WE WERE USING A CRIB BLOCK TO BLOCK OPEN THE TAILGATE OF A MACK MUD TRUCK SO WE COULD CLEAN MUD FROM AROUND TAILGATE, AS WE WERE PLACING CRIB BLOCK AGAINST TAILGATE, CRIB BLOCK SLIPPED & STRUCK THE RIGHT WRIST OF INJURED EMPLOYEE. HE DID NOT WANT TO SEE A DR. AT THAT TIME BUT LATER ON THE 24TH HE WENT TO A DR.

THE EE HAD MADE SEVERAL ROUNDS WITH HIS 777B HAULER WITH NO MECHANICAL PROBLEMS. AT 9:00 AM THE HAULER LOST ITS RETARDER AND THE EE LOST CONTROLOF THE HAULER. HAULER ROLLED 4 TIMES INTO A VALL EY FIELD. EE SUFFERED A FRACTURE OF THE LEFT THIGH AND NECK INJURIES.
HE WAS WORKING ON A D-10 DOZER. HE HAD BOTH PUSH ARMS BLOCKED AND WAS WORKING ON THE OUTSIDE OF THE ROLLER FRAME. THE LEFT SIDE PUSH ARM SLIPPED, ALLOWING THE ROLLER FRAME TO DROP APPROXIMATEL Y 4". A TRACK CLEAT STRUCK THE EMPLOYEE'S LOWER RIGHT LEG, CAUSING A CONTUSION. HE STARTED MISSING WORK ON 4/29/03.
EE WAS WALKING ON HAULRAOD. HE STEPPED ON A FLAT ROCK AND HIS FOOT SLIPPED OFF THE EDGE OF THE ROCK. THIS CAUSED HIS LEFT FOOT TO TWIST SIDEWAYS RESULTING IN A FRACTURE OF HIS LEFT FIBULA APPR OXIMATELY ONE INCH ABOVE THE ANKLE.

HE WAS REMOVING A TILT JACK ON A D-11 DOZER. HE HAD ONE FOOT ON THE PUSH ARM AND PLACED THE OTHER FOOT ON THE TRACK. THE TRACK WAS MUDDY AND HISFOOT SLIPPED CAUSING HIM TO FALL ON THE TRACK.
TRUCK WAS DROVE INTO DITCH AFTER LEAVING SEVEN FEET OF SKID MARKS AND TRAVELING FORTY FT AFTER SKID WITH NO SIGN OF BRAKING INTO DITCH. TRUCK WAS FOUND IN NEUTRAL AND EE BRAKE TURNED OFF. NO DAMAGE WAS DONE TO TRUCK.
LOADER BOOM HIT HAULER BED WHILE LOAIDNG HAULER. THIS CAUSED THE HAULER OPERATOR TO BE JERKED SIDEWAYS WHILE INT HE OPERATORS SEAT HE COMPLAINED OF BACK PAIN AND WAS SEEN BY A DR. DR SAID POSS IBLE PULLED MUSCLE AND RETURNED HIM TO WORK. ON JUNE 13TH HE RETURNED TO THE DR MRI BEEN OF WORK SINCE JUNE 14TH.
HE OPERATED A LOADER ON 2-6-02, LOADER TIRE HIT A POTHOLE IN THE ROAD CAUSING HIS BACK TO BECOME MILDLY PAINFUL. HE OPERATED THE LOADER ON 2-8 AND 2-9. ON 2-9 HE WENT TO A LOCAL EMERBENCY ROOM WHERE HE WAS DIAGNOSED WITH A HERNIA OF A VERTEBRAE DISK.

DOOR SLIPPED & FELL, CATCHING HIS LEFT LITTLE FINGER BETWE EN THE DOZER TRACK CLEAT & THE DOOR. THIS RESULTED IN A FRACTURE WITH LACERATION AT THE FIRST JOINT OF THE FINGER.
WHILE HE WAS TAKING A D-10 DOZER OVER A 4 FOOT DROP, THE DOZER BLADE CAUGHT THE GROUND CAUSING THE DOZER TO SUDDENTLY STOP IN AN STEEP POSITION. THE SUDDEN STOP AND THE STEEP INCLINE, CAUSED T HE OPERATOR TO BE THROWN FORWARD STRIKING HIS HEAD ON A METAL BEAM IN THE TOP OF THE CAB. THE OPERATOR WAS IN AN AIR RIDE SEAT AND WEARING A SEAT BELT.

THE FITTING TO BE BRAZED EXTENDED 1/4 INCH ABOVE TH E INSIDE BOTTOM OF THE AIR TANK. THERE WAS APPROX 1/4 INCH OF LIQUID "AIR DRY" (FLAMABLE) STILL IN THE TANK. WHEN HE TOUCHED THE FITTING WITH A TORCH FLAME THE FUMES IGNITED & FLASHED ON HIM.
HELPING TO LIFT A TRACK PAD FOR A D-11 DOZER WHEN HE EXPERIENCED PAIN IN HIS LEFT SHOULDER.
DRIVING LOADED COAL TRUCK UP A GRADE WHEN THE REAR WHEELS STARTED SPINNING. THE TRUCK ROLLED BACKWARDS INTO A DITCH AND TURNED ONTO HIS SIDE. LIMITED DAMAGE TO TRUCK BUT EMPLOYEE COMPLAINED OF BACK PAIN.
WHILE OPERATING A LOADER, THE FRONT WHEEL EITHER RAN OVER A ROCK OR INTO A SMALL DEPRESSION IN THE WORK AREA. THE JERKING MOTION CAUSED THE EMPLOYEE TO COMPLAIN OF PAIN IN HIS NECK.
TWO EMPLOYEES WERE MOVING TWO FIVE GALLON BUCKETS OF OIL FROM THE TRACK OF A D-10 DOZER. ONE EMPLOYEE HANDED A BUCKET FROM THE TRACK TO THE EMPLOYEE ON THE GROUND. EMPLOYEE ON GROUND WAS SITTI NG THE BUCKET DOWN WHEN HE EXPERIENCED SHARP PAIN IN HIS LEFT SHOULDER-DISLOCATION.
WHILE HOLDING UP THE DRIVELINE SO THE TRANSMISSION COULD BE SLID IN, THE DRIVELINE SLIPPED AND FELL ON FINGER CUTTING THE FINGER ABOVE THE JOINT.

