Employee stated was drilling hole in 4L cc. While installing finishing steel in the hole, a piece of rock fell pinching right thumb between the rock and pusher steel causing a crushing injury. EE was transported to Welch hospital. EE was transferred to RGH where EE was admitted overnight. EE underwent surgery and was released the next day. EE remains off work at this time.
Miner was walking outby the section load point in the #3 Entry when a slab of rock fell suddenly from the mine roof striking miner on top of shoulder.
Employee stated EE was installing a 6' resin bolt when a section of draw rock fell from the mine roof striking the tip end of EE's left hand ring finger. The rock measured 18" x 18" and approximately 3 inches in thickness.
Employee was squaring end of belt for splice when a piece of rock fell from the top striking them on the left arm. EE was administered 1st aid and transported to surface and then to hospital ER. Diagnosis was broken left ulna and laceration. EE fracture & wound was treated in surgery and was admitted to hospital.

Employee was bolting top in the #6 heading. As EE was removing drill steel out of the hole, a golf ball sized rock came down with the steel striking EE near upper lip. The area being bolted was approximately 10' high. Received 4 sutures.
