Injured employee reached with hand to straighten out a bent roof bolt EE was installing in the roof when EE mistakenly hit the rotation instead of the up pressure. EE's glove was caught by the roof bolt and injured EE's right hand.
Employee was removing a canopy from a shuttle car, when it shifted and caught right ring finger near the tip, resulting in a laceration and fractured bone in the tip of the finger.
The injured employee was working on the #1C/M on the South Mains #2 Unit, changing the left side ripper motor in the #4 heading. The process of changing the motor requires a lot of physical movement and the use of small tools to aid in positioning. While prying to move the motor in place the injured employee suffered pain in right shoulder.
The C/M operator was mining 4 right crosscut into #5 heading when car driver found injured employee at the boom of the miner suffering from head and face trauma. The exact cause of the trauma is under investigation.
An unintentional roof fall has occurred in the face of the number 5 entry inby spad 3708, involving MMU 004-0 Continuous Miner. The fall area was approximately 10' wide, 15' long and 6' in height. the fall pulled 6 roof bolts.

Employee was walking along scoop and got stuck in the mud, lost balance and fell into the batteries of the scoop injuring ribs.
EE was walking to the power center from the feeder area, when EE caught EE's foot on a rock and fell twisting EE's lower back. EE has started missing work from this injury.
Employee was operating the continuous miner and after backing from a cut, EE reached down to move the cable, when EE was struck by a piece of rock from the roof & rib. EE suffered bruising to the back and abdomen area.
Employee is a Roof bolter operator on 2 Right Section, when they put a strap in place and rotated the bolt. The strap moved resulting in a cut to middle finger of right hand. The laceration was cleaned and closed with glue.
Employee is a scoop operator on the South Mains section. Employee was putting roof bolt plates on the bolter supply tray, as they turned, they heard a pop and felt pain in left knee.

Employee was cleaning the threads on a bolt with a wire wheel when one of the wires came off and struck EE in the left eye. The employee was wearing safety glasses and fellow employees in the area confirmed the use of glasses.
Employee was loading bolting supplies on a dual head bolting machine. As EE was loading supplies, EE struck left knee against the machine.
Employee was working on the 4 Right Section and EE was helping move the C/M and as EE was hanging the cable EE felt pain in lower back.

An unintentional roof fall occurred in the No. 8 entry on the North Mains section. The fall occurred as the Continuous Miner operator was backing the miner out of the last lift. Material fell from the lift just completed and extended into the entry, pulling bolts and catching the miner head. No Injuries were suffered from this event.
An unintentional roof fall occurred in the 3 Left cross- cut of the North Mains unit. The fall occurred in the cut and travelled into the intersection. Area was approximately 16 to 18 feet wide, 30 feet long and approximately 10 to 12 feet high at the highest point. Area involved was two crosscuts inby spad 2129.
Was informed by letter that employee has evidence of pneumoconiosis.
Employee was loading a piece top structure into a scoop bucket and strained their back.
Employee was moving a water hose for a C/M from mud, when the employee felt pain in lower back. The injury occurred in wet & muddy conditions. X-Rays were negative and employee was diagnosed with a strain.
Employee was hanging cable when rubber tire on mantrip hit foot.
A miner received injuries while they and two coworkers were repairing a belt splice on the 1A conveyor belt. The miner had positioned themselves on top of the conveyor belt when it started.
Employee was riding the section mantrip to the surface after shift had completed. Employee was on knees when employee's jacket caught a roof bolt and employee then turned over. Employee continued to work until 5/5/2019 when employee first received medical attention.
A non-injury event occurred on MMU 001/ MMU 003 Mains Section, when a continuous miner cutting in the #7 Entry struck a borehole and water discharged into the mine.

Employee was pulling pump cable through a stopping, when EE slipped on the cable and twisted EE's left knee. Employee went to the Dr on 7/23/2018
Employee was walking to check on water sprays when ee stepped on a rock twisting right Ankle.
Drill steel came out of the hole, struck the drill boom then glanced into the employees face, causing a laceration to lower lip. Wound required stitches to close.
During installation of a 10' fly board it slipped off the ATRS when glue was being placed in the hole.
A non-injury roof fall occurred in the #5 Heading while retreat mining was being conducted inby the 7th Crosscut of the 3 left Panel. Fall pulled roof bolts and caught the continuous miner in the 4th lift of the entry.