THE DOCTOR DISCOVEREDA SMALL IMBEDDED OBJECT IN HIS LEFT EYE.
EE WAS PUSHING DIRT WITH A DOZER, HE BACKED OVER A ROCK WHICH CAUSED PAIN IN HIS BACK.
ROCK KNOCKER WAS STUCK TIRE MOVED A LITTLE, ROCK KNOCKER CAME LOOSE AND HIT CORNER OF LEFT EYE. EE HAD TWO STITCHES.
HE REPORTED TO WORK AND STATED HIS STOMACH WAS UPSET BUT HE DECIDED TO WORK. LATER HE WAS WORKING ON A ESCAVATOR AND FELL EITHER BECAUSE HE BECAME DIZZY OR LOST HIS FOOTING. AS HE FELL HE ST RUCK HIS RIGHT ELBOW ON A BRACKET. HE IS TO HAVE SURGERY ON 2-3-99.

EE WAS ASSISGNED THE TASK OF STEAM CLEANING A CAT 992 LOADER. THE EE SET UP A STEP LADDER AND WAS APPROX. 6 FT OFF THE GROUND WHEN HE STARTED USING THE STEAM CLEANER. HE WAS HOLDING THE STEAM CLEANING WAND IN BOTH HANDS. HE PRESSURIZED THE WAND THE STEAM MADE CONTACT WITHT HE COLD METAL OF THE LOADER AND WITH THE COLD AIR WHICH RESULT GAVE HIM ILLUSION AND HE WAS ABOUT TO FALL

THE SOCKET WAS THROWN FROM THE WRENCH AND STRUCK THE EMPLOY EE ON THE RIGHT LENSE OF HIS SAFETY GLASSES. THE FRAME OF THE SAFETY GLASSES WAS FORCED INTO THE EMPLOYEE'S RIGHT CHEEK 5 STITCHES.
EE ALLEDGEDLY STEPPED IN A HOLE THAT WAS FULL OF WATER AND MUD. THIS TWISTED HIS RT KNEE. THE EE WENT TO THE DR ON 5-29-97 WHERE FLUID WAS DRAWN OFF THE KNEE. THE EE RETURNED TO WORK ON 5-30-9 7.
WHILE BACKING UNDER LOADER THE HAULER BRAKES ALLEGEDLY FAILED CAUSING HAULER BED TO STRIKE THE BUCKE TO THE LOADER. THE EMPLOYEE STATED THIS ERKED HIM BACK IN HIS OPERATOR SEAT.
EE WAS REMOVING A ROLLER FRAM PIN FROM A DOZER. EE HAD INSERTED A CHISEL INTO THE PIN SPREADER CLAMP. EE STRUCK THE PIN WITH A HAMMER. THE VIBRATION DISLODGED THE CHISEL AND THE CHISEL STRUCK THE EE BOTH ABOVE AND BELOW THE LEFT EYE. THE INJURY REQUIRED FOUR STITCHES IN THE LEFT LOWER EYE LID.

EMP DISMOUNTING DOZEER WHEN HE SLIPPED ON THE BLADE PUSHARM AND FELL TO THE GROUND. DAY BEFORE INCIDENT JOB SITE HAD RECEIVED TWO AND ONE HALF INCHES OF RAIN AND CONSIDERABLE MUD WAS PRESENT. EMP DID NOT GO TO DR UNTIL MID DECEMBER. 1-8-97 DR DIAGNOSED EMP WITH FRACTURED VERTEBRAE (HAIRLINE CRACK). ALSO TWO HERNIATED DISKS. RESTRICTEDRETURN 1-22-97.
THE EMPLOYEE WAS OPERATING A HAULER. THE LOADER LOADED A LARGE ROCK INTO THE HAULER BED. THIS CAUSED THE BED TO "ROCK" AND THE EMPLOYEE COMPLAINED ABOUT PAIN IN HIS NECK. THE EMPLOYEE STARTED TAKING PHYSICAL THERAPY ON 3-25-96 FOR TIGHT MUSCLES IN HIS SHOULDERS. EMPLOYEE STARTED MISSING WORK ON 4-2-96.
HE WAS OPERATING A 992 CAT LOADER ON 2-5-96. HE STATED HE UNCOVERED A LARGE ROCK IN THE PIT. HE PICKED UP THE ROCK AND STARTED OUT OF THE PIT. THE WEIGHT OF THE ROCK CAUSED THE BACK OF THE LOA DER TO RAISE OFF THE GROUND. HE STATED HE ATTEMPTED TO LOWER THE BUCKET SLOWLY, HOWEVER, THE BUCKET CAME DOWN TOO FAST, CAUSING THE REAR TIRES TO STRIKE THE GROUND AND BOUNCE.
WHILE USING A KNIFE TO CUT A SUCTION HOSE ON A PUMP, THE KNIFE SLIPPED CUTTING THE EMPLOYEE ON THE INSIDE OF HIS RIGHT THIGH. 3 STITCHES WERE REQUIRED TO CLOSE THE WOUND.
