No 65 Coal

An unplanned roof fall above anchorage was discovered during the Preshift Exam at Survey Station 7893, approximately 1200' outby the 2nd Left working section. The fall measured approximately 20'L x 20'W x 9'H and has been dangered off from all approaches.

Employee was operating a shuttle car when he hit a pot-hole in roadway causing the employee to hit his head against the canopy.

Employee was turning a mantrip around when the front tires contacted a ledge causing the steering wheel to spin and the steering knob struck the employee's right hand.

Employee was installing a roof bolt with a bearing plate when he attempted to straighten the plate and pinned his middle finger on his right hand to the mine roof.

Employee was riding in mantrip to section when he stated that he hit his hardhat on a roofbolt.

A roof fall measuring 6- feet' X 19 feet wide X 20 feet long occurred at the No. 2 to Number 1 crosscut inby spad number 7656

A roof fall measuring 6-7 feet X 19 feet wide X 35 feet long occurred 150 feet inby spad number 7636 in the number 1 entry.

A roof fall meas. approx. 20' x 50' x 6-8' occurred on the 2nd left section between #6 and #7 cc. inby spad #7398. The fall will not be cleaned up

EE was setting in the shuttle car deck when a piece of rock measuring 16" X 12" X 2" thick rolled from the rib and struck him on the ankle.

Prior to employees shift starting, he was working on his mining belt (repairing light pouch) when he cut his hand with a knife.

Employee was removing a canopy from a scoop bucket and later felt a pain in his abdomen.

Employee stepped in a hole and twisted ankle.

A roof fall above anchorage was discovered in the Mains Intake Air Course at Survey Station 266 approximately 6000 feet from the Mains Section. The roof fall measured approximately 20' wide by 20' long by 5' high. All approaches have been dangered off and cribbed.

Employee was walking to mantrip, tripped on a rock and fell.

Employee was pulling lid back on a oil can when his hand slipped and struck sharp edge.

A roof fall above anchorage was found in the Mains Primary Intake Escapeway approximately 5,500 feet outby Mains 1st Left Section. All approaches to fall area was cribbed off. Life line was rerouted and all employees instructed on new escapeway entry. Fall will not be cleaned up.

Employee stated that while walking across section, he stepped in a hole and twisted his knee and back.

Employee stated that while unplugging the scoop batteries, a wire grounded inside the plug. This caused an arc from the plug.

On 3/10/11 the employee reported that while pulling belt with a scoop, the scoop slid over and the battery end of the scoop struck him on the leg. Employee did not miss work until 3/14/11.

While operating the rubbered tired mantrip the EE struck the rib and jarred his low back.

EE was cleaning the miner, while getting off the miner the EE slipped and fell causing him to injure his lower back. EE did not miss work until 3/10/11.

A roof fall occured on the 1st right section in the #3 heading, 1 break inby spad #6029. The fall was approx. 8' X 18' X 22. No persons injured and no equipment was involved.

Employee was helping on roof bolter when the roof bolter operator allowed the drill head to come down on his foot.

A roof fall measuring 20'x20'x6' occurred near spad number 1691 near old 2 left panel. This area is approximately 8000 feet outby face and the fall area will not be cleaned up.

EE stated that "while pulling on rubber mat that was caught under the 1st Rt. Power Center" he felt pain in his right shoulder blade area. *EE continued to work this shift and did not miss work until 10/28/10.

EE was bolting top and while pulling drill steel down, he hit the fast feed and dropped the head of the roof bolter on his knee.

While installing bit inserts on the miner, the EE struck his hand with a hammer.

EE was pulling a rock out of rock box on No. 1A belt, when the slate bar came back and struck him on his ribs. EE continued to work this shift.

A roof fall was discovered 2200' outby Jenny Branch section at spad #5025. Fall measured approx. 6' X 19' X 19'. "Will not be cleaned up".

EE stated that while walking near the No. 1 belt conveyor he stepped in a hole and twisted his knee.

EE was cutting the belt when the knife slipped and cut his hand.

A roof fall above anchorage occurred in the No.6 entry adjacent to the belt entry. Fall measured approx 7' X 18' X 20'. 3600 foot outby Jenny branch section.

While getting off end loader employee slipped and twisted his ankle

A roof fall measuring 20' x 22' x 5' occurred in the No. 2 entry near spad No. 5203, MMU 004-0. 18 cross-cuts outby the face.

Employee was getting on the loader when he slipped and fell striking his right shoulder.

A roof fall measuring 8' x 19' x 19' occurred on the Jenny Branch section near spad No. 5095. *The fall will not be cleaned up.

