Employee was scaling highwall when struck on the right thigh by conglomerate of mud/rock resulting in laceration requiring sutures

EE was cleaning a cross beam (in the redzone) at the head of C25 when EE was pulled into a return roller.

EE had parked truck at equipment line and gotten off truck (end of shift)when they decided to walk towards another parked haul truck to let the operator know about chock block. They walked over a small pile of wood from a new oil storage building that had just been built when the piece of wood moved causing them to lose footing and fall backwards onto ground.

Employee was setting up a ladder for another employee to work on a rock breaker. When doing so tripped backwards and cut their arm on a step of a service truck.

Employee stepped into a pile of sand and twisted EE's knee.

Violent storms over the weekend knocked down a tree, which in turn knocked down a power line causing the power to go out at the plant and to the pumps. Heavy rains in excess of 6 inches caused flooding. The heavy rain washed out berms and excessive water running down the decline into the mine. MSHA was notified about the flooding.

Employee was opening the can of welding rod and the lid cut their left hand.

Employee was hooking up input and output lines to a gear pump. Employee started feeling dizzy and briefly lost consciousness. EE was taken to the clinic for evaluation and treated for a personal medical issue.

EE was running the rock breaker while breaking a knot when the excavator jarred sharply. This sudden sharp movement tweaked something in their back. The did not seek medical treatment until July 2nd.

Employee pinched right middle finger between plate and wear liner, amputating the top portion of the finger midway on the nail bed, above the top knuckle.

2 employees were moving a wire cable reel in preparation to pull cable for a new service when one employee lost balance/footing and fell backwards. As they fell backwards, they landed on their rights side, mostly on their right elbow. Employee continued the shift/work as the supervisor and only felt like they bruised their elbow.

EE was removing a wear pad when it fell landing on EE's thumb.

EE was turning a wrench tightening a bolt on a screen when EE said that EE hurt something in EE's back, which caused immense pain. EE described it as a knife sticking between EE's back and EE's hip.

Employee was performing maintenance on the 992K loader bucket. While EE was swapping out the bucket teeth, EE sustained an injury to EE's left middle finger.

While replacing an HSI crusher lift cylinder, the component shifted, pinning the employee's finger against the crusher wall. The employee sustained an open fracture of the left middle distal phalanx. Treatment included stitches, a splint, and medication with work restrictions.

Employee was driving a haul truck and stated that EE's lower back was hurting so EE went to personal doctor. On 5/18 EE's MRI was read and was told EE had a bulging disk. Employee was then sent to a Workers Comp. Doctor on 6/3 and was put on restrictions, making it reportable.

Employee was climbing the stairs and as EE attempted to step up EE's foot slipped causing EE's weight to be completely supported by EE's arms resulting in EE dislocating EE's right shoulder.

Operator cut forearm on mast unloading a drill pipe off drill rig. Operator was using improper unloading techniques which resulted in loss of pipe control. This caused operator to be pushed into the mast and cut EE's arm. Operator has since been retrained on proper loading and unloading of drill pipe for Gill Beetle Drill on 4-28-26

Employee was removing the bucket on the skid steer when it fell landing on EE's foot.

Employee injured EE's ankle after mis stepping and jumping to the ground while exiting an excavator cab.

2 men working close together with knives. ! technician pulled EE's arm back as the other lifted EE's knife striking EE in the arm. EE was cut and needed stitches

EE was tracking Conveyor belt, and saw an Idler not rotating. EE used their hands to try to break the Idler free, The roller starting rolling which resulted in them being pulled into the Idler. their left arm was pulled between the Idler and belt.

EE was attempting to move a rock that weighed approximately 40 lbs. from the catwalk. When they swung the rock, it did not clear the middle handrail and pinched the tip of their left ring finger between the handrail and the rock.

Employee was trimming rubber for a back skirt on a conveyor belt. While cutting the rubber the razor knife slipped and cut the outer right thigh.

Employee was replacing a transmission in a pick-up truck. They used a floor jack to lower the transmission. While lowering the transmission, it became unstable and started to roll. When it did, the employee smashed their finger between the transmission and the jack. Their right-hand pinky finger had a laceration that required sutures and fractured tip.

