Yard 95-Manteno Metal/Non-Metal

Employee was splicing belt. EE was placing board underneath belt to help support for cutting. EE flipped the rubber belt over board and felt a shocking pain in finger tip on three lower fingers of right hand. Employee was feeling pins and needles in those fingers, employee then started feeling pain in their right thumb.

At 1:00 pm the employee was cutting new shim plates for Jaw crusher, as EE was cutting with the Torch the sparks from the cutting caught on EE's left pant leg, it caught on fire and when our employee noticed EE quickly put it out with water.

EE was cutting skirt rubber in a tight spot with a utility knife. EE was cutting towards self. Holding the rubber taught with EE's left hand, EE's right hand slipped and EE lost control of the knife and cut EE's forearm, right above where EE was wearing cut proof gloves - transported to emergency room where EE received 3 stitches.

Employee used the 980F as transport from C4, where employee was working, to the employee break room up top. At approximately 2:30am, the employee hit a bump & said the seat "bottomed out" & employee's back crimped. Employee initially thought employee was ok but, when employee tried to move a troughing roller, employee realized that employee couldn't do it because back hurt.

2 employees picking up a piece of steel (aprox. 100lb)-last of 8 that they were standing up. Come a long was used to assist employees with moving steel. Employee heard a "pop" in elbow. Pain through forearm. 3/2 went to Physicians and diag. with elbow strain and lateral epicondylitis - no restrict. Went to Riverside 3/9 - restrictions: <10 lb 4-6 hours, no push/pull, grip 1-2 hrs.

EMPLOYEE HAD GOTTEN DUST IN LEFT EYE WHILE WORKING IN SURGE TUNNEL

Lifting a quality control bucket and felt strain in right arm.

Approximately 10 inches of snow had fallen over the previous 24 hour period. Employee walked out of break room and slipped on an icy patch falling forward onto her arms.

While checking wheel lugs with torque wrench, wrench slipped, spun around and hit employee's face.

Employee was attempting to transfer lanyard from structure to the basket of a boom lift and fell approximately 14'.

Hurt back while helping change lower deck screens.

EXITING LOADER W/O 3-POINT CONTACT & MISSED THE STEP ON THE LADDER, HE FELL TO THE GROUND.

Operating Cat 992 loader.

SLIPPED WHILE PUTTING SAFETY PIN IN CONVEYOR LIFT CYLINDER CAUSING DAMAGE TO THE KNEE AREA.

BACK STRAIN DEVELOPED FROM USE OF CONSTRUCTION AIR TRACK HAMMER.

OPERATOR WAS OPERATING A HAND OPERATED AIR DRILL, - THE EXHAUST AIR WAS CONTAMINATED WITH EXCESSIVE AMOUMTS OF AIR COMPRESSOR AIR & WAS BREATED IN WHILE OPERATING THE DRILL.

WHILE REMOVING A CRACKED PALTE FROM 475 LOADER A HOT CHUNK OF METAL FALL UPON OXY LACY HOSES BURNING THRU HOSE CAUSING A FLASH FIRE INJURED JUMPED BACK OFF OF STEP LADDER THUS LANDING WITH ANK LE UNDE HIM CAUSING SPRAIN.

THE SUBJECT PERSON WAS WORKING UPPER PLANT MAINTENANCE ON A RAINY DAY WHEN LIFTING HE HAD A FOOT SLIP OUT CAUSING TO STRAIN HIS LOWER GROIN AREA

BLAST SHIELD WAS BEING PLACED INTO POSITION. HE WAS ON L.H. SIDE PULLING BOTTOM INTO PLACE, CHAIN UNHOOKED, LETTING SHIELD TIP FORWARD TOWARDS LOADER, HE WAS PUSHED ASIDE & DOWN BY SHIELD FOLD ING WORKER FORWARD THUS INJURING BACK AND SIDE AND SIDE AREA.

A CHANGE OF SCREEN CLOTH WAS BEING DONE IN THAT ACT ONE OF THE 5' X 6' CLOTHS SLIPPED WHILE BEING MOVED IN PLACE CAUSING THE INJURY TO RT RING FINGER. SOME BLEELDING, AFTER FIRST AID, THE MINE R DECIDED TO SEEK HIS DOCTOR FOR FUTHER TREATMENT.
