The Company was notified on 12/17/2020 that this former employee was diagnosed with pneumoconiosis on 11/4/2019.

Company was notified on 10/7/2020 that former employee was diagnosed with pneumoconiosis on 6/30/2020 by B reader. EE's last exposure to dust was on 4/27/2020 according to the medical documentation I received with the diagnosis.

A roof fall occurred in the number 2 track entry on the number 3 belt at break number 8 at survey station #256. The fall was approximately 20 feet wide, 55 feet long and 6 feet high. No equipment was involved and no injury occurred.

A roof fall occurred one crosscut inby survey station 220 in the #3 entry on the #2 conveyor belt. The fall was approximately 60 feet long, 22 feet wide and up to 4 feet high. No injury occurred.

Employee was re-positioning self inside the deck of the roof bolter at which time the roof bolter teetered from running over a rock, striking EE's right middle and index fingers between the deck canopy and the mine roof.

We were notified on 4-28-2020 that employee had been diagnosed with pneumoconiosis on 4/22/19 by a B Reader on 4-1-2019.

EE was holding the handle of a hammer that was used to brace the conveyor chain that was being put together, another employee was knocking the strap on when a piece of metal came of the strap penetrating EE's left arm, we were informed on 4-27-20 that EE had the metal removed and received sutures upon returning back to work on 4-27-20 since being furloughed on 4-3-20.

Company was notified on 3-27-20 that employee was diagnosed with occupational pneumoconiosis

Employee was riding the outby end of the #9 mantrip when the operator failed to stop at the airlock doors at the 5 head and caught employee's foot against the grating on the mantrip and the door causing 3 broken toes.

Employee was hanging curtain in the face when EE seen the rib begin to move, EE instinctively put EE's hand up to keep it from hitting EE. The rib bent EE's wrist back resulting in a fracture that requires surgery.

Employee was setting a timber when a small thin piece of rock measuring 4" x 3" fell hitting EE on the right cheek causing a cut resulting in 5 stitches.

This office was notified on 06/26/2019 that this individual was diagnosed with occupational pneumoconiosis on 03/28/2019.

This office was notified on 06/21/2019 that employee was diagnosed with Occupational Pneumoconiosis on 05/12/2019

The employee and several others were recovering high voltage cable from the 1st left panel freeze fork when EE's foot was caught under the high line sled as it was being pulled by a scoop.

Removing a cam roller from 72" structure turned body to lay roller on ground, right knee hurt. Testing shows osteoarthritic changes, medial joint narrowing. EE was taken to ER on 4/30/19 released back to work, went to another ER & hasn't worked since 4/30/19. Prior knee Sx. Ask this be deemed an occ. Ill. not lost days due to no single event caused pain, history of knee problems.

EE was spraying an intake stopping with a TC Spray machine when the machine got stopped up with debris. When the debris broke free the wand swung around spraying the TC Spray sealant under the EE's glasses & hard hat. The material got into the EE's left eye causing discomfort & swelling. EE was prescribed eye drops & returned to work on EE's next regularly scheduled shift.

This office was notified on 04/03/2019 that EE was diagnosed with occupational pneumoconiosis on 12/06/2018.

On 01/28/2019 this mine was notified that this employee was diagnosed with Occupational Pneumoconiosis.

EE was checking splices on the overland belt, when EE lost EE's footing and got EE's undershirt sleeve caught on a piece of tin that covers the belt line. While EE's sleeve was caught by this tin EE jerked EE's arm back to free EE's shirt causing a laceration to the right forearm. Area was slick and wet due to temp and moisture. Belt was not running at the time of accident.

EE squatted down to look for an oil leak on the shuttle car, when a piece of draw rock fell striking EE on the hard hat and shoulder. This caused EE to cut EE's forehead. Required seven sutures.

Loading track, member was helping load track rail onto a Fairchild 35C scoop. Load slid hitting employee in the right knee.

Employee was tearing down a wall with a sledge hammer and hit hand causing an injury.

Employee was tramming a supply motor around a curve, the motor derailed causing EE to bump chin on the dashboard of the motor. EE returned to work next scheduled shift and worked until pre-scheduled vacation of 8/21/2019 - 9/03/2019. This office was notified on 10/07/2019 that during this time EE had surgery on 8/21/2019 to correct TMJ, making this a reportable injury.