Employee was using a hammer when it glanced off and struck their left index finger, causing a laceration.

Employee was gripping a screen by putting EE's fingers through the openings to lift it. As EE was picking it up, it caught against another screen as EE pulled up, it came loose dropping and pinching EE's finger between the two. Employee sustained a cut to a finger on EE's left hand and received six stitches.

Employee was getting off of the back of water truck. Upon exiting, EE's glove became stuck, lost EE's footing and was swung towards the back bumper which is about chest high. Employee sustained soreness to the ribs. After a CT scan on 05/13/2026, it was discovered the employee had a fractured rib.

While mounting drill, employee alleges they lost footing, slipping but catching themselves and felt pain in their back.

A 600 psi air hose on the drill ruptured causing the window to shatter. The operator suffered a laceration on the top of their head requiring 3 staples to close.

The employee was changing the jaw cylinder when a bolt fell landing on their pinky finger.

At 7:05am it was discovered that the wildfire originating off-property had spread onto the west and north boundary lines of the property. The fire did not result in any property damage or injuries to employees. Facility was able to reopen the following day.

Employee claimed that EE hurt EE's shoulder while digging in the muck pile with the pit loader.

Employee was operating a loader when they alleged they hit a pothole that jarred their back.

Employee was welding when a piece of slag got into their eye causing irritation.

Employee was using a personal knife to cut off a piece of tread from their boot. When the employee attempted to close the knife, they closed it on their left middle finger.

While changing cutting edges on a 992 loader bucket, employee reached under to check if bolt was spinning or not. While grabbing the bolt the employee accidentally pulled the trigger on the impact gun and turned the bolt causing it to slice their finger.

An employee was removing a lever hoist used to align a shaft during a bearing replacement. During the lever hoist removal, an employee's nose was lacerated when struck by the lever hoist. The initial X-ray results were negative for any fractures or broken bones. A fracture in the nose bone was revealed during a follow-up CT/MRI scan on 3/3/2026.

Department of Agriculture inspector had Left Pinky Finger stuck between weights cart and weight that led to 7 stitches

Employee was assembling a smudge pot heater when they felt something snap in their left hand. They sought medical attention February 11, 2026 and was diagnosed with a sagittal band rupture. Employee is currently on restricted duty.

As employee was exiting the wash plant they jumped over a drainage ditch and felt a pop in their left knee. On February 16th employee was diagnosed with an ACL tear and is currently on restricted duty.

Loose clothing got caught on rebar causing trip.

Employee alleged that EE wiped EE's eye and inadvertently transferred something from EE's glove into EE's eye.

Employee was helping replace the swing bracket on a Backhoe. EE was disassembling the crate that contained the new bracket. They struck a board downward on one side, causing the other side of the board to come up and strike them in the face. Employee's lip laceration was sutured and they were released with restrictions.

Employee was in a haul truck when a rock fell and jarred the truck. Employee complained of back pain and utilized the RICE treatment until the January 24th. Up until the 24th, the employee stated multiple times they were feeling better each day. Employee went to ER with back pain and was released with prescriptions.

While welding the employee noticed irritation to one eye and realized EE had something in EE's eye.

An employee was cutting a hole in a urethane cover with a blade to access the fill plug when the blade slipped and cut into their hand, causing a laceration between the thumb and index finger.

Employee was attempting to remove a screen from the lower deck, when the screen became stuck. The employee chipped out some frozen material and EE attempted to remove the screen again. As the screen came loose, the employee jolted backwards and hit EE's head on a crossmember in the screen. The employee decided to seek out medical attention on 12/17/25.

Employee was using a 12-ton bottle jack to lift forms in order to adjust a wheel position for form reduction. The employee's left hand was positioned between the bottle jack and the formwork. Upon engagement, the forms shifted, pinching the employee's left index finger between the form and the jack

On 10/30 employee had filled crusher with diesel & was descending the ladder. As EE was stepping down their foot slipped causing them to hang onto the handrail by their right arm. First aid was administered from that day forward. On 11/14 EE requested to go to the clinic. Diagnosed with strain of their right shoulder rotator cuff. Prescribed medication and physical therapy.
