On 1/25/25 at approx. 8:30am a drill operator was injured when a hook from the drill winch cable fell from the top of the drill mast. The hook struck them over the left eye causing a laceration over their left eye. EE was taken to the ER, receiving multiple stitches. The operator had a follow-up appointment on 1/27/25 was released back to work 1/28/25 without Restrictions.

The employee was in the process of cleaning material off the concrete pad under screen #3 at approximately 11:00 am. While setting the hose on the ground, debris that had accumulated on the structure above broke loose and fell to the ground, striking the employee in the back of the hardhat and their upper back. Employee sustained multiple injuries.
Two employees were working on guarding at the motor station for the crusher. Below 12 feet to the left two more employees were working on the lube system automation when a flat bar that was resting across the handrails above fell and struck one of the employees in the right lower leg. Resulting in laceration requiring 11 stitches.
While another employee was removing rock from a scalping screen, a piece of rock fell approximately 40 feet and struck the injured employee on the right forearm, causing a fracture to the forearm bone just above the wrist.
Employee was crawling under the access stair platform when a piece of debris fell into his eye.
The injured employee was injured when a rock fell from overhead, striking him in the hard hat & head area.
Employee was shoveling under tail wheel located beneath primary dump hopper when a rock weighing app. 10-15 pounds fell from the side of the dump hopper and hit him in the head. No trucks were dumping at the time of the incident. The rock penetrated through his hard hat and caused a laceration which required 4 stitches.
Employee was cleaning up on Tower #1, as he was descending the stairs to the ground level a rock fell approx. 25' from a hole in a chute and struck the employee on the right wrist.
INJURED WORKS FOR OTHER MINE- AS A QC TECH. ON THIS DAY HE WAS WITH A CONSULTANT AND WENT TO THE PIT TO INSPECT ROCK AT THE TOE OF A SLOPE ONA RECENT SHOT. A ROCK ABOUT THE SIZE OF AN ORANG E ROLLED DOWN SLOPE STRIKING SHOE THEN BOUNCED ONTO HIS SHIN CAUSING A SMALL CUT. INJURED HAD BEEN HAZARD TRAINED PRIOR TO ACCIDENT.
INJURED WAS WELDING IN PLACE, GRATING MATAERIAL FOR WALK-WAY AROUND NEWLY INSTALLED JAW CRUSHER IN A SQUATTING POSITION. HE TURNED TO REPOSITIONHIMSELF & A PIECE OF MATAERIAL TO BE USED 1/4" X 4' X 42", STANDING VERTICAL, FELL STRIKING HIM ON THE LEFT CHEEK.
AN EMPLOYEE WAS MEASURING FITTING SFOR A CRUSHER OILER WHEN HE WAS STRUCK ON THE RIGHT CHEEK BY A FALLING ROCK RESULTING IN A LACERATION.
1ST EMPLOYEE WELDING ON QUARRY WATER LINE WHICH GOES UP BESIDE QUARRY WALL. SECOND EMPLOYEE WAS STANDING IN BOAT TYING A ROPE TO A PRY BAR WHICH #1 EMPLOYEE NEEDED. WHLE TYING ROPE ROCK FELL FROM QUARRY WALL STRIKING #2 EMPLOYEE IN THE HEAD.
EMPLOYEE WAS WA5KING UNDER THE RIP RAP STRUCTURE WHEN A ROCK FELL FROM ABOVE STRUCTURE AND HIT EMPLOYEE ON HIS HEAD. EMPLOYEE WAS WEARING HIS HARD HAT AND SAFETY GLASSES.
EMP WAS IN THE LOAD OUT TUNNEL SHOVELING ROCK FROM UNDER CONVEYOR WHEN A PIECE OF TIN THAT HAD BEEN POSITIONED OVERHEAD TO KEEP WATER FROM RUNNING ON WORKERS FELL STRIKING EMP ON LEFT EAR INFLICTING A LACERATION REQUIRING 4 STITCHES
EMPLOYEE WAS STANDING ON THE NE LOADING BIN LOADING THE STOCKPILE TRUCK A STOCK OF SCREEN CLOTH WAS STORED AGAINST THE WALL ON THE FLOOR ABOVE THE WEIGHT OF THE SCREENS BENT THE TIN WALL OUT ALLOWING A 3 FT LONG BOARD TO FALL HITTING TRHE EMPLOYEE BELOW ON THE RT HEEL
