EE was operating a 3 wheeled personnel carrier and the tram petal stuck running under a separate ride.

EE was duck walking looking for bad places in a piece of cable. The ee slipped on a wet piece of draw rock falling aprox 26" hitting his elbow.

EE was tramming 3 wheeled personal carrier to the outside. The ee stated that he drove through the flypads holding them up with his foot. His foot got caught in the top.

EE was crawling off the top of a set of scoop batteries after putting them on charge. Swung leg over side and caught a piece of metal.

The EE was loading tool boxes into scoop bucket. He pulled one end of the box around and felt something pull in his back.

EE ran into corner with S/C causing the corner of the rib to roll off. The EE got out of the S/C to move the the rock out of the way and the rest of the corner fell hitting him in front of the right ear.

Employee had changed a pressure gauge on the continuous mining machine, employee was positioned at the rear bumper of the machine, he asked the machine operator to start the machine, the machine operator bowed the head of the machine causing the machine to go to the mine floor mashing the employee's foot.

Employee was repairing the shuttle car cable when his knife slipped and cut his hand.

The EE was greasing the feeder and was getting up and felt a pop in his right elbow.

Removing belt structure from under old belt set up @ old 10 head, employee slipped and fell breaking his ankle.

The EE was working on a s/c and his wrench broke hitting his arm with sharp edge.

An unplanned roof fall has occurred, above the anchorage, in a previously rehabilitated area, 2000+ outby the working section, in the N0 3 entry, approximately 30 feet outby survey spad 1417. The fall was approximately 80 feet long, 20 feet wide and 5 foot high. Ventilation and/or the transportation of men were not affected.

Victim was traveling through a fabricated air lock ventilation door, riding on a battery powered three wheeler, the door was raised by a steel rope attached to a wench, the rope broke suddenly and struck the victim on the forehead causing a laceration which required 4 stitches

The EE was making a splice in a cable when his knife slipped cutting his hand.

The EE was Hanging Miner Cable and injured his back.

A piece of rock fell out between 2 roof bolts. The injured employee was struck in the left knee and ankle.

A roof fall occurred in the N01 entry face, of the 001 section, survey spad #2773 +75 feet approximately, above the anchorage of the roofbolts being used. The area was known outcrop and additional roof control measures and precautions were being taken. There were no injuries. The fall did not impede passage or interrupt ventilation in the mine.

employee was operating a 3 wheeler battery powered transporter when he struck his head on the mine roof, knocked his hat off, and cut his head on the mine roof.

The injured employee was traveling on a 3 wheeled personal carrier, he was in reverse motion and hit his head into the mine roof twisting him around and the 3 wheeled personal carrier stuck the rib.

The injured person was reaching for a crib block. A coworker of the injured person was tossing crib blocks. As the crib blocks where being tossed one of the crib blocks struck the hand of the injured person which has resulted in a laceration requiring stitches.

Occupational Pneumoconiosis

The injured miner was installing roof bolts, when a piece of draw rock measuring 6" x 2" x 3' long fell, striking miner in lower back.

Injured employee opened airlock door, pulled the hoist cable up too far and cable broke, causing the door to fall.

Injured employee was pulling the miner cable out of the way with his foot and a shuttle car came around the corner and hit his foot.

Employee was operating a shuttle car tramming to the feeder, had his left elbow outside of the operators deck and caught arm between shuttle car and coal rib.

Putting roof bolt resin in hole when draw rock fell striking left hand.

Injured employee was crawling through crosscut, when a piece of draw rock fell on him.

Injured employee was installing outside roof bolt, when a piece of roof rock fell on his left hand that was placed on top of the machine, resulting in a fracture.

Continuation of an old roof fall.

Rock fell out between bolts & struck him in the head.

Operating scoop, tramming scoop with his hand behind deck and caught it against rib, causing loss of fingers (ring and pinky on right hand).

Spot roofbolting and draw rock fell out beteen the roofbolts stiking employee in the head, neck, shoulder and knee.

Foreman was making up roofbolts when draw rock struck him in lower back.

Roof fall 10-14 ft. high, 18 ft wide, 110 ft long.

Roof fall outby spad # 1442. Fell 50', 18' wide and 4' high.

Roof fall fell approx. 8-12 ft high, 18 ft wide, 305 ft long.

The operator of the #1 RRII Fletcher bolter was attempting to put the outside bolt up when a piece of draw rock (size 1' long, 8" wide, 2" thick) fell from the top lacerating his hand.

Throwing crib blocks out of entry, he smashed his fingers against metal object. Left little finger required 4 stitches.

WHILE CLIMBING UP LADDER FOOT SLIPPED OFF 3RD FROM TOP STEP OF LADDER. FOOT AND LEG WENT BETWEEN LADDER AND WATER TANK CAUSING 1 1/2 WIDE CUT ON LEFT SHIN. CUT REQUIRED 10 STITCHES.

Tramming feeder through area with close clearance. Had arm between feeder and rib. Smashed arm between feeder and rib.

Changing intermediate shaft on operator side of miner. While rolling outer drum out of way it tumbled over and pinned right arm and right hand against miner.

While hanging board on wall of mine office board slipped and metal edge of borad caught ee s right thumb laceration.

BENDING OUT ROOF BOLT FOR LAST PUSH UP TO INSTALL IN TOP. BOLT TURNED & SPRANG BACK, STRIKING EE ON THE TOP OF HIS NOSE.

Replacing structure on no. 1 belt. Pulling bottom structure out, hand slipped and right elbow struck a piece of top structure.

KNEE SWELLED.

GOING UP STEPS TO LAMPHOUSE SLIPPED AND ICE AND SNOW AND FELL.

LIFTING AND PUSHING/PULLING MINER CABLE HURT LOWER BACK.

LIFTING BAGS OF ROCKDUST... HURT BACK.

PINNING TOP - GOT FINGER PINCHED BETWEEN DRILL STEEL AND TOP.

RUNNING SCOOP IN REVERSE-GOT CAUGHT BY BUGGY CABLE AROUND NECK STRAINED NECK MUSCLES.

PINNING TOP JAMMED HEAD AND NECK INTO TOP.

OPERATING PINNER AND STRAINED LOWER BACK INSTALLING ROOF BOLTS.

RUNNING SCOOP- RAN OVER ROCK AND GOT HEAD INTO TOP.

WATER AND MUD IN FACE- MINER OPERATOR TWISTED AND SLIPPED IN DECK OF MINER.
