The EE was closing a hatch on 332BE01 bucket elevator when EE struck EE's forehead at the left eyebrow level on the hatch keeper located above the hatch. The keeper is normally in the "up" position but fell down as the EE began to stand. The laceration required 5 stitches to close.

While installing new cassette parts in the clinker cooler, a millwright pinched the fleshy pad on the back of rt thumb between the porta-ram shim plate and the bottom of the cassette when the cassette unexpectedly fell into place after initially hanging up.

The L8 drill operator was exiting the cab of the drill when EE pinched and cut the pinkie finger of right hand on the door grab bar and latch. The operator had removed the leather utility gloves was wearing a pair of light nitrile gloves as EE was going spray paint markers for drill holes.

While cleaning the pre-heater tower at the level 1A feed shelf, and wearing the proper personal protective equipment, a large plume of dust came up and under EE's face shield and safety glasses and got into EE's right eye. The tower went slightly positive. The injury is a corneal abrasion of the right eye.

EE and another ee were moving a 3" steel pipe. The other ee dropped end causing the pipe to bounce on ee shoulder. EE worked for a few days and was than later seen by a doctor on 11/25/16, which brought no concerns and ee returned to work. On 12/8/16 ee went to a follow-up visit. This doctor placed ee on miner work restrictions.

Employee(EE) was working with a crew to install a platform which consisted of rigging with chain falls and slings. While adjusting the nylon sling the rigging slipped and caught the EE's finger between the choker and beam causing a fracture. EE did not request medical treatment until two days after the incident.

EE was working with a Journeyman who was working on a lift. EE knew the lift was to be moved shortly and saw a metal pipe in the way. EE decided to move it. It was wet and heavy and it slipped while lifting and hit EE in EE's arm causing a slight fracture in forearm near EE's right wrist.

Employee fractured right pinky finger when it became caught between two pedestals. Employee was terminated on 5/29/16 due to becoming incarcerated.

Employee was turning a large nut onto an anchor bolt. A co-worker lost his grip on the anchor bolt and it dropped 6 inches. Employees finger was caught between the nut and the equipment base. His right ring finger was lacerated, he received 6 stitches.

Employee was prescribed physical therapy on 8/28/15 after a shoulder strain that occurred on 5/30/15. Strain was treated with first aid until 8/28/15.

EE was pulling heavy cable from an old cable tray that was being replaced when he felt a twinge across his mid-section. EE continued to feel worse as the shift proceeded. EE has an umbilical hernia.

EE was shoveling spilled material from the floor around the shuttle-belt puller motor when he felt a burning sensation in his right groin. Took short break, felt well enough to continue, changed up position and continued at easy pace. Notified supervisor of pain, kept eye on area.

A construction worker was forming columns on the second floor of a new pre heater structure. He was located inside of the beam form. There was another worker on the beam side of the concrete form, when an iron worker stepped on the form causing it to tip over onto the carpenter causing the injury.

Employee reached to open the door of the loader with his left hand. When he pulled the level the door sprung open and caught his thumb and pulling it backwards.

Employee was placing a metal plate onto a cutting table. The plate was not placed completely onto the table, so the plate fell and landed on the employee's foot.

An employee was cleaning the oxygen analyzer when dust from above fell getting in his eye.

Employee was opening the gate to the yard. A bolt is inserted into a bracket to ensure the gate stays open. As the employee pulled the gate to the bracket, his left index finger was pinched between the gate and the bracket. A cut on the finger required stitches to close the wound. The employee was released for regular duty and returned to work the same shift with no impairment.

A sudden gust of wind caused foreign particles to get in employee's eye.

Employee injured his shoulder while placing pallet toppers on a pallet.

Temp. worker was performing cleaning task when he indicated his back was sore but could not identify a specific incident other than repetitive twisting motion.

The employee was installing a railcar vibrator when he stepped on his lanyard causing him to miss place the vibrator straining his shoulder. First aid of an ice pack was administered. After two weeks the employee felt it was not improving and requested to see a Dr, who prescribed a muscle relaxer.

Employee was helping putting a scaffold together when an X brace came off and hit the employee in the mouth chipping front teeth.

A mechanic was rigging a bearing cap with 2 come alongs, when side loading caused the load to fall from support, striking the employee on the foot. Xrays of the foot revealed no breaks - just swelling.

While lifting the bed of a gator to check the oil an employee felt a pain in his stomach.

Employee was shoveling material when a jackhammer, which was propped near by,fell and struck him on the foot.

Employee reported back injury while lifting safety gate at the manlift elevator. Note: Employee didn't see doctor until 11/1/2009.

Employee was inspecting 8t carrier deck. Lifted engine cover but failed to use cover rod to hold engine cover up. Engine cover slipped out of his hand and fell on his left hand. Fractured 3 fingers.

The employees hand was pinched between a nitrogen bottle and a handrail.

Employee was pulling up a bucket filled with material when his helper stopped pulling the rope causing the weight of the bucket to pull the employee across the railing causing a strain on his lower back.

Employee hurt his back while pulling buckets of rock and material from one level to another. Initial diaganosis: possible back strain; return to work 4/14/08 at full duty. 2nd opinion: 4/22/08: Light duty for seven days and medication.

Employee was removing guard off a belt and it fell. He grabbed it and injured lower back.

EE was working with team to clear plugged cyclone, when the screw conveyor stopped, caused hot material to spill out onto the floor.

Employee alleges lumbar pain while lifting a box. Note: Incident occurred on January 3, however employee didn't seek medical attention until January 18, 2008.

Employee was lifting a bag of masonry and felt pain in right leg. Note: Injury not reported until 8/20/2007.

Employee was checking an air slide on a silo to make sure it was not plugged up. While doing so he notice the pin had backed out of the cylinder head that operates the gate that feeds the air slide. When he tried to replace the pin back in the head of the cyclinder it went closed & wedged his finger between the cyclinder head & a sharp metal edge.

Please note: this injury was not reported by the employee to his supervisor until 08/22/2007. Lower Back strain from lifting cement bags.

Employee was separating green tires when he pulled a muscle in his lower right back.

Employee was trying to open tipping valve slide gate on LSI system to #1 Kiln. Gate broke loose and closed pinching right thumb between lip of slide and tipping valve housing.

Employee strained right shoulder while moving cement bag on line.

Supervisor suffered mild concussion, bruises, lacerations and chipped teeth when he opened a tail pulley guard that was full of stone. The pressure behind the guard sprung the guard open and the guard struck the supervisor.

EE injured his right ring finger and pinky when attempting to dislodge a 4" round x 48" long bar stock that came from the 2X belt scale.

When claimant was pushing clinker with industrial shop broom, he felt a shart pain across his mid-back section.

Was under the bulk truck releasing the air valve on the air tank & while getting out from under the truck hurt back.

Employee strained back while removing dust collector bags.

Employee developed burn, after cement kiln feed got into boots.

EE was installing a cable tray, when cement dust, which contains lime, went down his collar onto his neck. Resulting in 2nd degree burns.

Cement dust got into right eye while cleaning, employee was wearing safety glasses.

Employee was filling a cement bag when the bag exploded out the top and cement dust went into employee's left eye

Employee twisted right knee walking down stairs.

Employee was placing scrap metal into a recycle bin. He noticed a length of conduit sticking out over the top of the container. He reached up and started to push it into the container. His left forearm grazed a sharp pipe that was also sticking up resulting in a cut to the left forearm.
