Miner went into c-9 tunnel conveyor to unjam feeder. At this time the cause of the incident can't be determined. Management and the MSHA inspector could not interview the miner due to injury. Miner lost left arm in the tail pulley. At this time why miner was at the tail pulley is unknown.

Employee was taking a cutting edge off a pallet, and the cutting edge slipped. The employee did not move left hand out of the way fast enough, and it was pinched between the cutting edge and the floor. The employee waited until the next day to seek treatment. It was determined that the tip of middle finger was fractured.
EE was removing pusher bolts from the crusher. Using left hand to guide the impact wrench to remove the bolts, the left index finger got caught between the lip of the crusher, and the impact wrench fracturing the finger. EE was taken to the ER. EE was diagnosed with an internal fracture of the left index finger between the first and 2nd knuckle. On light duty.

Employee was loosening a bolt on an engine mounting when the wrench slipped off the bolt head. Employee slammed his hand into the engine bracket causing a contusion. Was first treated as first aid only by medical professionals. On later follow-up visit miner was prescribed meds and PT.

Employee was cleaning a screen near a conveyor. Employee stood up abruptly hitting his head on a nearby support beam. Employee suffered a laceration requiring stapling to close the wound. Employee was instructed to take off 1 day before returning to work with restrictions.

Employee was walking around maintenance shop building. He stepped on a nail that was sticking out from the ground. Nail punctured bottom of safety boot and into bottom of employees right foot. Suffered an infection from the nail puncture to the bottom of right foot.
Miner was repairing lower screens of a screen deck. Took catwalk section out to service. Did repair and climbed back up to upper level. Did not replace decking. Turned to do another task when he fell through opening. Fractured ankle 4' down when struck lower hand rail of lower deck.

EE had Helper Tighting the setting of the crusher. a small size stone intzaferred with the hypaolic ram used for turning the top of the crusher. He reacher through the safety cover to remove the stone between strokes. The cylomone came out while his fenders were there, crushing then between the ram and rep of top.