Claim filed for pneumoconiosis on 9/18/2017. X-ray read 04/04/2018. We received notification on 04/26/2018.

This office was notified that EE was diagnosed with Occupational Pneumoconiosis on 4/17/18. EE is currently employed at this mine.

Employee was installing the last inside bolt in 5-right, when the canopy of the roofbolter came down striking in the head.

Employee was in the process of removing a cover panel from the continuous miner and employee's right, ring finger got caught between the handle and the panel itself, mashing finger.

Employee was throwing a slate bar onto the continuous miner and a bur on the bar caught employee's hand causing a laceration to employee's index and middle fingers that required sutures to close.

Employee was attempting to put the forks back on a Caterpillar 930H loader that had fallen off while picking up 6' belt structure. While trying to slide the bottom of the fork into place the top came out of the groove and overturned, striking the employee on left leg, fracturing the tibia and fibula.

Rock had fallen from the working face into the conveyor of the continuous miner and was then loaded into the shuttle car. The rock then fell off the shuttle car, striking the employee's left leg, causing fractures to the femur, tibia, and fibula.

A non-injury roof fall, approximately 30' long and 20' wide occurred in the #7 entry at survey spad #11536.

While cleaning on a CM conveyor motor, a piece of draw rock approximately 3" in size fell hitting employee on the wrist resulting in a laceration requiring 6 sutures.

Two employees were laying track rail, one on each side. When one employee flipped a track rail over onto a track rail that was connected to the track tie the injured employee was in the process of lugging to the rail on employee's side, employee's hand was caught between the track tie and the mine floor.

Employee was removing a rock from the continuous mining machine being operated when a finger became caught between the continuous miner and the rock resulting in a laceration requiring 6 stitches.

An unintentional roof fall occurred outby the 001 MMU in the #3 entry in-between spad #'s 18483 and 18428. The fall measured 50' in length, 18-20' wide and approximately 6' in height. There were no injury or equipment involved with the fall.

Employee has been off work since 08/17/2015 for a work related incident. On 09/29/2016 this mine was notified that employee has been diagnosed with Occupational Pneumoconiosis.

Unplanned roof fall occurred at cross cut #21 on 10 Belt in the primary escape way in the old #6 entry.

Employee was changing a center roller on a top structure when the slate bar slipped mashing finger between the bar and the belt structure causing a laceration and broke finger tip.

On 5/16/2016 at approximately 7:00 p.m. EE straightened up after being bent over. EE felt a burning pain in the right side of lower back. EE didn't know what happened it just started hurting when EE stood up. EE finished shift and sought treatment. EE was given two days off from work and the diagnosis was degenerative disc disease with herniated disc.

Employee retired from work on 05/01/2009. On 04/19/2016 this mine was notified that this employee has been diagnosed with an occupational illness of pneumoconiosis

As EE was putting some hyd. hoses into a scoop bucket a couple of 9' timbers that were lying in the bucket became dislodged and fell onto EE's right hand. X-rays at the ER were negative for a fracture. EE continued to have swelling so returned to the doctor on 5/17/2016 where X-rays revealed a small fracture and the doctor recommended physical therapy

The employee has filed for occupational pneumoconiosis, we were notified on 12-11-15 but he still is currently employed with this mine and continues to work

Employee filed for Occupational Pneumoconiosis we received this notification on 12-02-15 his last date of work was 10-17-15 due to a reduction of workforce

Employee was using a lifting jack to re-rail a flatcar that had derailed, the jack slipped hitting the ring finger on his right hand resulting in 4 stitches.

on 09-15-15 this mine was notified that this employee was diagnosed with Occupational pneumoconiosis

Employee was attempting to pull cable off the roofbolter that was fouled up; when he touched the cable and the frame of the roofbolter he recieved an electrical shock to his upper body. He was taken to the hospital and kept overnight for observation.

Employee was using a slate bar to pry a rock off the miner when he felt pain in his lower back, he was taken to the ER and given a full release to return to work, he did return on his next scheduled shift but has not returned to work after 08-19-15

EE was walking up to the face when he saw some bad bottom in the roadway, ee was attempting to step across the hole in the road when his foot slipped in the muddy bottom causing his weight to shift resulting in a fracture to his fibular bone in his left leg

EE was standing in the #8 entry looking outby when a piece of the coal rib measuring 86" X 25" X 6" rolled off striking him in the back

Employee slipped and grabbed the lever that raises and lowers the drill pot setting the drill pot down on his knee. This injury happened on 07-27-2015 but the employee continued to work until 11-06-2015.

