EE was exiting EE's haul truck at the end of EE's shift. When EE stepped down to the ground EE stepped on EE's ankle wrong and fell down. When falling EE's right elbow hit the ground. EE was taken to OMP on 8/17/26 for further evaluation and placed on restricted duty.

Employee slipped and fell against a handrail bruising EE's right ribs

There were no conditions that contributed to the injury

Employee placed their hand on a stationary safety ladder positioned on the forks of a forklift. Forklift operator was instructed to extend the boom. Ladder struck a piece of steel, ladder bounced and struck EE on the left side of head, resulting in lacerations to left cheek and behind ear. EE was transported for treatment.

Employee experienced an ankle sprain when they had stepped over a vacuum hose while performing their job duties.

Employee was cleaning under the conveyor, feet slipped on the rocks and they did a split. Felt pain in groin and rectal area. They thought the pain was a hemorrhoid so they did not report it as work related. They went to the ER on 7/31 and it is not a hemorrhoid, employee needs to see a surgeon.

While applying downward force on a valve an employee experienced pain in their right shoulder.

IE was climbing a fixed ladder on the crusher structure to reach the next landing when a self-closing safety gate above, misaligned and protruding into the ladder access area, struck EE's mouth/face as EE looked up. Impact chipped part of EE's upper right front tooth. IE descended safely and reported injury. Gate was removed from service and repaired/repositioned.

At Ostrander portable EE was using 25-30 lb spud bar to dislodge buildup between conveyers when the bar struck a rock awkwardly. Spud bar continued forward and EE's hand went into the conveyor and crushed/pinched L pinky. EE was not wearing gloves and first aid was performed. EE was given stitches and antibiotics at hospital and diagnosed with tip/nail of left pinky tuft fracture.

While walking the employee tripped on the catwalk grating section that connects the two different platforms and fell forward, landing a knee on the grating. The employee got up and noticed they had a laceration just above the kneecap.

EE was grinding a piece of steel and metal entered EEs left eye. EE was wearing the proper PPE (welding mask and safety glasses) at time of incident. EE utilized eyewash as a first aid measure but was unsuccessful. EE was then taken to urgent care by a Foremen. At the facility, the object was removed from EEs eye with a needle. EE was diagnosed with a corneal abrasion and released.

Employee was climbing the stairs on the preheat tower and called over the radio to a co-worker that they needed to rest and felt dizzy. When the co-worker arrived, the employee was lying on the floor unconscious. Employee was brought conscious by a few shakes and hydrated and took to emergency room for check. They was diagnosed with heat syncope.

Employee was moving scaffold racks with a sky track forklift when EE drove through a pile of CKD dust and it blew into EE's eyes. EE was wearing safety glasses at the time.

Contractor was using the clinker silo passenger elevator when it stopped in the middle of going to the top. They immediately called the Control Room to let them know, and an electrician was called to assist. After 35 minutes the car was lowered to the ground and the contractor was able to get out safely.

Injured was placing wear liners inside of impact crusher. The liner got caught on a ledge and the injured felt discomfort while freeing the liner. Injured was diagnosed with a hernia on 08/04/2026.

Employee was attempting to remove a sump pump through a manhole in the old landfill when they lost grip of the pump and their left middle finger became pinched between the manhole and the pump housing. Employee had a small laceration on the fingertip that required (1) suture to close.

Employee reported losing consciousness while operating a haul truck and struck a highwall. The employee sustained a minor head laceration, and the haul truck incurred minor damage.

Employee was finishing washing out tail pulley with a hose and went to get back into skid steer. When employee swung leg into the equipment, felt a pop in RH side groin area accompanied with severe pain and numbness of the thigh. Employee reached out on radio and returned to shop. Employee was seen by Fit for Work who recommended medical follow-up - suspected hernia.

Employees were removing jacks from under #2 kiln tire (jacks free of any weight). Employee was guiding jack off the base; it came off awkwardly and pushed EE off balance. EE placed EE's right arm out to catch myself and felt a pop in EE's shoulder.

EE was tasked with other employees to change screens. When placing screens on forklift they were off center and slid, pinching EE's finger causing a laceration.

Employee fainted while working in the blend silos. A co-worker found the employee on the floor. The employee was seen by the co-worker just a few minutes earlier. The employee was diagnosed with heat syncope and administered IV fluids.

Employee was shoveling and hurt the back of EE's hand.

