Fall occurred in the track entry at spad #112. Fall was 20'L x 20'W x 8'H. Intersection was bolted with 4' resin bolts. Fall was approx 3400' from face. Men were evacuated and area has been timbered. off. A cleanup plan has been posted and is being followed. There were no injuries.

Member was removing drill steel ouf of hole after drilling. Rock fell and caught right hand between drill steel and rib. Received 12 stitches.

A roof fall was discovered in return entry of #7 on 10-27-05 at 11:00 am the fall measured app 30' x 20' x 7' high. no personal injuries or machinery was involved. Prelimary investigation revailed that the fall was above the anchoraye point.

Size of fall was 60' long 20 wide 8' high. Fell in intake at spad #124 2000' from face. Ventilation was affected. Area will be rehabilitated. Passage of miners was not affected. No injuries. MSHA was notified immediately as well as state officials.
