On 8/2/16, the employee requested medical treatment, was taken to the company clinic, diagnosed with a L-ankle sprain, was returned to full duty and was prescribed physical therapy. The employee alleged that they rolled their left ankle while walking towards the back of the quality control lab.

SPILLED DRAIN ROCK WAS BEING SHOVELED AWAY FROM THE CONVEYOR BELT.
EE WAS STANDING BY WHEN OTHER WAS HITTING A CUT BOLT. WHEN HE HIT THE CUT BOLT A PIECE OF THEIR HAMMER FLEW OFF AND HIT EE IN THE SIDE.
EE WAS GRADING WITH OUT CAT 12 GRADER. WHEN HE PUSHED DOWN ON THE UP DOWN LEVER IT KICKED BACK AND HE PULLED A MUSSEL IN HIS SHOULDER.