An unintentional roof fall occurred on the #3 belt line at x-cuts 6-7 at spad number 241-252 The fall measured 6'X 20'X 110'. No airways or escapeways were affected.

Employee retired from working on 01/01/2014. On 06/10/2015 this mine was notified that this employee has been diagnosed with an occupational illness of pneumoconiosis.

Employee had an accident on 5/20/15 he was taken to hospital and released back to work, he continued to have pain, he went back to the ER on 7-6-15 and had more testing he was told to remain away from work until seen by specialist which he did on 8-4-15. We was unaware of him missing work due to the injury on 5-20-15 until he brought us his off work slip.

While traveling to section in battery powered ride the breaker knocked and the ride rolled backwards and into the corner of the rib. Hyperextension of the left foot causing fracture to the 2nd metatarsal bone.

Claim filed for pneumoconiosis. X-ray read date of 2/5/2015.

The employee has filed for occupational pneumoconiosis.

As employee was walking back to section and walking under overcast, he stepped on a half block with his left leg and fell forward catching himself with his right arm against the overcast. This caused his arm to extend backwards, dislocating his right shoulder. The employee began missing work on 4/16/2015.

Roof Fall at Break 1 to break 2 on Rum Creek Travelway,Fall was aprx 8' high and 100 ' long by 18 ' Wide, area has been inspected by both agencies ,roof fall will be cleaned up to re-establish the track travel way.

Bolting in #3entry, had drilled 3'of a 6' hole, he inserted the sectional steel into the hole and was aligning it with the bolt wrench, he bumped the rotation while having his hand on the drill steel to assist in aligning the drill steel with the bolt wrench, this rotation grabbed the kevlar sleeve twisting his forearm around the drill steel causing fractures to his forearm.

Claim filed for pneumoconiosis. Diagnosis date of 12/2/2014. Notification date of 2/5/2015.

Employee was walking across # 8 entry when he slipped and landed on his left knee. He completed his shift and filed a notice only incident report. The next evening he noticed his lower leg was red and swollen. He went to the ER and was diagnosed with cellulites.

We was notified on 8-28-14 that employee was diagnosed with coal workers pneumoconiosis. He hasn't worked at this mine 5-1-2009.

Employee was rolling up curtain when rock fell and hit his shoulder and knocked him down. Employee was treated and received full release at Logan General Hosp. Continued to work until 08/19/2014 where he was seen by Dr.for back pain and an MRI is scheduled. Employee is off waiting on MRI and results. This is being reported due to the Pt.50 10 day rule.

I was notified on 8/8/2014 that employee was diagnoised with occupational pneumoconiosis and we would need to report his illness. His retired from this mine on 12-23-2013.

Employee was helping hang the miner cable and water line, he bent over and picked up the miner cable and felt discomfort in his groin area, he continued to work until 9-24-14 when he was taken off work and has been diagnosed with a hernia and is awaiting surgery to repair the hernia.

Employee was bolting in the # 5 heading on the 002 mmu and his wrench fell off the torque tension bolt he had just installed causing it to bounce off drill the pot up into his cheek causing a laceration which required sutures to close the wound.

Employee was bolting in the #6 entry when a piece of soapstone or middleman came off striking his left leg, causing a fracture to his knee joint. Rock measured 7 1/2'L X 2 1/2'W x 2"to 6" inches thick.

On March 27, 2014, this mine was notified that his employee has been diagnosed with occupational pneumoconiosis

Employee was operating continuous in #6 entry and was throwing cable out of the way when he strained his groin. He was seen at ER and released. On 3-21-2014 he was diagnosed with a hernia.

Employee was driving nails into a 6' belt splice when a 8 pin drive (Knocker) came dislodged and struck him in the mouth chipping his right front tooth and caused his right eye tooth to become loose,

Employee stated he was recovering fly pads and felt pain in his lower back. He was transported to LCMC where CT scan showed bulging disc. Employee stated he had been diagnosed and previously treated for this condition. The LCMC ER. Dr. told him to follow up with his prior treating Dr. for this condition. His appointment is scheduled for 03/05/14.

