At the Truck Shop, EE was unloading flat steel from a flatbed truck. EE picked up the steel. The weld broke off the washer on the right side as one leg of the chain broke on the right side of the steel which left the whole sheet of steel fall on EE left hand, and causing extensive damage to EE's left ring and pinky finger. Employee was getting out of the triaxle truck at the fuel pumps in the parking lot. Employee slipped on the step while getting out of the truck and grabbed the hand hold to keep from falling. EE felt a pop and then pain in the right arm. EE went to the emergency room and was diagnosed with a torn bicep muscle.

Employee was using the crane on a service truck to lift a CAT D11 final drive in the shop. EE was turning the hub with the crane and using a chain to guide the hub. The hub turned quickly and pulled the guide chain and in turn pulled EE's shoulder. EE experienced pain in right shoulder. Did not receive any medical treatment until 6/23/2023 when EE started therapy.
Employee was removing the hood springs on a tri-axle truck in preparation of removing the radiator. The spring was under tension and slipped out of their hand and the hooked end of the spring penetrated the skin of their right hand. Medical attention was required to clean and stitch the wound.

Employee was going to grease a truck and had climbed down the ladder to the ground. EE had parked on a flat area near the fuel tanks. EE turned around after climbing down the ladder and tripped over some rocks, causing them to fall over backwards and they landed on left shoulder, causing a sprain.
Recordable hearing loss discovered during annual hearing test. The miner has been one of our drillers for over 30 years and was a driller prior to being employed by us. Over that time, EE has run various drills on various mine and OSHA locations. RES was chosen because I spoke to OIEI on 11/16/20 at 09:45 and was advised to pick a mine EE drills at.
Employee was helping clean out a box trailer that will be used as a job trailer. EE was stepping up into the trailer from EE's pickup tailgate and experienced pain in the right knee. The pain got progressively worse during the day. EE did not seek medical attention until 10/5/2020.
Employee was removing the plow bolts on a loader bucket by using an air gun to loosen the nuts. The worn head of the bolt spun and cut through employee's glove and caused a laceration on left palm. The laceration required stitches.

Employee was helping mechanics change track on a CAT D11 carry dozer. EE hit the dozer track with a sledge hammer in order to move the track into position to connect the track. A piece of metal chipped off the track when EE struck it with the sledge hammer and struck EE in the left forearm. The metal penetrated EE's skin and thus required a visit to the emergency room.
The employee was changing drill bits. EE was hitting the lock ring of the drill bit with a hammer. A piece of metal came off the hammer head and struck the employee in the back of right leg. The metal penetrated the skin. This required medical attention to remove the metal.

The employee was pulling the starter rope of a gasoline powered pump at a diesel fuel tank. The rope was momentarily hard to pull. This caused a strain to the right shoulder.
Employee was kneeling on the ground while welding on the ladders of a front end loader. This resulted in pain and bruising, abrasion to the right knee. Employee was diagnosed with an infection in right knee. Employee did not notify RES Coal of the accident or seek medical attention until 7/29/2015.

Employee was climbing down the ladder from a CAT 777C rock truck. Right foot was already on the step and employee tried to put his left foot on the same step and left foot slipped off of step due to mud on the step. This caused his right leg to twist, straining his right groin muscle.
The employee was hooking up chains to a crusher plate so he could load the crusher plates onto his truck with his truck mounted crane. The area around the crusher plates was snow covered and icy. Employee slipped on the ice when hooking up chains and tried to catch himself from falling and twisted his lower back.
The employee was operating the Hitachi 1800 shovel when the front engine shut off. He left the operator's cab and proceeded to check the oil level on the front engine. He bent over to check the oil and when he stood up he struck the top of his head on the fire suppression system. This caused a laceration to the top of his head.

Employee was changing the drill bit and was using a sledge hammer to strike the bit to loosen it for removal. He felt pain in his groin area after swinging the sledge hammer. Employee has continued to work and did not have any lost time until 8/5/2013. Procedures were diagnostic until 8/5/2013. Surgery was done today, 8/5/2013, to repair hernia.

Clinic Visited 08/3/12. Employee got something in his eye at work and flushed the eye. The employee and supervisor thought they had everything cleaned out. A couple days later the employee was feeling discomfort and decided to get the eye looked at. A prescription was given to prevent potential infection.
Employee was walking around water truck and stumbled on some rocks or stones. He started to fall and reached out with his left hand to grab bracket for suction line to prevent his fall. He injured his left shoulder and left foot. He fell to the ground and landed on his right knee.
Operator was cleaning snow off the walkway of Hitachi shovel. He was using his foot to clean off the snow and he struck his toe against a rubber door stop causing pain in his left knee. He worked the rest of the shift and did not tell his supervisor until 12:00 p.m. on 1/21/2012.

While performing an equipment pre-check, the equipment operator slipped and fell onto his back side in a small puddle of water. He worked until March 1, 2011 when he sought medical attention for pain of the lower back. MRI shows possible slipped disc (L5) in his back.

Removing left hood spring from tri-axle truck and the spring rapidly decompressed and came off of attachment point striking employee on left index finger at the tip
While fueling Cat 992C Loader, the operator had his left foot on the bottom rung of the access ladder and pulled the fuel hose towards him and the hose caught on a rock causing the employee to lose his balance and fall backwards hurting his left ankle and hitting the back of his head several inches above the neck.
Bouncing around of truck caused discomfort to lower back.

Using a large adjustable wrench to loosen a fitting on an oil transfer pump for the purpose of bleeding air. The fitting loosened and ee's left index finger proximal phalange struck the oil tank then the wrench struck the finger.
