Miner was coming down the surge bin ladder and fell landing on the ground.

EE was using a scrapper to remove material from the side of the surge bin. EE's right hand came into contact with the roller guard during this process resulting in a broken right pinkie finger.
This incident became reportable when the miner finally agreed to go to the doctor and an MRI revealed a torn meniscus on 7-18-22. On the date of the incident the miner said miner was fine and no medical care was needed. On May 12th the miner slipped on the bumper of miner's stockpile truck when shutting the hood during pre-operation inspection.

THE MINER GOT CAUGHT IN THE ROLLER OF THE TAIL PULLEY ON THE OVERHEAD CONVEYOR ON THE SCREEN BELT. LEFT WRIST AND FOREARM HAVE BROKEN BONES. RIGHT ARM HAS CUTS AND FRICTION BURNS. THE TIP OF RIGHT INDEX FINGER WAS CUT OFF AT THE FIRST KNUCKLE AND THE FINGER WAS SKINNED.

REPLACING IMPACT HAMMERS. NUT WAS STUCK IN SOCKET. MINER PUT BOLT BACK IN THE NUT AND WAS HITTING THE NUT WHEN THE SOCKET SLIPPED AND HIT MINER'S LEFT INDEX FINGER.

Plant man was working on the piggy back conveyor for the surge bin, he had the guard propped up, a gust of wind came knocking the guard down hitting him on the helmet and causing a laceration in his forehead.

EE was checking to see if a wire was loose on the fan in the air conditioning unit in his loader. EE did not turn the motor off on the loader and stuck his fingers too far into the unit. EE had partial amputation of the very tip of middle finger on right hand requiring two stitches.

EMPLOYEE WAS USING A PICK AX TO STRIKE MATERIAL WHILE ATTEMPTING TO REMOVE IT FROM THE FEEDER FLOOR. HE HIT THE METAL FLOOR WITH THE AX, WHICH RESULTED IN THE AGGRAVATION OF A PRE-EXISTING INJ URY THAT HAD BEEN SUSTAINED IN A NON-WORK RELATED INCIDENT. THE PHYSICIAN DIAGNOSED A BRUISED HAND AND PLACED THE EMPLOYEE ON NO WORK STATUS.
