Woodworth Aggregates Metal/Non-Metal

Employee had started the engine on the water pump and went to engage the clutch. When clutch was engaged the coupling broke. As the coupling came apart, it distorted the shape of guard, pinching his thumb between the clutch and guard.

Employee was cutting crown bar rubber at the 2-deck shaker (cutting excess off), as he was cutting the blade cut his knuckle on his left thumb. We took him to the clinic for medical attention, the doctor put stitches due to the location of the cut not the depth.

Employee was preparing to change the screens on the desander. While cutting the red stops off, the knife slipped and cut his finger on his left hand. He was taken to the clinic for medical attention.

EMPLOYEE WAS PREPARING THE DREDGE TO BE PUT ON SHORE FOR REPAIRS. SOME TYPE OF FOREIGN OBJECT FLEW INTO HIS RIGHT EYE. UPON SEVERAL ATTEMPTS OF WASHING THE OBJECT OUT, HE DECIDED TO WAIT UNTIL HE GOT HOME TO GET IT OUT. THE NEXT MORNING HIS EYE WAS SWOLLEN AND WE TOOK HIM TO THE CLINIC FOR MEDICAL ATTENTION

EE HAD PICKED UP AN ACETELYNE BOTTLE TO PUT ON THE WORK BARGE TO DO SOME MAINT. WORK ON THE DREDGE. WHEN HE TURNED TO PUT THE BOTTLE ON THE WORK BARGE HIS LEFT HAND CAME INTO CONTACT WITH A PAD EYE WELDED ON TOP OF AN ADJACENT PONTOON. THE FORCE OF THE CONTACT CAUSED HIS LEFT INDEX FINGER TO SMASH & BUST OPEN. HE WAS TAKEN TO THE HOSPITAL WHERE 10 STITCHES WERE PUT IN

EE was retrieving 1x1 screens in shaker box that had fallen during the night shift. While in box, EE slipped and fell with left elbow absorbing the blow. Slopes of the box and dampness were causes of fall.

EE WAS CHIPPING ON A PIECE OF METAL & A SMALL PIECE OF WELDING RESIDUE SOME HOW FLEW, HITTING EE IN THE CORNER OF HIS RT. EYE.

EMPLOYEE WAS ATTEMPTING TO RETRIEVE SOME PIECES OF IRON FROM SCRAP PILE WHEN HE FELT A STING ON RIGHT ELBOW, LOOKED DOWN TO SEE SPIDER CRAWLING OFF.

EMPLOYEE WAS LOADING FRONT END LOADER. HE TURNED TO LOOK BEHIND HIM AND FELT PAIN IN HIS NECK.

STOCKPILING SAND AT MID STATE SAND & GRAVEL PLANT. EMPLOYEE HAD RAISED TRUCK BED TO DUMP A LOAD OF SAND WHEN THE TRUCK TURNED OVER CAUSING THE EMPLOYEE TO SLAM AGAINST THE PASSENGER SIDE DOOR.
