EMPLOYEE WAS REACHING OVER THE HANDRAIL ON A EUCLID R35 FOR A SAMPLE TRAY FOR A SHALE CHECK. WHEN HE LEANED ON THE HANDRAIL FOR THE TRAY, THE HANDRAIL GAVE WAY. HE FELL APPROX 6 FT TO THE GROU ND. EMPLOYEE HAD NOT REPORTED THE DEFECTIVE HANDRAIL TO MINE MANAGEMENT. EMPLOYEE FAILED DRUG TEST. DOCTOR SAYS HE WILL BE UNABLE TO WORK FOR 6-8 WEEKS.

EE WAS HAULING ROCK FROM THE FACE TO THE CRUSHER. ON HIS WAY BACK TO THE FACE, THE LEFT FRONT SPRING BROKE CAUSING THE VEHICLE TO JERK LEFT. WHEN THE VEHICLE JERKED LEFT, IT SLAMMED THE DRIVER INTO THE DOOR. THE EE WAS TREATED FOR A SPRAINED LEFT SHOULDER.
EE WAS PERFORMING SCALING WITH THE BUCKET OF LOADER. HE BACKED UP UNAWARE THAT HE WAS CLOSE TO A PILLAR. HE BACK INTO A PILLAR CAUSING NECK AND BACK STRAINS.
THE EMPLOYEE WAS ENGAGED IN HAULING SHOT ROCK TO THE CRUSHER. ON HIS RETURN TRIP TO GET ANOTHER LOAD HE LOST CONTROL OF THE TRUCK AND STRUCK A PILLAR. IT IS BELIEVED THAT THE EE FELL ASLEEP WH ILE OPERATING VEHICLE.
EMPLOYEES WERE TEARING DOWN AN OLD SCALEHOUSE TO MAKE ROOM FOR A NEW SCALE TRAILER. THE EMPLOYEE STEPPED ON A BOARD THAT HAD NAILS IN IT. TWO NAILS WENT THROUGH HIS BOOT AND PUNCTURED HIS LEFT FOOT.

HE WAS RECHECKED BY DR 9-15-98 WITH A SPECIALIST AND THEY DETERMINED THAT THE STOMACH MUSCLE HAD NOT PROPERLY HEALED. SURGERY WILL BE NEEDED TO REPAIR TORN STOMACH MUSCLE ON 10-8-98.