Employee was filling water tank with water out of a pump hose and tripped over the hose and fell about 4 ft from back of pickup truck to ground hitting shoulder area.

A roof fall measuring 19' wide x 16' high x 30' long. Occurred on the Jenny Branch Section near spad No. 5094.

While operating scoop, employee placed his hand on top of the scoop canopy, and his fingers were caught between the canopy and coal rib.

EE stated that while cleaning up around No 4 belt head he slipped on a rock and fell. When he got up he could not put much weight on his foot.

EE was working to construct an overcast. He had stood up a metal leg and while attempting to place a bolt in the leg it fell over and struck him on his lower leg.

EE was bolting top when a piece of rock fell on EE's hand.

A roof fall meausring approx. 6'x20'x20' occured on the mmu 004-0 in the intersection near spad #4552. This area was outby section.

EE was shoveling the belt when he tripped and was struck above the eye by a shovel held by a co-worker. EE alleges knee injury. The accident is being contested. (See Attachment)

A roof fall measuring 20' x 20' x 9' occurred near spad No. 4333 on the Jenny Branch section.

A roof fall measuring approximately 80' x 20' x 10' occurred on the Jenny Branch section between the No. 6 - No. 7 crosscut.

A roof fall occurred on the pillar line of the 6- right section-Rock fell on the miner hand.

Driver grabbed tail gate pin and attempted to pull out on gate. Loose coal fell out taking pressure off gate causing gate to rapid shut and catching his right hand ring finger, causing amputation at first joint on finger.

Employees foot was caught between the metal frame of a airlock door and a rubber tired pers. carrier.

Employee was tramming a MRS across the break when a piece of rock rolled off the rib and struck his foot. Rock measured 93" x 21" x 12" thick.

A roof fall measuring 6' x 20' x 20' was discovered on 1/31 during a weekly examination of old airways. The fall was located 1 crosscut inby spad #152. The fall area has been dangered off and the fall will not be cleaned up.

Employee was cleaning off the top of the miner when a piece of draw rock fell and struck his left hand.

A roof fall measuring approx. 20' x 20' x 6' occurred on 7 right section #3 entry near spad no. 2595.

Employee was working on a 480 volt receptacle. When he opened the dust cover, the receptacle flashed "arched" causing burns to the back of employee's hands.

Employee was checking the operational controls on the 21-SC shuttle car, when the tram pedal was pressed. The shuttle car inadvertently moved, causing the shuttle car to strike the employee.

Opened Air Lock Doors and air caught doors and blew back on the left arm of EE.

EE was bolting top in #7 heading on 1st left section, finished his row of bolts and was getting ready to advance pinner, when rock broke loose in between bolts and struck him on the head and shoulder.

Right mains across from spad 1249, #8 entry in intersection. 8'high x 40'long x 16'wide. Mined Oct.2005. Bolted with 5'resin bolts & spot bolted with 8'bolts.

Roof fall on belt line at crosscut #56. Fall was 14'high x 16'wide x 60'long. Roof was bolted with 5' resin bolts. Area mined Oct. 2005.

Unintentional roof fall above anchorage point fell in crosscut at Survey Station 971 in Return air course. Fall occurred sometime between weekly exam of return and was discovered during weekly exam on 9/22/05. No injury - no equipment involved.

Unintentional roof fall inby Section dumping point, between Survey Station 1191 and 1230, no injury, no equipment involved, fall investigated by MSHA, not cleaned up.

#9 entry intersection stack rock with very small coal streaks.

Mining coal in the no. 10 heading 1st right section, a self-extinguished, frictional methane ignition occurred.

MINING UNDER HOLLOW AND LOW COVER AREA. STRATA HAD COAL STREAKS IN IT.

Bolter operator pulled bolter forward; pulled wheel on employee's right foot. He continued to work after he was injured to end of shift. He called on the 9th of March stating he was going to be off.

Alleged accident and injury. EE elleged he fell backwards striking his back and head against the mine floor. Investigations made by the mine foreman and WV state mine inspector revealed no physical evidence to support the ee's claim.

While installing a roof bolt, a rock was dislodged and struck ee on the shoulder.

Employee stated that while getting out of the truck, his foot got caught causing him to fall, he stated that his left leg and back hurts.

Employee was removing a fuel pump from truck, when he attempted to get off the truck he lost his footing and fell approximately 6'.

EE STATED THAT WHILE OPERATING THE ROOF BOLTER HE REACHED OVER TO GET A STICK OF GLUE. WHILE IN THIS POSITION, THE DRILL HEAD CAME DOWN ON HIS LEG.
