While swapping and cleaning Niro filters per schedule, employee was using improper P.P.E. and not following proper procedures, exposing skin to potential hot slurry spillage. EE opened the filter lid before closing off inlet/outlet valves resulting in hot slurry to land on the back of EE's neck causing a burn.

Employee was locking the deck back in place after changing screens on 3-3 green screen using the manual locking bar. Upon applying downward pressure using EE's right arm the employee felt a strain in EE's pectoral muscle.

Employee mounted a tire on a rim. They then placed a ratchet strap onto the tire & applied pressure to help seat the tire. The tire was inflated which applied additional pressure to the strap. Upon releasing the strap, the employee's glove got entangled in the gear of the strap & subsequently caused a laceration to their left middle finger.

As employee was walking on the bottom floor of the main building on L3, they stepped over a small berm to go up the steps to the Niro, lost balance and fell. At the time of the accident the employee didn't think medical attention was necessary.

Wet, slippery floor due to maintenance being performed, breaking open a slurry line which then created a slip, trip, fall hazard. Injured employee lost footing and fell hitting head- concrete floor requiring medical treatment. Accident -policy update retraining employees on Hazard Recognition & Workplace Examination with instructions for slurry pump maintenance.

Washing off screens on Line 2 blunger with pressurized hose. A splatter of slurry went in eye, caused a burn to the cornea. Not reporting to supervisor in a timely manner and due to possible lack of eye protection and lack of proper flushing /cleaning of eye after accidental exposure. Lack of involvement of first aid trained/qualified supervisor.

Employee was cleaning a plugged lance and positioning lance to install back into the niro hit a light fixture above. Hitting the fixture with the lance caused the globe/dome to break and fell on the back of the worker therefore, injuring EE. PPE required for the task was used. The worker received a laceration on the back and right arm.

Pulling lances out of Niro 5 pulling with right hand and was placing left hand under the lance to grab a hold of it, the lance slipped out of right hand smashing the left index finger.

Changing out a slurry transfer pump and lifting to another location EE felt a pain in lower back and right leg. Slurry was on the floor which could cause a slippery surface.

Calibrating weigh scales in DSI silo. Weights were being added to calibrate the scales / weights typically are not secured as it would add weight/ weight not being lifted/removed slipped off ledge falling onto employee's right foot.

Accident:Hooking bag straps onto forklift forks. When moving the forklift in place to lift a supersack bag the employee had his hand pinched between the forks and the straps of the supersack bag. This caused his first two fingers on his right hand to be cut. Employee was using all required PPE at the time of accident. ie-gloves.

Valve was tied off to hold it in it's position and when trying to untie the rope, the last knot failed causing the valve wheel to spin unexpectedly. Valve was under more pressure than anticipated. Facial contusion/trauma, right facial swelling without loss of consciousness. Closed fracture of zygomatic arch.

Clearing of 2D silo discharge gate watching for the material to free up and had safety glasses on at the time.

Laceration to pinky finger (Fifth Finger)- Left hand requiring stiches. Employee was knocking off lances on Niro 7. When closing the door on Tank 6 on the burner side, the door pulled out of his hand due to the negative and smashed his little finger on his left hand. Employee did have on all of his PPE at the time of incident.

Breaking down electric manlift at end of job. Employee was moving the lift backwards and stated he had the pipe in view. He released the pedal and the lift drifted and wedged him between the pipe and control panel.

Miner was disconnecting lanyard from tie off point when snaphook on the lanyard struck him in the mouth, causing a laceration to his lip, resulting in 4 stitches to his lip.

Clearing material from inside of the niro lance after shutdown to prevent the lance from becoming plugged during startup. The operator was raising and lowering a lance to remove buildup. While lowering the lance back into the niro the operator did not remove his left hand from underneath the lance collar and smashed his left index finger.

Employee reported that he had a sharp pain in his groin area along with a knot that was sticking out. He wasn't sure what caused it to stick out. He was doing his normal job duties with no major or heavy lifting.

Non-work related ailment, employee stopped taking blood pressure medication causing his blood pressure to be unstable.

Working in A103 injury removed gloves. Gloves soaked. After removing gloves he started rolling up a hose.Pulled hose with right and grabbed with his left hand & ran his fingers over the hose clamp which caused a cut to his left middle and ring finger. After shift he went to local emergency room and the middle left finger received stiches with no limitations.

Employee was removing a canister-type filter from service for cleaning. He mistakenly mis-aligned the values resulting in the filter canister remining on-line when he attempted to disconnect the canister. Hot kaolin slurry (170 degree F) sprayed onto his chest, arms, neck and face producing 1st and 2nd degree burns.

Operator was attempting to open a ball valve, the ball valve was very tight and operator used his shoulder against valve handle to assist in opening. Operator sustained a bruise to the shoulder.

Employee had completed typing the rebar for the foundation shab and was walking across the rebar mat to leave area. As he approached the outside form, a wall tie that had not yet been welded to the rebar mat, caught his boot or pant leg. Causing him to trip and fall. He hit the form with his knee and his elbow landed on asphalt paving.
