The EE was performing services on the AC condenser of a 770 Caterpillar haul truck. The EE right hand slipped during repair and came into contact with the condenser fan causing a laceration to their right pinky finger that required stitches.

Mostrando los 12Employee was walking around secondary area when they slipped falling forward. They stated that when they were falling hard hat fell off and strike their head. They had a open wound on their head and was taking to the urgent clinic for further evaluation.
EE hot the drill on the left side corner. At the time of accident there were no injuries and EE did not claim any illness. Three days after the accident EE called about their back hurting. EE then went to the doctors and claimed workers compensation and they declined it.
Employee became ill due to heat stress and was taken to urgent care.
The ground conditions were wet due to rain. The employee attempted to grease the front bucket of a front end loader by climbing onto the bucket. The employee slipped and fell hitting left shoulder and neck area. The employee suffered a strained/sprained left shoulder and a strained/sprained neck.
The employee was standing on the catwalk of a shaker. EE was attempting to hand one end of an extension cord to a welder and placed one foot on the mid rail. After handing the extension cord, the employee lost footing and fell over the rail. The employee suffered contusions and swelling to back.
He dismounted the dragline and walked to his truck. On the way he stepped on a rock and his foot slipped. The damage was not noticeable for several weeks.
HE WAS INSIDE THE ROCK CRUSHER TRYING TO UNSTOP IT AND ROCK ROLLED DOWN INTO THE CRUSHER AND MASED HIS LEFT INDEX FINGER ON ONE OF THE HAMMERS.
A HOIST CABLE BROKE. IT CAME INTO THE CAB OF A DRAGLINE ONTO THE OPERATOR. THE CABLE HIT HIS LEFT LEG, KNEE AND RIGHT HAND.
INJURED WAS AFENDING TO CRANE LIFTING A HAND RAIL. WHEN HE PULL ON THE CHAIN, WHICH SLIPPED UPWARD PINNING HIS LEFT THUMB AGAINST THE HANDRAIL. INJURED PULLED THUMB FREE. IN PAIN INJURED PULLE D GLOVE OFF, FOUND LACERATION AND SWELLING TO LEFT THUMB.
EE STATED HE STRAINED HIS LOWER BACK WHILE DRIVING HAUL TRUCK OVER ROUGH GROUND BUT DID NOT CLAIM INJURY UNTIL THIRTEEN (13) DAYS LATER AFTER USING SHOVEL FOR A WEEK.
INJ WAS CLIMBING DOWN STEPS OF HAUL TRUCK. HE MISSED BOTTOM STEP, AND WHEN HE CONTACTED ROUGH TERRAIN, HIS RT ANKLE TURNED, CAUSING PULLED LIGAMENTS
