EMPLOYEE IS ON HAULAGE DRIFT REPAIR AND HAD MADE ROOM TO STAND A STEEL POST. I NAME LIFTED THE STEEL POST TO SET IT IN PLACE, FELT A PAIN IN THE UPPER BACK.
AT START OF SHIFT WOOD SHAVING FISCOVERED ON 2-2 STATION AFTER 1ST MANTRIP WAS MADE IMMEDIATE SHAFT INSPECTION REVEALED 3 DAMAGED GUIDES. 21 PERSONS WERE UNDERGROUND. CREW WAS THEN EVACUTED FR OM MINE THRU ESCAPEWAY TO OTHER MINE SECTION 35 CAGE SHOE ON NORTH SIDE OF CAGE SLICED 3 GUIDES AT SETS 111, 202, & 211 DAMAGED. REPAIRS MADE AND SHOE CHANGED
AT 4:15 PM EMPLOYEE STARTED CLIMBING THE 3014 MANWAY CARRYING A P.O.C.I. ON A STRAP OVER HIS RIGHT SHOULDER. AS HE WAS PASSING THROUGH THE SECOND LANDING, HIS HAND SLIPPED OFF THE RUNG AND HE FELL APPRX. 25 FEET TO THE FIRST LANDING. HE HIT WITH MOST OF HIS WEIGHT ON HIS LEFT LEG CAUSING FRACTURES OF THE KNEE AND FOOT. THE PROBE WHICH WAS FALLING WITH HIM PUNCTURED HIS RIGHT FOOT,
EMPLOYEE PACKING PUMP. HAD FINGER BEHIND PACKING GLAND WHEN HE STRUCK THE GLAND WITH HAMMER.
WHILE GOING FROM UNDERGROUND STATION TO SURFACE, ON CAGE A FOREIGN OBJECT ENTERED HIS RIGHT EYE, IMPAIRED VISION, SCRATCHED EYEBALL.
THE MAN STEPPED DOWN ON THE RAIL WHEN DISMOUNTING FROM THE MANTRIP MOTOR TO THROW A SWITCH AT THE STATION AND 0900 INTERSECTION. AS THE MAN S WEIGHT CAME DOWN ON HIS RIGHT FOOT, IT TURNED ON T HE RAIL TWISTING THE ANKLE.
EMPLOYEE WAS UNLOADING POWDER CAR AT THE M & E. HE PULLED A BOX OF POWDER OUT OF THE CAR AND HIT HIS ELBOW ON THE DOOR OF THE CAR. EMPLOYEE BRUSISED HIS FIGHT ELBOW. EMPLOYEE GIVEN A REPORTABL E PRESCRIPTION BY PHYSICIAN. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 05-24-84.
EMPLOYEE TWISTED HIS LOWER BACK. HE WAS CARRYING A BOX OF POWDER THROUGH A LOW OVERHEAD DRIFT TO THE BLASTING SITE.
MONTOYA WAS SLUSHING ON #2-3 DRUM JOY SLUSHER. THE SLUSHER BUCKET GOT HUNG ON A BOULDER. MONTOYA WAS TRYING TO PULL THE BUCKET IN THE SLUSHER BEAMS PULLED LOOSE ON THE BACK BOLTS. THE SAFETY SLINGS ALSO BROKE THE RINGS ON THE SPLIT BOLTS HOLDING DOWN THE SQUARE WASHERS THAT SECURED THE SAFETY SLINGS. THE SLUSHER TURNED & THE END OF THE BEAM HIT MONTOYA ON THE LEFT ELBOW CAUSING
SPRAGING ON ORE CAR ON THE TRACK. THE LAGGING SLIPPED AND STRUCK THE EMPLOYEE CUTTING HIS UPPER LIP.
THE EMPLOYEE WAS UNLOADING 10X10 TIMBER FROM A GLAT CAR IN 1300 DRIFT. HE CAUGHT FINGER BETWEEN TWO PIECE OF TIMBER, CHIPPING THE BONE ON HIS LAST JOINT OF HIS LITTLE FINGER.
HELPING HOIST TIMBER UP RAISE ,PIN HOLDING SHEAVE BLOCK CAME LOOSE AND CABLE AND SHEAVE BLOCK CAME DOWN RAISE.ATTEMPTED TO GET CABLE AND SHEAVE BLOCK UP TO TOP OF RAISE,SMALL SLABS DROPPED FRO M BACK AND HIT ON HIS BACKSIDE RESULTING IN SORE BACK AND RIB CAGE.MED ATTENTION-2-15-84.
OTHER EMPL CLIMBED 4903 MANWAY TO OPERATE TUGGER TO HOIST 5 GAL BUCKET OF TAR TO SEAL BULKHEAD.EMPL PUTTING BUCKET ON TUGGER CABLE.CLEVIS HAD CAUGHT ON BOTTOM OF RAISE LINER WHICH WAS TORN AND JAGGED.WHILE HOSE WAS FLIPPING TUGGER CABLE BACK AND FORTH TO LOOSED CLEVIS,IT CAME LOOSE AND FELL DOWN HIT EMPL DIRECTLY ON MOUTH.
