EE was duck walking looking for bad places in a piece of cable. The ee slipped on a wet piece of draw rock falling aprox 26" hitting his elbow.

EE was tramming 3 wheeled personal carrier to the outside. The ee stated that he drove through the flypads holding them up with his foot. His foot got caught in the top.

EE ran into corner with S/C causing the corner of the rib to roll off. The EE got out of the S/C to move the the rock out of the way and the rest of the corner fell hitting him in front of the right ear.

Employee had changed a pressure gauge on the continuous mining machine, employee was positioned at the rear bumper of the machine, he asked the machine operator to start the machine, the machine operator bowed the head of the machine causing the machine to go to the mine floor mashing the employee's foot.

An unplanned roof fall has occurred, above the anchorage, in a previously rehabilitated area, 2000+ outby the working section, in the N0 3 entry, approximately 30 feet outby survey spad 1417. The fall was approximately 80 feet long, 20 feet wide and 5 foot high. Ventilation and/or the transportation of men were not affected.

Victim was traveling through a fabricated air lock ventilation door, riding on a battery powered three wheeler, the door was raised by a steel rope attached to a wench, the rope broke suddenly and struck the victim on the forehead causing a laceration which required 4 stitches

A roof fall occurred in the N01 entry face, of the 001 section, survey spad #2773 +75 feet approximately, above the anchorage of the roofbolts being used. The area was known outcrop and additional roof control measures and precautions were being taken. There were no injuries. The fall did not impede passage or interrupt ventilation in the mine.

The injured person was reaching for a crib block. A coworker of the injured person was tossing crib blocks. As the crib blocks where being tossed one of the crib blocks struck the hand of the injured person which has resulted in a laceration requiring stitches.
