Teichert-Grantline Plant Metal/Non-Metal

Employee was recycling an iron plate out of work vehicle. The plate snagged left glove as they were tossing the plate into the bin, and came down on the tip of left middle finger, fracturing it.

Employee was cutting a 2 inch hose and punctured their leg with a utility knife.

The employee was leaning to pick up a water bottle and hit a bump while operating the scraper. After hitting the bump the seat bounced causing EE to bounce up and cut EE's ear on a GPS bracket.

Employee injured EE's heel while trying to get a crusher part.

A SMUD transformer exploded causing a grass fire near the mine site in the Aerojet easement. The fire was never actually on the mine site but called in for reporting purposes.

Cut hand on roller that wasn't machined smooth, had a razor sharp edging. Cut required 6 stitches.

Conditions were sunny and clear. Employee was doing repairs to West "P2" Conveyor when he reached on to the frame and at same time a large cobble fell off the belt onto his right ring finger. Employee heard a popping noise and right ring finger began to bleed.

Employee was doing repairs in outside in the Plant during hot weather conditions.

Weather and surrounding conditions were clear. The employee at the time of injury was changing out roller on conveyor #16 and #18 which required bending and lifting.

Employee approached job in progress and was struck under chin with the end of fork-lift causing a small cut requiring several stitches.

Picking up ladder, twisted and felt pain in his back.

While installing spray bars on screen one, he hit elbow on handrail which caused goosed egg on elbow. Swelling never went down.

EE WAS CUTTING SKIRT RUBBER. THE BLADE SLIPPED, CUTTING EE'S UPPER RIGHT THIGH.

EMPLOYEE WAS CUTTING A HOLE IN A RUBBER BELT, HIS HAND SLIPPED CAUSING HIM TO STAB HIMSELF IN THE LEFT THIGH WITH A BELT KNIFE.

EE WAS SHOVELING UNDER TAIL PULLEY ON CONV. #1. HE WAS DISTRACTED, THE SHOVEL CAUGHT THE TAIL PULLEY, HIS HAND WAS SLAPPED BETWEEN HANDLE AND CONCRETE.

EE WAS TICKETING TRUCK, YELLOW JACKET FLEW INTO THE OFFICE & STUNG EE ON UPPER THIGH.

EMPLOYEE WAS WORKING IN AREA TO ADD WATER FROM WATER TRUCK TO HOLDING TANKS. EE SUFFERED FROM AN ALLERGIC REACTION TO BIO-MEDICATION SOLUTION.

EMPLOYEE DISMOUNTED THE CAB OF THE WATER TRUCK TO OPEN THE BACK GATE. HE DIDN'T NOTICE SMALL COBBLES, STEPPED ON A COBBLE & TWISTED HIS ANKLE & FELL ONTO HIS BACK.

HIT THUMB WHILE ADJUSTING #2 SANDSCREW.

EE WAS STANDING ON LOADER BUCKET CLEANING GRIZZLY HOPPER. HE TRIPPED AND FELL INTO THE GRIZZLY.