Employee was operating off side right roof bolter. He was putting up roof bolt and drill pot kicked sideways and bent drill steel. The bent steel struck employee above right temple.

Employee was throwing miner cable out of roadway and felt pain in lower back. He continued to work and did not seek medical attention until 02/18/14, where he was seen at LRMC where he was treated and released. On 02/19/14 he was seen at St. Mary's Occ. Health Center where he was put on restricted duty.

Employee was B bonding the back side of a stopping at Brk. #33 of #1 entry crosscut of 6 belt when a piece of draw rock measuring 46' x 46' x 2" thick fell out between bolts striking him on the back side of both legs.

Employee was helping electrician remove rub rail on miner when a piece of rib fell and struck the right side of his middle to lower back. Employee continued to work until 11-18-2013 where he was taken off work due to his illness.

Employee was attempting to hook chain around SCBA box in scoop bucket. When he asked the scoop operator to swivel the bucket, his left hand finger was pinched between the chain and the hook on the top of the scoop bucket.

Employee was installing a core breaker on the rt. continuous miner. He was striking the core breaker with a flexco hammer when a piece of the core breaker broke off hitting him in the left eye.

Employee was dropping MRS cable down that had a rope hanger on it. When he unhooked the hanger the cable had tension on it and slipped out of his hand hitting him on the nose causing a laceration requiring 3 stitches.

A roof fall occurred adjacent to the #6 belt in the primary intake airway at crosscut #49. The fall measured 25' long, 20' wide and 6' high. No equipment was involved and no injury occurred.

Employee was walking from the flatcar to the garbage hole when his knee went out.

Employee was using a chain ratchet to pull on a shuttle car when the roof bolt broke he was hooked to allowing the chain to strike his ear causing a laceration requiring stitches to close the wound.

We were notifed of this diagnosis on 7-17-2013 and he continues to work

Employee was standing beside the roof bolter when a piece of loose draw rock fell hitting on top of the canopy, sliding off the canopy hitting him on the side of the side of his head above his left ear resulting in 14 staples.

Fall was found on 5-30-13 unknown when roof fall occurred, location was the # 8 entry 1000' feet off the 003 section at spad number 13222. Fall will not be cleaned up area was dangered off and inspected by MSHA inspector.

An unplanned roof fall occurred in the #3 entry at spad # 0241. Approximate size is 22'L X 20'W X 4.5'H.

Employee was trimming a flapper off the 6C belt. While cutting towards himself, the knife went through the belt and into employee's right leg causing a laceration.

Employee worked from 9/8/06 thru 1/23/07. He has been diagnosed with pneumoconiosis on 1/27/13. The company was just recently notified.

Employee was bolting top when a piece of draw rock fell from the roof when he was pushing his bolt up. The piece of rock glanced off the ATRS boom and then striking employee in the mouth.

Employee slipped on a piece of draw rock that was laying on the mine floor causing him to fall backwards. As he fell, he tried to catch himself with his right hand causing him to sprain his wrist.

Employee was exiting the AL Lee forklift when his right foot got stuck under the machine's controls causing him to fall backwards and twist his right knee.

While using a pick hammer to mark a block for breaking, the hammer glanced off the block striking the employee's left hand at the base of the thumb next to the palm. Employee continued to work regular duties up until he had surgery on 12/14/12.

Roof Fall occurred 7/2/12 at section 003 at spad # 11550. 1000' feet from active face. No personal injuries, No Equipment involved. No ventilation or passageways affected. 220'L. x 18' W. x 8-10' H. Entrances have been breakered off.Mining under 300' cover.

An unplanned roof fall occurred in the #3 entry at the ethel return shaft at spad # 7484.

Employee was checking the MRS cable on the offside of the miner when a rib rolled striking his right lower leg.

Employee was helping continuous miner operator hang a cable when it slipped and caught his pinky finger on his left hand.

Employee was attempting to remove a rock that had fell on the track. As he was lifting on the rock, he felt a pain in his back. Employee continued to work up until 5/29/12 and was then taken off work.

Employee was bolting 7R when he attempted to bend a bolt. As he was bending the bolt, he felt a sharp pain in his right wrist.

