Employee was repairing a poly pipe when some unconsolidated material from the hole employee was working in sluffed off. The material pushed employee's knee into the pipe causing pain. The employee was later examined by a doctor and was not given restrictions until 1/3/2025.

EE struck by falling object. EE was inspecting bucket elevator in plant when light fixture fell striking a glancing blow to EE's hard hat. EE had been treated with first aid until 8/26/22 when physical therapy was prescribed.

EE received 2nd degree chemical burns from high PH material that released from the main conditioning tower during plug clearing. The EE was standing ~10 feet away, material fell from the tower, exited the vessel, hit the deck, and splashed onto EE. The material impacted upper chest, neck, cheek, and forehead. Eyes and mouth were protected by a N95 mask and safety glasses.

Employee was installing shell liners when one of the old liners in the next compartment fell hitting another liner in which a piece broke off, entering employee's right leg, thigh area.

While finishing up working at the finish mill, the roll up door came down on contract employee, hitting them on the hard hat, left shoulder and left upper back. Co-worker saw the door on top of employee and came over to pull the chain that controlled the door movement. Re-latched chain securely after incident and faulty door chain reported to appropriate Ashgrove personnel.

Employee was with a crew removing demo material. EE was outside the hole removing buckets that the crew inside was filling up. As EE reached inside to remove a bucket the crew inside shouted at EE to watch out because a chunk of refractory material fell from side wall above door. Due proximity of the falling material, EE was unable to avoid being struck in the left hand.

Customer driver exited the truck, stood on the back bumper to clean back of truck, when frozen material came down pinning customers leg between body of truck and frozen material. Customer was able to release their leg and remained on the ground for EMS arrival. Customer was taken to hospital. Compound fractures.

Secondary plant chute was brought down to ground level for maintenance on sides. Two temporary legs where attached to lift up. Left leg came unattached causing the chute's right leg to slide off a metal part onto employee's foot. Bottom of temporary leg entered the portion of steel toe boot where steel and boot material meet.

Employee was checking the wear liner on crusher, when he bent down to lift inspection access door an angle iron fell on his shoulder/upper back.

Employee was adjusting screen discharge belt on primary screen. As he stepped back from frame he was observing belt for proper alignment. He was struck in the mouth by a falling rock. Employee states he did not see where rock came from.

EE was cleaning dust build up from a chute, when the wall of the bin - hopper directly above him gave way, then a large amount of dust was allowed to fall onto him pushing him downward into the chute & railing where he was standing.

Worker was clearing chute when rock from above fell and hit right hand. Middle and ring fingers were broken.

While washing out an area of the plant with a water hose, a rock from some unknown area fell and struck him in the right shoulder.

FRONT END LOADER (988FCAT)OPERATOR WAS FEEDING JAW CRUSHER & WAS USING MATERIAL TAKEN FROM AN AREA WHERE THE DOZERS WERE PUSHING OFF FROM THE AREA HAD BEEN PREVIOUSLY BERMED. HOWEVER THEO OPERTOR HAD REMOVED A SECTION OF THE BERM

WHILE LAYING OUT A BLAST, A BLASTER WHEN WALKING WENT OVER A 36 FT FACE, WHEN THE GROUND BELOW HIM GAVE WAY. HE RODE THE MATERIAL DOWN TO THE FLOOR WHERE HIS HEAD STRUCK A ROCK CAUSING INJURY.

WAS STRUCK ON RIGHT HAND BY ROCK THAT FELL FROM CATWALK.

RETAINING WALL COLLAPSED INJURING 2 EMPL.ONE EMPL-BROKEN ELBOW AND FRACTURED HIP.THE OTHER EMPL BROKEN LEG AND HEEL.