SCREW ON HOIST MTR CONTACTOR ARM BROKE AND SHUT DOWN HOIST AT 11:13AM.CREW PULLED FROM WORKING PLACE 11:19.REPAIRS COMPLETED AND SAFETY RUN MADE 12:21PM.HOIST DOWN AGAIN 9:53 CONTACTOR ARM OUT OF ALIGNMENT FROM PREVIOUS REPAIRS-HOIST BACK IN OPERATION 12:21AM.

HOISTING MUCK IN 4.6 TON CAPACITY SKIP COUNTERBALANCED WITH MANCAGE.SKIP HUNG UP ON STEEL GUIDE ABOUT 150\ FROM SHAFT BOTTOM.PIECE OF GUIDE CAUGHT BETWEEN SHOE AND GUIDE.ALL PERSONS REMOVED FR OM WORKING PLACES AND ASSEMBLED AT LUNCHROOM.SECTION OF GUIDE WAS REPLACED.REPAIRS MADE AND SHAFT INSTPECTED BEFORE HOISTING CRIW AT 9 PM.
EMPLOYEE WAS SECURING A SHEAVE BLOCK USED ON THE IDLER IN THE 2-2 TRENCH, STANDING ON TOP OF A ORE CAR. SH SLIPPED HITTING HIS RIGHT RIB CAGE ON THE SIDE OF THE ORE CAR AND FELL INTO THE TR ENCH, BRUISING HIS RIBS. EMP. DID NOT SEEK MEDICAL ATTENTION UNTIL 9-29-83.
HE WAS IN THE MANTRIP CAR ON HIS WAY BACK TO HIS WORK LOCATION, WHEN THE MANTRIP CAR HE WAS IN DERAILED. THE CAR HIT A POST CAUSING IT TO STOP SUDDENLY. I THINK THE WENT OFF THE TRACK BECAUSE HITCH OR COUPLING ON THE WAS BINDING. THE CAUSED THE EMPLOYEE TO RECEIVE WHIPLASH.
WALKING THROUGH LOW SUB-DRIFT AND HIT HEAD ON THE BACK.NECK STARTED HURTING NEXT DAY.
THE ENGINEERS WERE SETTING UP TO PLUMB THE O RE PASS. THEY WERE HOISTING A BAG FULL OF EQUIPMENT BY HAND WHEN IT BECAME TANGLED & COULD NOT BE MOVED UP OR DOWN. SALCIDO CLIMBED DOWN THE ORE PASS & ENTERED THE SERVICE COMPARTMENT & UNTANGLED THE BAG WHICH WAS ABOUT 14\ UP THE RAISE. SALCIDO LOST HIS FOOTING & F ELL TO THE MUCK PILE W'%** & SLID DOWN THE DRIFT. SALCIDO SAID HIS SH
THE EMPLOYEE WAS ON TOP OF THE #320 MOTOR ADDING HYDRAULIC FLUID. THE MOTOR WAS PARKED AT THE ENTRANCE TO THE SHOP AND THERE WERE TWO MOTORS PARKED BEHIND IT. ANOTHER EMPLOYEE STARTED A MOTOR BEHIND HISAND IT ROLLED INTO THE 320 MOTOR CAUSING PADILLA TO LOSE HIS BALANCE AND FALL TO THE GROUND, BRUISING HIS LEFT KNEE.
HE STEPPED OFF A DIESEL MOTOR IN 0700 DRIFT AND LOST HIS BALANCE. HE FELL TO HIS KNEES ON A GUARD RAIL CUTTING HIS LEG JUST BELOW THE LEFT KNEE.
RAYMOND PADILLA WAS COMING TO THE STATION FROM 0700 TIMBER YARD WITH #320 DIESEL MOTOR & ONE EMPTY FLAT CAR. TOBY PADILLA, HAULAGE DRIFT HELPER WAS GOING TO 8100 DRIFT FROM THE STATION WITH 5 EMPTY ORE CARS & #319 DIESEL MOTOR. THE COLLISION BETWEEN THE ORE CARS & #320 MOTOR HAPPENED ON THE CURVE NEAR THE MECHANIC SHOP, ABOVE 200\ FROM THE 0200 TRAFFIC SIGNAL KNOCKER. BOTH MEN CLA
EMPLOYEE WAS SITTING IN THE MANTRIP CAR WHILE THE MOTOR WAS BEING SWITCHED AROUND THE MANTRIP CARS JUMPED THE SCOTCH CHAIN CAUSING THE CARS TO RUN INTO A PARKED TRAIN EMPLOYEE TWISTED HIS NECK
THE MINER WAS DRILLING A PINHOLE FROM ON TOP OR MUCK PILE IN A SUB-DRIFT HEADING. HE WAS NOT IN AN UPRIGHT POSITION WHEN HE PULLED MACHINE AND 12\ STEEL OUT OF THE PINHOLE. HE APPARENTLY STA INED HIS BACK WHEN PULLING ON DRILL MACHINE AND STEEL.
EMPLOYEE WAS UNLOADING STULLS FROM CAGE HAD CABLE AROUND STULLS TO BE PULLED OUT OF CAGE. GARCIA STATION TENDER, WAS IN BACK OF STULLS CHAVEZ WAS STANDING IN FRONT OF STULLS. THEY TOLD VALDEZ WHO WAS OPERATING TUGGER, TO GIVE THEM SOME SLACK ON THE CABLE INSTEAD OPF GIVING SLACK HE TIGHTENED CABLE CAUSING CHAVEZ TO BE PINNED BETWEEN STULLS AND CAGE BRUISING HI
