EE WAS WORKING ON TIRES. TIRES WERE SPARES FOR TEREX 3305B. EE WAS ASSEMBLING A TIRE AND HOLDING LOCK RING UP WITH BAR. WITH HIS LEFT THUMB HE WAS PUSHING "O" RING INTO PLACE AND THE BAR SLIPP ED AND LOCK SNAPPED DOWN ON THUMB.
EE WAS WORKING ON QUARRY DRILL. THE BOOM WAS OUT AND HE WAS MANUALLY PUSHING BOOM BACK INTO PLACE. HE WAS STANDING ON SOME CAT TRACK NEXT TO DRILL AND WHEN HE STEPPED DOWN HE TWISTED ANKLE CAU SING HIM TO FALL INTO DRILL FRAME.
EMPLOYEES WERE WORKING ON THE ROLLER FRAME OF A QUARRY DRILL AND USED HEAT TO REMOVE A BOLT. THE INJURED EE CAME ALONG TO HELP AND SAW A LARGE NUT LAYING ON THE FLOOR AND PROCEEDED TO PICK IT UP BURNING HIS RIGHT PALM & FINGERS.
EE WAS WORKING ON THE FRONT SPRING OF A MACK TRUCK AND SWINGING A SLEDGE HAMMER. HE TWISTED HIS BACK RESULTING IN A PINCHED NERVE.
EE WAS REMOVING A GASKET FROM THE REAR END PLATE OF AXLE. THE GASKET SCRAPER HE WAS USING SLIPPED, CUTTING HIM ON THE LEFT HAND BETWEEN THE THUMB & FOREFINGER. REQUIRED 2 STITCHES.

EE WAS USING A BALL JOINT FORK TO SEPERATE BALL JOINTS FOR REPLACEMENT. EE HIT THE FORK AND IT CAME LOOSE STRIKING HIM IN THE LEFT ANKLE CAUSING A PUNCTURE WOUND.
EMPLOYEE WAS REMOVING A TRANSMISSION COVER PLATEON A CAT D8L. HE WAS USING A SMALL COMBINATION END WRENCH. AS HE PUT PRESSURE ON WRENCH IT SLIPPED OFF AND WHEN IT DID HIS RIGHT ELBOW STRUCK THE SIDE OF FRAME. ELBOW SWELLED UP AND SOME FLUID BUILT UP. WILL RETURN TO DOCTOR AT END OF MONTH FOR CHECKUP.

EE WAS SERVICING HIS TRUCK. A ANCHOR LOCK NEEDED TO BE REPLACED BECAUSE IT WAS LEAKING AIR. AFTER ANCHOR LOCK WAS REPLACED EE WAS BENDING OVER TO PICKUP THE ANCHOR LOCK. AS HE BENT OVER EE'S B ACK WENT OUT OF PLACE. EE IS A VERY LARGE MAN & IT IS HARD FOR HIM TO BEND OVER. THE ANCHOR LOCK WEIGHS ABOUT 4 LBS.
EMPLOYEE WAS REMOVING AC UNIT. HE COMPLETED HIS WORK AND WENT TO CLOSE THE HOOD. AS HE CLOSED THE HOOD EMPLOOYEE SLID HIS HAND ACROSS A SMAP THAT WAS SHARP. THE SNAPS HOLD ON THE WINTER FRONT COVER. EMPLOYEE SUSTAINED 2 SMALL CUTS. EMPLOYEEHAD 1 STITCH IN EACH CUT. HE WENT BACK TO WORK IMMEDIATELY AFTER DOCTOR VISIT.
EE WAS WORKING ON A CAT A-FRAME. HE NEEDED TO ROLL FRAME OVER SO HE HOOKED A-FRAME TO A CHAIN HOIST & PROCEEDED TO LIFT & ROLL FRAME OVER. AS HE SET FRAME DOWN ON THE FLOOR, HIS ONE FOOT WAS I N A POSITION THAT IF THE FRAME ROLLED BACK IT WOULD PIN HIS FOOT. THIS IS WHAT HAPPENED WHEN THE CHAIN WAS LOOSENED. A-FRAME WAS NOT SET & BLOCKED PROPERLLY. EE BRUISED TOP OF LEFT FOOT.

EE WAS UNLOADING A LOAD OF PIT ROCK OUT OF HIS TRUCK BOX. HE HAD RAISED THE BOX A LITTLE TOO HIGH CUASING A LARGE AMOUNT OF THE LOAD TO SLIDE OUT. TO CORRECT THIS SITUATION, EE HE QUICKLY WENT TO MOVE THE HOISE CONTROL LEVER FORWARD, HE CAUGHT FORWARD TO SLOW THE FLOW RATE OF THE ROCK. ASHE WAS MOVING THE LEVER FORWARD HE CAUGHT THE SMALL FINGER ON HIS RIGHT HAND ON THE MIDDLE PUP

EMPLOYEE OPERATES THE TIRE TRUCK FOR SCC. THE TIRE MACHINE IN THE SHOP WAS TEMPORARILY DOWN AND REPAIRS WERE ON ORDER. HE CONTINUED HIS WORK MOUNTING 11R 22.5 TIRES MANUALLY ON THE CONCRETE FL OOR. THE TRUCK RIM WAS LYING FLAT ON THE FLOOR AAND AFTER PROPERLY LUBRICATING THE RIM WENT TO PUSH THE TIRE ON THE RIM. IN DOING SO HE CRACKED A SPUR ON HIS RIGHT KNEE.
EMPLOYEE IS A WELDER AND WAS LINING A LOADER BUCKET WITH 1/4" STEEL. HE HAD POSITIONED THE STEEL IN PLACE ON THE BOTTOM OF THE LOADER BUCKET. USING A PORTA POWER AND 1" ANGLE IRON EMPLOYEE PU T DOWNWARD PRESSURE ON THE STEEL PLATE. AS HEAT WAS APPLIED TO THE STEEL EMPLOYEE TOOK A 10 LB. SLEDGE AND HIT THE STEEL PLATE. AS HE DID SO THE ANGLE IRON POPPED LOOSE AND HIT HIM IN MOUTH.