Employee was trying to put on a scoop tire when his index finger and thumb got pinched between the tire and scoop fender.

Employee was installing a 6' torque tension bolt. As he was straightening the bolt in the hole, he felt a pain in his right shoulder.

Employee was helping build a stopping. As he was handing a hollow block to another worker, he stepped on a block that was lying on the ground causing him to twist his right knee.

An unplanned roof fall occurred in the #3,4,5 entries in the Northeast Freeze Fork Submains inby spad #9687.

An unplanned roof fall occurred in the #1 entry in the Northeast Freeze Fork Submains near spad #9660.

Employee was changing hoses on the right roof bolter, offside boom hinge, had hand up on roof bolter when a piece of rock fell from top striking his right ring finger. Roof showed no signs or warnings of draw rock. Rock was approx 2.5ft X 1.5ft X 2-3" thick.

Employee was coming down the ladder located inside the elevator. As employee was coming down, he slipped. As he went to catch himself, he cut his left hand on the inspection door located in the roof of the elevator.

An unplanned roof fall occurred in the intake along the #7 belt at break #4 in the #6 entry. 4,000' from the active face. Passage was not affected.

An unplanned roof fall occurred along the #2 belt from break 26 to break # 29. 20,000' from the active face.

Employee was getting rock off the continuous miner. As he lifted, the slate bar slipped and the rock slid down striking employee on the left hand.

Employee was lifting on a kennedy stopping. As he picked up on the stopping, the metal cut through his glove causing a laceration on his left middle finger.

An unplanned roof fall occurred on the #7 belt in the #5 entry, 3 breaks inby spad # 7710. The fall measured 25'X20'X8'. No airway restrictions. Lifeline and phone lines are unharmed. Fall area has been dangered off. State and Federal offices have been notified via telephone.

Employee was hanging a curtain in the #3 entry when a rock rolled off striking him on the left foot. Rock was approx. 1.5' X 1.5' X 6" thick.

Employee was on his knees while bolting the #2 entry. He was positioned at his previous drilling location. As the bolter trammed forward, the side of the bolter hit his rescuer that was located on his left hip causing his left knee to twist into the mine floor.

EE was setting feeder on to tail piece on the 002 section. While cleaning out the gob underneath the cats to put the cribs in place the EE slipped and fell forward. EE put his left hand on the raised jack to catch himself. At this time another EE let the jack down and pinched the EE's left ring finger between the jack and the frame.

Member was removing a large piece of rock off of the miner, when a sharp edge of the rock cut through the glove and cut his ring finger on his right hand.

Employee was making a splice on the 6D belt. While lifting a piece of 48" top structure, employee felt a pain in the groin area. Employee immediately visited Logan General where he was treated and released with no restrictions. Employee continued to work regular duties up until 7/14/11. Employee had surgery on 7/14/11 to repair a hernia that possibly resulted on 3/31/11.

Member was reaching a 8" water coupling up to another member, when he did not get a good grip on the coupling causing it to fall back down and smashed his index finger between two couplings. Member tried to work until 2/23/11 when he re-injured his finger on a tall guard.

An unplanned roof fall occurred along the #10 belt in the #3 entry at break #6, 1,700' from the active face at spad #9192.

Member was operating the maint. scoop when some items fell off of it. Member tried to stop welder bonder from falling when it smashed his left middle finger against the frame of the scoop causing a laceration.

Member had left the section walking to the supply car to get some dust hose for the roof bolter, when a rock fell and hit him on the back. Member worked every shift until 2-3-2011 when he was taken off work due to increased pain in his back and shoulder.

Member was installing a fire valve with a sledge hammer, the hammer slipped and struck his left thumb causing a laceration and a fracture to the tip of his thumb.

Member was assisting another member in resetting a timber that had become dislodged. While holding the timber, a piece of draw rock fell striking member on the right arm causing laceration.

The foreman was in the process of removing a belt pin from an old splice with a pair of hammers. EE was assisting the foreman when a piece of metal struck him in the right eye. EE was transported to the Logan Regional Medical Center by ambulance where he was released and referred to St. Francis hospital for surgery.

Member was operating a brookville mantrip along the #9 belt when a rib dislodged and knocked some timber loose causing it to strike the member on the right hand and the ribs on the right side of the body

Member was trying to attach the flinger duster to the bucket of the 442 fairchild scoop, when he slipped and fell. Member hit his mouth on the scoop bucket chipping both front teeth.