EE operates a CAT D8 Dozer on a daily basis which involves repetitive motion. EE experienced discomfort and was taken to the doctor for evaluation.

Employee cleaning dust off ground with air hose and lance, as employee stepped over truck guide, they tripped. Causing the person to make contact with a SafeRack pedestal with right knee.

Employee was pulling on a roller frame when it came out of the conveyor frame. It subsequently hit EE in the face, causing a laceration.

On June 19, 2026, an employee was descending a fixed ladder on a crusher when EE lost EE's balance and fell to ground.

Employee was removing the gearbox on the hydro pump on the pit dredge. When they removed it, it fell on their hand

Employee was working on an airslide that transports kiln feed. When they went to close the port on the airslide kiln feed went up left sleeve and some dust got into right eye. They were given restrictions on 06/26/26 for their left wrist.

Employee was cleaning an oil container out using brake cleaner, just outside the overhead door for ventilation. The wind kicked up and the cleaner went back in EE's face. EE had on a hard hat, gloves and glasses but a little landed on EE's eyebrow. EE was sweating and it dripped in EE's eye. Rinsed it out with eye wash and was doing ok.

Employee was using an impact gun to loosen a carriage bolt on skirt board. Left middle finger made contact with the sharp bolt head, ripping through the cut resistant glove.

Employee reached down into the control box to remove plastic gates to drain water from the pond. While pulling them out EE strained EE's lower back muscle.

Employee was loading out shot rock and was making berms to contain the boulders as they would fall while EE was digging the shot. Operator was digging on a 45-degree angle to get around a high spot in the floor and a massive boulder fell with a rockslide. The boulder jumped over the berm and hit the right front tire of the loader causing damage to the front wheel and the operator.

Strained back while rolling up a conveyor belt on the ground.

Production employee was performing the routine task of sampling from a customer loadout truck. Employee pulled/strained right shoulder. Employee reported incident the following morning. Employee was taken to clinic and given a sling and 800mg ibuprofen. Employee is on restricted duty for right shoulder strain until 6/25/2026.

Employee was cutting a piece of handrail pipe in K2 RM bucket elevator pit and cut EE's left forearm, taken to clinic and received stitches.

When using a chain binder to chain pipe to floats, the employee lost their balance and fell off the pontoon, landing on the ground. When falling they knocked the cheater pipe off the float they were using to tighten the binder, causing it to fall and strike the employee, causing a laceration to the forehead.

While drilling a hole in the roof of the water truck to mount an AC unit. EE was drilling a hole with a hand held drill and had EE's left index finger right where the drill bit broke through the roof and drilled into their finger. EE received 4 stitches and antibiotics.

The gate to the Tire System was left open and the heat from the kiln caught a tire on fire which lead to the belt line. We were able to contain the fire to the system itself and no other area was affected.

A roof fall occurred in L3/C88 due to water and poor roof geology in this area. The area was barricaded immediately.

a loader operator, was climbing down from the 980H loader, as they stepped down with left foot they felt a wobble. The next morning their left knee was tight and stiff. After receiving the MRI results, 7/10/26, it was determined operator has a slight meniscus tear. This is why it wasn't reported prior to now

EE was working on a hydraulic line that was leaking. When EE loosened the fitting hydraulic fluid sprayed on EE. EE was taken to the ER for precautionary reasons and prescribed antibiotics.

Patient states EE was bending over with a caulking gun to patch a hole, when EE "tweaked" EE's lower back. Strain with RICE method and OTC medications.

Employee stated that EE was wearing EE's safety glasses and hard hat while working. When EE took off EE's hard hat, something fell off on top EE's eyelashes. Foreign object in eye.

EE was splitting a sample in the test lab. EE was lifting a splitter box to dump and felt EE's right bicep get tight. EE was taken to OMP for evaluation and referred to a specialist for follow up.

While walking down the shop stairs the employee felt and heard a pop in EE's knee causing pain and trouble putting weight on. Employee was prescribed medication for swelling and a knee brace resulting in a reportable injury.

Employee received abnormal results on hearing test for right and left ear. Employee was retested and for a second time received abnormal results for right and left ear.

While performing maintenance on a gearbox, a secured motor shifted after a support bolt was removed and struck the employee. The employee sustained a neck injury.

Employee exited truck without putting truck in park. Truck started to move and employee tripped while attempting to stop the truck. Truck came to a stop by berm. Employee failed to implement emergency brake and chocking tires. Original diagnosis was First Aid
