An unintentional roof fall occurred 1 cross-cut inby spad 4203. The fall measures approximately 20' long by 14' wide by 8' in height.

Injured employee reached with hand to straighten out a bent roof bolt EE was installing in the roof when EE mistakenly hit the rotation instead of the up pressure. EE's glove was caught by the roof bolt and injured EE's right hand.

Employee was removing a canopy from a shuttle car, when it shifted and caught right ring finger near the tip, resulting in a laceration and fractured bone in the tip of the finger.

The injured employee was working on the #1C/M on the South Mains #2 Unit, changing the left side ripper motor in the #4 heading. The process of changing the motor requires a lot of physical movement and the use of small tools to aid in positioning. While prying to move the motor in place the injured employee suffered pain in right shoulder.

The C/M operator was mining 4 right crosscut into #5 heading when car driver found injured employee at the boom of the miner suffering from head and face trauma. The exact cause of the trauma is under investigation.

Employee is a Roof bolter operator on 2 Right Section, when they put a strap in place and rotated the bolt. The strap moved resulting in a cut to middle finger of right hand. The laceration was cleaned and closed with glue.

Employee was cleaning the threads on a bolt with a wire wheel when one of the wires came off and struck EE in the left eye. The employee was wearing safety glasses and fellow employees in the area confirmed the use of glasses.

An unintentional roof fall occurred in the No. 8 entry on the North Mains section. The fall occurred as the Continuous Miner operator was backing the miner out of the last lift. Material fell from the lift just completed and extended into the entry, pulling bolts and catching the miner head. No Injuries were suffered from this event.

An unintentional roof fall occurred in the 3 Left cross- cut of the North Mains unit. The fall occurred in the cut and travelled into the intersection. Area was approximately 16 to 18 feet wide, 30 feet long and approximately 10 to 12 feet high at the highest point. Area involved was two crosscuts inby spad 2129.