Member was loading a conveyor flight into scoop bucket, when it slipped out of his hand smashing his right middle finger between the flight and the scoop bucket.

Member had stopped to hang miner cable out of the roadway, when he was struck by a rib roll

EE was attempting to cross the 5B belt at the 5B flow-thru tail piece and mistakenly turned belt switch to the "on" position and was carried into the tailpiece. His right foot then became lodged between the skirt board and impact roller, causing a sever laceration to his foot.

Injury was cleaning track pick up rock heard a popping sound in back. Back started hurting, continued to work and finished shift-no incident report fill out at mine site.

An unplanned roof fall occurred on 5D belt in the #5 entry 1000' from the face.

Member was getting ready to cut a breaker timber in the #1 entry on the #2 section when a piece of draw rock fell striking him on the back of the head.

An unplanned roof fall occurred on the # 1 section from the # 2 entry to the # 7 entry 80' from the face.

Member was supplying the offside tray of the left roof bolter on the #1 section when a piece of draw rock fell and hit him on the right elbow.

Employee was supplying the tray on the operators side of the left roof bolter when a piece of draw rock fell and hit him on the right forearm causing a laceration.

Member was riding in a mantrip and started up an incline when they lost traction resulting in the mantrip sliding back into a second mantrip. Employee complained of neck pain.

An unplanned roof fall occured on #2 section in the #5 entry at spad #5525 3000' from the face.

Member was cutting down a rope hanger with a utility knife, when the knife slipped and cut his left middle finger.

Member was operating the continuous miner in 5 right, when a piece of rock fell striking him on the right hand and left foot. DCN: 220082760058-We are amending this form based on diagnosis from the dr stating that member had pulled ligament and will require time away from work.

An unplanned roof fall above anchorage on the northeast mains on brk 46 in return. The fall is located 14,000' from face.

Throwing a track switch on track and twisted right knee out of socket.

Member was installing a corner bolt in #7 heading when a piece of draw rock fell and hit him on the right forearm.

Member was using a belt knife to cut a piece of belt to use as a mat for the power center. The knife slipped out of the belt and cut the members left knee.

Member tripped over a rock while loading supplies and landed on his left hand causing a sprain to his wrist. Note: member was not taken off of work by doctor until 7-28-2008.

EE was trying to remove a bearing off of a tailshaft with a hammer, when a piece of metal broke off and hit him in the forehead.

Member was loading a piece of 6" water line onto a scoop. One end of the water line fell off the scoop and bounced causing the other end to strike Member on the chin resulting in a laceration requiring stitches.

Roof fall above the anchorage on the #10 Headgate Panel at Break #40 Survey spad 3740 in the #4 entry. The fall covered two shields and 1/2 of the mule.

An unintentional roof fall was discovered by foreman in old works. Fall measured 20' wide x 70' long x 8' high. Fall located between 40 cross cuts out by #2 section near spad #4605.

Getting chains out of rail, rail fell on right middle finger, cutting the tip.

EE was operating the left shuttle car on the #2 mining unit. He pulled up to the feeder to empty his car when he noticed his cable had came down. He got off of his car to rehang the cable, when he slipped on a rock causing laceration to his right hand.

Employee went up in the #5 entry #3 room on the #1 section to assist in roof bolting when he was struck by a piece of rock that caused a laceration to the forehead requiring 6 stitches.

An unintentional roof fall was discovered by mine examiner in old works. Fall measured 20' wide x 50' long x 4' high. Fall located between spad #4695 & spad #4631.

An unintentional roof fall was discovered by mine examiner in old works. Fall measured 20' wide x 350'-400' long x 8-10' high. Fall located northwest mains 1st left from spad #4561 to spad #4554 then to spad #4557.

Member was helping another member bolt up a fall area when a piece of coal fell striking member above his right eye.

Roof fall above the anchorage on the #1 section 001 at survey station 4617 outby 50 ft. Size of fall was 55 ft. x 20 ft. x 6-8 ft. Area was supported with 48" resin bolts.

Member was operating tail side of shear at shield #85 when a piece of rock fell from the top onto the drum. The drum kicked the rock out and struck member on his left index finger.

