The IE was air arcing in the main shop when IE's clothing caught fire and caused burns to IE's skin.

During shovel disassembly, cutting operations were taking place and a hot spark started some combustible materials smoldering. The employee scooped the material with a dust pan and when he dumped it the material flared up causing 2nd degree burns to the face and neck. The doctor placed him off work until the blisters healed to prevent infection.

EE set the parking brake and was preparing to exit the truck to inspect it, when he noticed the truck fire. He shut down the engine and activated the suppression system by pulling the pin and striking the actuator. EE jumped from the drivers side catwalk. EE sustained a fractured left hip and dislocated his left elbow. Fire was extinguished in approx 20 minutes.

EE WAS ATTEMPTING TO START THE REAR ENGINE ON THE SCRAPER USING STARTING FLUID. AFTER SEVERAL ATTEMPTS FAILED, EE USED OXYGEN FORM A WELDER TO ENHANCE THE COMBUSTION PROCESS. AFTER SEVERAL ATT EMPTS TO START, THE MIXTURE IGNITED.

EMPLOYEE STATED HE WAS STANDING ALONGSIDE THE JOY 12CM27 MINING MACHINE PERFORMING OWRK ON THE METHANE MONITOR READOUT DIAL AND WITHOUT WARNING A PIECE OF ROCK FROM THE MINE ROOF FELL AND STRU CK HIM ON THE HEAD, NECK AND LEFT SHOULDER. THE ROCK MEASURED TO BE 6'X3'X3". THE EMPLOYEE WAS TRANSPORTED TO A MEDICAL FACILITY. ER BY AMBULANCE WHERE HE WAS TREATED AND RELEASED. A WITNE

AFTER DOING OIL CHANGE ON D-11 DOZER, EE CLEANED HANDS WITH STARTING FLUID, AND REACHED INTO HIS POCKET FOR A LIGHTER. BEFORE HIS HAND WAS OUT OF THE POCKET, THE LIGHTER SPARKED CAUSING 1ST & 2ND DEGREE BURNS ON RIGHT HAND. HE WAS TAKEN TO HOSPITAL WHERE HE WAS TREATED AND RELEASED WITH RESTRICTED USE OF HAND UNTIL FOLLOWUP VISIT ON 5-10. WHEN HE WAS RELEASED WITH NO RESTRICTIONS.

EMPLOYEE WAS WELDING A LATCH ON HIS TOOL BOX WHEN HE BLEW A HOLE THROUGH HIS TOOL BOX WHICH IGNITED A CAN OF OIL. A FLAME BLEW OUT THROUGH THE HOLE AND BURNT HIS FACE.
