Employee had removed cover guard to put oil in reservoir. EE set the guard beside the reservoir and after adding the oil, EE and another employee went to put the guard back into place. After lifting it up in tandem they turned to the right to align when employee felt a sharp pain in their lower back.

Employee was lowering the belly pans so that EE could remove the engine oil pan because of crack. EE had a chain tied to the front lower eye weldment and ran the chain to the rear of dozer under the counterweight and then vertically tied to the hook on the crane boom to hold tension on the chain to keep the pan from dropping. The pan dropped causing EE's finger to be pinched.

Working on a bearing of a tail pulley he was in the process of repositioning the tail pulley to access the bearing on the left side. After the block was removed the pulley became lodged above the conveyor frame on the mounting bolts. He attempted to free the pulley using his hands when the pulley shifted and caught the pinky finger of his right hand.

EE was releasing come-a-long from a conveyor that he had tied off while making repairs to the belting. Upon releasing the latch the cable hung, ee jerked the come-a-long with his right hand. The latch released pinching his right thumb. He went to the urgent treatment clinic and they gave him a tetanus shot and placed 2 stitches in the side of his thumb.

EE was repairing a water leak in a PVC line. He cut the line using a utility knife and was cutting towards himself, when he struck his right thumb. He was taken to the Powell County Clinic and the injury required 6 stitches. He retruned to work the next morning.

Employee was using a 2' long pry bar to open the inspection lid on the door of a USI crusher. He was pulling bar toward himself when the bar slipped striking employee in the right corner of his mouth. The corner of the employee's mouth was cut and required 4 stitches.
Employee was cleaning chute feeding conveyor belt & struck side of conveyor support with left wrist & bruised hand. Went to Urgent Treatment Center for x-rays & found small bone in left wrist bruised. EE struck side of support brace when exiting the conveyor. Will be on restricted duty until 6-19-08.
EE was tightening roller bolts on under carriage of a d9r dozer. After completing the project he stood and starting walking to his truck when he felt a catch in his left knee. He went to powell clinic and knee was x rayed and he was given an anti-inflammatory medication and placed on light duty until 4/30/08 an at that time he will be re-evaluated.

EE WAS ASSISTING ANOTHER EE TO CHANGE O-BELT ON THE DRIVE OF A IMPACT CRUSHER. HE THINKS THAT WHILE LEFT THE MOTOR USING A PRYBAR HE STRAINED A SMUCLE IN HIS BACK. AFTER X RAYS DRS. DETERONINE D THAT HE POSSIBLY HAD STRAINED MUSCLE AND PRE-CRIBED PAIN MEDICINE TO EASE SYMTONS AND EXCUSED HIM FROM WORK UNTIL 6-6-99.
EE WAS INSTALLING THE SCREEN SUPPORT COLUMNS ON A NEW SCREENING TOWER. HE WAS ALIGNING THE BOLT HOLES UP WHEN HE HAD GOT OUT OF THE BUCKET TRUCK AND CLIMBED ONTO THE SCREENING BINS. HE ATTACHE D A 2 TON COMEALONG TO A LEG AND WRAPPED THE CABLE AROUND THE LEG HE WAS ALIGNING. HE POSITIONED HIMSELF ON A 12" H-BEAN AND STARTED TWISTING THE LEG. HE APPLIED TOO MUCH PRESSURE TO THE COME

CONTRACT HAUL TRUCKWAS GOING TO STOCKPILE TO BE LOADED AND RAISED HIS BED. THE TRUCKER IN UNATTENTIVE NOT FORGETTING THAT HIS BED WAS UP AND HUNG BED INTO THE ELECTRICK WIRES, JERKING THE UTIL ITY POLE DOWN AND STARTED A FIRE IN THE SWITCH HOUSE (EVERYTHING DESTROYED).
EE WAS TIGHTENING BOLTS ON A CRUSHER AT QUARRY AND WRENCH GAVE WAY AND HE FELL ON A PROTRUDING BOLT, STRIKING RT ARM.
EE CAUGHT RIGHT FOOT IN HYDRAYKUC ORESS WGUIE DRILLING, FRACTURING RIGHT FOOT.

WHILE ENGAGING A STARTER ON MOTOR A SPRING BROKE ON THE MANUAL STARTING LEVER. THE SPRING FELL AND SHOPRTED OUT ACROSS TWO OF THE TERMINALS. THE FLASH FROM THIS CAUSED BURNS TO THE FACE AND BURNS TO THE WRIST.
