While operating hand valve on dewatering equipment, employee slipped on muddy ground and struck knee against the pipe flange causing two small (approximately 1") lacerations. Employee went to ER and they closed the wound with medical adhesive, gave EE a prescription antibiotic and returned EE to full duty immediately.

Employee was unhooking a trailer from a pickup. The ball was stuck in the cast, the employee stepped on the bumper to release it and it rolled slightly and caught his left foot between the hitch and bumper. Employee was taken to the doctor and has a contusion of the left foot.
Three Employees were loading metal panels on to a trailer. Each employee was holding a corner. When the panel was put down, it smashed one of the employee's middle left finger, causing a laceration to the nail bed.

Injury was operating a bull dozer and he was hooking a lime bin to the dozer so the bin could be moved. After the bin was hooked up and ready to move he noticed a rock underneath a wheel on the bin and attempted to remove it. As he did this the dozer pulled forward. The wheel went over his arm, shoulder, back, and center of his body.
Employee was operating a haul truck and had back pain. He believes sitting in the haul truck strained his back.

Employee had stockpile truck with concrete stone. Went to scale to weigh and forgot to dump his load at stockpile. EE started down road to the pit. He was not going very fast but weight of truck and decline made it difficult to stop. He came to curve and truck tipped over. He was wearing seatbelt. He climbed out and was taken to hospital for x-rays and stitches.

PLANT HAD SHUT DOWN AND EE WAS CLEANING ROCKS OFF CONVEYOR BELT. HE HAD 3 POINTS OF CONTACT ON CONVEYOR AS HE WAS CLIMBING DOWN. HIS FOOT SLIPPED ON A TIRE AND HIS FINGER CAUGHT ON A SHARP CORNER AND SLICED HIS LEFT RING FINGER, REQUIRING 9 STITCHES.
