EE was in white ford for 2 hrs, clutch went out, pre-shifted another truck, #C-24, and worked for 5.5 more hrs until EE went into turn with excess speed and rolled onto the trucks passenger side while heading to stock pile. Resulting in EE getting thrown into passenger side due to not wearing seatbelt.
EE was washing out a screen deck. As EE was performing this duty EE felt a pop in EE's back, causing pain.

Removing rock blockage from chute. A large rock slipped and thumb was under it and landed on another rock. Left thumb broken.

The employee was washing off the pad under the crushing plant when a rock fell off belt #2 hit the ground and then hit EE's foot. The rock hit just above the steel toe. The employee went to the hospital for an x-ray and was put in a boot while awaiting treatment.

Excavator operator was working to close to the Highwall. The Dozer working on the bench above knocked a bolder free that fell on the cab of the excavator, injuring the operator.
EE our plant Supervisor had a S.T.S. on our annual hearing test performed on 02/01/2016. EE was re-tested with-in 30 days (on 02/29/2016) and a mild loss resulting in a S.T.S. was again reported. EE has been with the company for 10 years.

Employee bent over to pick up his hard hat and when he stood back up to put it on he hit his head on the tail pulley on belt #3. He cut his head and had to have seven staples put in his head.

Employee reached into a heater fan that he was working on to retrieve something that had fallen in. The fan came on and cut his 3rd finger on his right hand. The cut required stitches.
Employee was driving a king pin out of a truck axle when a nut flew off the pin and struck him above the left eye, cutting his face and cracking the bone.

THE DRIVER DID HAVE HIS EYE PROTECTION AND HARD HAT ON.
