Graymont Western Us Inc · Over-exertion in lifting objects

Employee was loading a goat shot (side of mountain up hill) with 40lbs buckets, after they were done, they wer experiencing pain on the left shoulder, reported it to HSE and declined medical attention. On July 6 they went to get checked at the hospital, x-ray revealed calcination of the shoulder joint and given a steroid (cortisone) injection to help with pain and discomfort.

Employee went to put a flame in kiln 1, EE was standing on a 29" rolling ladder, there was pulverized coal dust still in the hood of the kiln, when they introduced a flare to ignite the coal, the kiln produced a blowback, knocked the employee to the ground from the 29" ladder and they sustained burn injuries to right arm and lower back, 2nd degree burns.

An employee was hooking up a drum basket to a forklift, EE bent down to lift and slide the right-side fork to align with the basket, when EE lifted the fork, EE felt like an impact to EE's lower back. EE was taken to the Tooele Utah hospital where EE was diagnosed with lower back muscle sprain. EE received 2 injections for pain and inflammation.

Employee was flagging a shot at around 2:00pm, suddenly the ground gave way and the employee fell in a hole, the right leg outside the hole, the left leg inside the hole, the right knee was at about EE's chest, then EE felt a pop on EE's right knee, EE then called a co-worker to help EE get up.

EE exited EE's truck on the lower scale and started climbing the steps to the scale house. EE tripped on the top step and hit the bridge of EE's nose on a structural beam. EE exclaimed that EE was in a hurry and that is what led to EE tripping and falling. EE's nose was broken as well as a minor spinal cord injury from the impact of the fall.

Emp was trying to open a dump chute using the slide hammer. As EE was sliding the hammer, the handle broke and EE's thumb slipped between 2 pieces of steel, pinching it. EE was taken for treatment and received sutures. EE was released full duty and returned to work.

On 6-26-2020, emp was backing up a loader and struck a parked haul truck. Was jarred and had soreness in neck. EE was taken to the ER to be evaluated and was told to rest for the next 2 days, missing both shifts. EE returned to work on 6-29-2020 at full duty.

The miner was dumping lime dust when the latch on the rear gate of the dump truck wouldn't shut. The miner exited the vehicle to unstick the latch traveling thru the dust. The lime dust entered the miner's boots and was not removed before causing burns to the miner's feet. The burns were caused by the reaction of the dust and perspiration.

INJURED WAS WALKING ACROSS THE YARD WHEN HE COLLAPSED FROM AN APPARENT HEART ATTACK.HE WAS NOT PERFORMING ANY WORK AT THE TIME AND HAD STARTED HIS SHIFT JUST 30 MINUTES BEFORE.AN EMT WAS WITH HIM WITHIN MINUTES.THE AMBULANCE WAS ON THE SITE AT APPROX 9:05AM AND HE WAS PRONOUNCED DEAD AT 9:41AM.
