Still dark and snowing and visibility was poor. Dozer was pushing dirt up on stockpile when truck slightly struck tractor cab.
Changing dryer screen at wash plant. Screen was worn on and sharp on edges. Victim sliced his little finger on side of screen.

Employee had been fueling equipment and he drove to the 988 loader. When he was getting out of the truck his foot slipped and he fell out onto the pile of coal. He had fuel on the bottom of his boot making it slick.
A 992 LOADER WAS LOADING THE TRUCK WITH DIRT. SAID THE JARRING AROUND HURT HIM
Employee was hauling dirt and the ground was frozen he was pulling up to stop and the steering wheel jerked and wrenched his wrist.

EE WAS DUMPING HIS LOAD AND THE DUMP GAVE WAY AND TRUCK OVERTURNED. NIGHT TIME, DRY MATERIAL (DIRT AND SHALE MIX)
EE WAS CLIMBING ONTO THE BACKHOE WHEN HIS KNEE POPPED & GAVE OUT.

EE WAS BENDING OVER TO PICK UP A PUMP HOSE AND HIS LOWER BACK POPPED, CAUSING GREAT PAIN. HE HAD NOT EVEN PICKED UP THE HOSE YET.
IT WAS DARK, & EE CLIMBED ONTO THE TOP OF INTN'L POWDER TRUCK TO SEE IF IT NEEDED NITRATE IN THE BIN. THE DOOR HAD BEEN LEFT OPENED & HE FELL INSIDE THE TANK OF THE TRUCK, HITTING HIS BACK ON THE METAL SIDE OF THE BIN.
OUR WELDER WAS CUTTING ANGLE IRON GUARD OFF OF TRUCK SCALE, AND EE WAS STANDING DOWN ON GROUND BELOW, WHEN THE ANGLE IRON WAS CUT IT FELL, HITTING HIM ON OR ABOVE THE RIGHT KNEE.

NARROW SPOT IN HAUL ROAD. DRIVER OF EMPTY TOLD DRIVER OF LOADED THAT THERE WAS PLENTY OF ROOM. DRIVERS MUSJUDGE DISTANCE, BOTH BEDS HIT. BOTH DRIVERS HAD SEAT BELTS ON. BOTH BED NEED TO BE TAK EN OFF & REPAIRED.
EMPLOYEE CLIMBED A 10' LADDER TO FUEL TANK UP; WHILE HE WAS LEANING ON THE 300 GAL. TANK, THE STAND IT WAS ON GAVE WAY; HE FELL WITH HIS FEET UNDER LADDER, TWISTING LEFT ANKLE. HE RADIOED FROM FUEL TRUCK TO GET SUPERVISOR TO AREA; THEN HE WORKED FROM THAT TIME UNTIL AROUND NOON. SEEATTACHED SHEET.