Roof fall above anchorage in the #2 entry at or near survey station 4587. Approx. 60' in length and 12' in height.

An unintentional roof fall was discovered by mine examiner in old works. Fall measured 25' wide x 25' long x 5' high. Fall located 4 rt panel @ 3 cross cut rooms, 2 cross cuts from spad #3699.

Roof fall above the anchorage at 1 break inby spad 2215 #3 entry #5 belt conveyor.

While trying to adjust bearing on stationary roller in a take-up, the roller slipped when the bearing cap was removed smashing the middle finger on left hand. This resulted in a laceration to the tip of left middle finger, requiring stitches.

EE was changing inserts in the shear drum when a piece of one of the inserts broke off & hit him in the chin causing a laceration to chin.

A roof fall in the #2 Entry on #11 Headgate section inby S.S. #4479. The fall was approx 90' long and 20' wide and approx 10' in height. This was a neutral Entry along the #11 Headgate section belt.

WAS FUSING PIPE - LIFTED UP ROLLER HANDLE TO RAISE DONE PIPE.

While putting oil inthe LWshear, a piece of rock fell from roof between shield tip to face. Rock struck cutter drum, slide down ranging arm striking EE on left foot.

Member was cleaning garbage off of bolter when he cut his left index finger on a piece of metal.

Member was eating lunch when a piece of drawrock fell and struck his right hand.

Employee was operating shear cutting out in headgate when the shear cutter head propelled a rock into his right hand.

A roof fall occurred app. 4000 ft outby active face between spad #2225 and 2229 in the #5 entry. Fall was approx. 90ft l x 20 ft w x 15 ft h.

Employee was removing a rock from the shear cable & back board. During this process the rock came up & caught his hand between the shield.

Employee had stepped up on an old overcast to take a methane test. As he stepped down off the overcast he stepped on a piece of cinder block & twisted his left knee.

EE was using a bow saw to cut a timber. Saw slipped, causing a laceration to index finger.

Employee was going to cont miner with shuttle car. Hit hole in roadway causing his head to hit the canopy.

Member stepped on diesel tractor to check transmission fluid. When he stepped down, he stepped on a small piece of metal hurting foot.

Member was driving a diesel tractor, the tractor started to bounce, as member hit brake, causing member to hurt right elbow.

Member was working on shield 132. He was crossing shield 130 when he stepped on relay bar and fell hurting his right side and chest.

Member was attempting to close small door, beside equipment doors. He hurt his hand on door hinge on equipment doors.

EE was raising a belt line when chain ratchet slipped hurting his thumb.

Member was walking, checking rollers, stepped in a hole twisting his knee.

Member was attempting to get off shield 125, he slipped causing twisting of leg.

Member was unloading flat car of 8 inch water line. Member got hand caught between water line and track rail, causing bruise.

Member was bending a 6ft bolt. Plate slid down bolt striking member on tooth.

A roof fall occurred 2300' from #2 section in the #1 entry of 10 headgate, inby spade 3719. The fall measured approximately 20' wide x 1 break long x 12' high.

Member hit head on a belt hanger, hurting head and neck.

A ROOF FALL OCCURRED ON THE MAIN #2 ENTRY, SPAD #2227, 7800'FROM ACTIVE FACE. THE FALL MEASURED APPROX. 40'LONG X 30'WIDE X 5'HIGH.

EE was using the shop hoist lifting a getman tractor. He was blocking cribs under the tractor when the chain slipped causing tractor to smash EE's fingers between tractor and crib blocks.

A roof fall outby on #3 belt line on the #1 entry, 1 break inby spad #369. The fall measured approximately 40' lenght, 20' width and 6' high.

A roof fall occurred outby on #2 belt line on the #1 entry, spad #205. The fall measured approx. 60' length & 20' width.

EE was fire bossing #2 head when he slipped and twisted left knee.

A fall occurred on a section of storage unit of longwall belt at spad #3272, approximately 100 ft long and 10 - 12 ft high.

Track entry fell in at 3875 and from tail across to 10 tail fell in.

A fire occurred at the motherbelt storage unit that was not extinguished within 30 minutes.

Member was cutting bottom coal, a piece of rock came from head drum stiking member in the right arm.

Member was on knees, member turned around to pick up tool and twisted his left knee.
