120' long, 18 to 20' wide and 6-8' high.

Employee was loading a belt for a belt move, while pulling the belt with channel locks employee felt a pain in lower back.
Employee was cleaning roadways when the canopy of the scoop got caught on a bent bolt and it hit employee in face.
Roof fall inby spad #127 1 break in #11 entry, approximately 45' long, 18' to 20' wide, 8' to 10' high.
ROCK FALL OCCURRED ONE CROSS CUT INBY SURVEY STATION #131 IN THE #4 ENTRY AND WAS 40' WIDE BY 20' DEEP AND BETWEEN 4' TO 6' HIGH.
ROCK FALL OCCURRED ONE CROSSCUT OUTBY SURVEY STATION #138 IN THE #8 ENTRY, MEASURING 20 FEET DEEP AND BETWEEN 4' AND 6' HIGH.
Employee had his hand between two sections of hauler. He steered hauler, mashing his middle and ring finger on right hand.
While tramming a unihauler, the canopy caught a 1/2 header hanging down which broke the canopy loose and it fell off striking employee in the head and neck. Employee did not miss any work until 11/29/2004.
Employee was putting conveyor chain back together in miner. He was using a hammer to put the chain back together when the hammer slipped and hit his index finger.
Employee was hammering roller when a piece of metal broke off and flew into his arm.
