Employee was diagnosing an issue with the swing operation of the manlift basket while in the basket. The employee leaned over the control panel while moving the basket causing the employee to strike head on the mine roof & the controls of the manlift. The employee was seen in the ER & given stitches for a laceration to the chin. The employee was released to light duty the same day.

Bed of truck came up after dumping load and bed hit ceiling. This is the second report being completed I have the initial report completed on file too.

Employees were maneuvering a full 55 gallon drums into a containment area. When placing a drum, the employee's finger was in the line of fire between the drum being moved and one already in the containment. Consequently, two fingers were impacted the middle finger losing the tip and requiring minor surgery and the ring finger which was broken both on the right hand.

Underground facility, shop area. Nothing within the conditions contributed to this accident.

When the BTI powdertruck was parked at the end of the shift, a small spark was seen by 2 employees under the operator's seat. All employees evacuated the mine and the equipment operators reacted by extinguishing the flame with 2 fire extinguishers. MSHA was called and communicated to us to stay out of mine.

EE returning to loader for a load turning around a pillar while downshifting and drove through low spot causing wheel to jerk as he was turning causing left hand to separate and strike door frame of truck.

EE was holding onto aftermarket steering knob, when wheel jerk occurred, the force caused knob to roll over and become flat with steering wheel. As wheel came around, and with EE still holding onto knob, his left hand was smashed between knob and inside of door frame, breaking a bone above left pinky finger.

EE was cleaning rooms when he was summoned by another ee at the North Face. He drove over & after arriving stopped the loader & shut off the engine & lights. He got out of the loader & walked approx. 75' to talk to other ee,discussion & getting paper towels out of the drill he walked back to the in the dark & when doing so he struck the loader bucket with his face.

EE was loosening oil plug to do preventive maintenance on engine & when doing so, wrench slipped out of plug resulting in his hat coming off his head, out of reach. He proceeded to loosen drain plug to drain oil & after oil started draining he proceeded to crawl out from under loader when he raised up too early & hit his head on bottom of counterweight on back of loader.

Employee was scaling when a rock fell in front of the scaling machine. A piece of the rock came down the scaling arm and through the glass. He received a small cut on his leg from the glass. The cut required on stitch.

Employee was using a prybar to move a heavy object when the prybar slipped and pinched his right thumb against a concrete wall directly behind him.

EMPLOYEE WAS CUTTING A HOSE WITH A KNIFE. THE KNIFE SLIPPED AND THE EMPLOYEE CUT HIS LEG. CUT REQUIRED STITCHES.

EMPLOYEE WAS ATTEMPTING TO SET UP A TRAILER. THE TRAILER SLIPPED AND CUT HIS FINGER TIP.

EE WAS SUPERVISING MAINTENANCE WHEN A METAL PLATE SHIFTED BUMPING INTO THE EMPLOYEE.

EMPLOYEE WAS LOOKING OUT THE WINDOW OF THE CAT HAUL TRUCK WHILE BACKING UNDER THE LOADER BUCKET TO GET LOADED WHEN HE FELT SOMETHING HIT HIS RIGHT EYE.

EE ALLEGES CARPAL TUNNEL SYNDROME IN BOTH HANDS, AGGRAVATED BY WORK. EE ADMITS THE CONDITION STARTED WHILE WITH HIS PREVIOUS EE, WORKING AS A DRILLER. EE HAD SURGERY ON THE FIRST HAND ON 2/8/0 0 AND THE SECOND HAND ON 3/21/00.

EE WAS HAMMERING A BOLT THROUGH A HOLE WHEN THE HAMMER HEAD CAME OFF AND HE HIT HIS HAND ON THE SCREEN, HE WAS GOING TO REPLACE, BREAKING HIS THUMB.

EE WAS DRIVING A CAT D25D HAUL TRUCK, THE TRAILER PART OF THE TRUCK TURNED OVER AND THE EE GOT OUT OF THE CAB AND WAS RUNNING IN THE MINE TO GET THE LOADER OPERATOR TO UPRIGHT THE TRAILER, WHE N HE RAN INTO A ROCK PILLAR IN THE MINE AND HIT HIS FACE. THE EE HAD NO LIGHT TO ILLUMINATE HIS PATH.

WHILE EMPLOYEE WAS CHANGING SCREEN DECK ON SCREENING PLANT HE WAS DRIVING A WEDGE TO SECURE THE SCREEN DECK. HE HIT THE STEEL WEDGE AND IT CAME OUT AND HIT HIM IN THE FACE, JUST BELOW THE RIGH T EYE.

OPERATOR WAS DRIVING AN EMPTY HAUL TRUCK FROM THE PLANT TO THE MINE. HE WENT AROUND A CORNER TOOFAST AND LOST CONTROL. HE WENT INTO A DITCH AND HIT A TREE. HE BOUNCED AROUND IN THE SEAT, WAS NOT WEARING A SEAT BELT AND INJURED IS BACK AND NECK.

EMPLOYEE WAS HELPING TO SPLICE A CONVEYOR BELT WHEN HE LOST HIS BALANCE AND FELL OFF THE CONVEYOR TO THE GROUND BELOW, WHICH WAS ABOUT 10 FEET. EMPLOYEE LANDED ON HIS FEET ON LOOSE ROCK. INJUR ED HIS RIGHT FOOT AND ANKLE.

EMPLOYEE WAS UP ON A LADDER FILLING HOLES WITH EXPLOSIVES WHEN THE TOP OF THE LADDER SLID SIDEWAYS AND HE FELL TO THE GROUND.

EE STRUCK BY FULL BARREL OF HYDRAULIC OIL, WHICH FELL FROM BUCKET OF LOADER. BARREL WEIGHED ABOUT 350 POUNDS, AND BUCKET OF LOADER WAS 7-8' HIGH FROM GROUND.

EMPLOYEE WAS CLIMBING OUT OF THE CAB OF HIS TRUCK TO WIPE OFF THE HEADLIGHTS. DUE TO MUDDY STEPS ON HIS TRUCK, HE LOST HIS BALANCE AND TWISTED HIS KNEE ON THE STEP.

HE WAS LIFTING 5N OIL BARREL WHEN HE FELT SOME DISCOMFORT IN THE LEFT GROIN AREA.AT THAT TIME HE THOUGHT NOTHING OF IT.IN THE PAST 3 WEEKS, HE BEGAN TO FEEL DISCOMFORT AND PAIN IN THAT AREA, A LSO SWELLING. 3
