EMPLOYEE WAS OPERATING OUR 980 LOADER. LOADER WENT OVER SAND BERM. MATERIAL IN LOADER BUCKET BROKE WINDSHIELD. EMPLOYEE SUFFERED MINOR CUTS TO LEFT SIDE OF FACE FROM BROKEN GLASS. WENT TO URGENT CARE FOR MEDICAL ATTENTION. FIRST AID WAS ADMINISTERED. EMPLOYEE WAS RELEASED BACK TO FULL WORK DUTIES SAME DAY. NO FOLLOW UP DOCTOR VISIT NECESSARY.
Employee was cutting a piece of metal with a blow torch. While cutting, a shard of hot metal flew out and landed in employee's left ear. Employee was treated with first aid measures for burn and returned to full work duties on same day.

Employee was assisting another employee, on a Bobcat, with moving around barrels in the yard. While moving the barrels, one began to tip over. Employee attempted to prevent barrel from tipping over and pulled a muscle due to the extreme weight of the barrel.
Employee was cleaning the rock crusher. During cleaning, a piece of concrete rebar, protruding from the machine, punctured the employee's left hand. Employee's wound treated at doctor's office and employee was returned to full work duties on same day.
Working on a piece of equipment to remove a jammed rock in pully belt. When reversing the belt to remove rock, the belt spun and pinched employee's thumb between pully and belt. Employee's thumb was injured as a result of the jammed rock. Received stitches in thumb. Returned to work 1 day following accident on modified work duties.
Employee working on equipment with a screwdriver. Screwdriver slipped and angled upward toward employee's face. Screwdriver hit eye and lacerated his right eyelid. No loss of work incurred. Employee returned to normal work duties the following day, since incident occurred toward end of work shift.
Employee welding tool box onto company service truck. Heat from welding unit burned through attached torch hose, causing gases inside hose to ignite. Ignition lead to small fire. Employee inhaled smoke from fire and had trouble breathing. Sent to urgent care to be checked out. Employee returned to normal work duties immediately following incident.
