Fire in coal mill dust collector was reported to MSHA phone hotline. No injuries.

Employee tripped on a beam and reported sore pain in ankle and calf, at initial report thought to be a sprain. Later in the week, employee was diagnosed with leg fracture.

Employee was driving a loader when bucket hit ground. Employee lunged forward hitting chest on the steering wheel.

While exiting vehicle in parking lot before shift start, employee slipped and fell. Employee hit their head and had minor abrasion on right pinky finger. On 2/10/25, employee was diagnosed with concussion.

While moving a ladder, employee knocked dust off of light. Dust got into employee's eyes.

Employee dropped roller on gloved hand fracturing middle finger and causing bleeding.

Employee was actively building scaffold in the mill when EE came into contact with an unidentified live wire. Employee received a shock which caused EE to lose balance and fall approximately 2'. Employee's harness caught EE, EE got self off the catwalk and went for medical treatment. Received scratches to left arm and was taken out of work by doctor until 8/23/24

Employee was shoveling and felt pop/pressure in abdomen. Employee diagnosed with hernia. Doctor restricted employee to limited lifting until follow-up.

Worker felt pain in right elbow while lifting a piece of scaffold from ground level.

While accessing the loader, the employee lost their grip on the door handle and strained their back.

Employee was removing the lid of the level 6 tipping valve, as lid was tensioned using a come along the lid dislodged and swung into employee's left shin causing a laceration requiring 10 stitches. Assessment of risk was not fully completed for line of fire hazards. The employee returned to work for next scheduled shift.

Employee was using a jackhammer to clear out a choked vibrating feeder, after working and clearing the blockage, employee reported soreness in left shoulder. Employee went to the doctor on 8/14/2023 and was put on restricted duty.

Driver was walking from driver's truck into a small building on site to sign for driver's load. Driver states driver thinks driver tripped over the door tread. The driver fell to the concrete floor suffering a broken hip. Weather was dry and clear. Walking surface was clean and dry.

4 contractors were entrapped in the preheat elevator at 2:20pm MSHA called at 2:50pm. Miners rescued at 5:15pm entrapment of 4 contractors - investigation ongoing

4 contractors entrapped in side pre-heat tower elevator for 36 minutes. MSHA was notified at the 30 minute mark.

Driller was tramming to drill site, when the drill tipped over. Operator was not severely injured. Minor cuts and staples applied to laceration on head

Not serious - Repetitive strain injury to left shoulder. employee reported injury to shoulder from operation of loader controls

Employee was walking in warehouse when EE stepped on the side of a pallet and rolled ankle.

Operator was climbing a ladder when operator heard a pop in left shoulder.

Miner coming down from preheater tower when elevator stuck between level 1 and ground floor. Contact control room and elevator company contacted. Entrapped for 1 hour 20 minutes.

Employee was cleaning plugged air slide. Dust in the air passed through seal of glasses and got in EE's eye.

Installing a rock anchor into the tunnel roof. Piece of rock fell from the roof and hit worker in the face, EE fell into the tunnel wall. Co-worker was installing the rock anchor. The worker installing the anchor, heard the injured employee yell. Startled, co-worker dropped the bolt wrench striking the injured employee on the back of their head.

The employee reported that left shoulder was sore and stiff after jack hammering under a conveyor belt during scheduled shift. The injury became reportable on 3/3/2022 when physical therapy was assigned.

Smashed left pinky finger between wheelbarrow and wall.

Two contractors stuck in pre-heat elevator (entrapment)

3 employees trapped in freight elevator on preheater tower between floors for 3 hours. No injuries and the condition of the employees was monitored via radio communication at all times.

While removing a chock block from the haul truck, the employee felt a pain in their right shoulder. Diagnosed with rotator cuff arthropathy. Employer learned of diagnosis on 10/5/2021.

Elevator malfunction.

While lifting limit amp contactor out of electrical cubicle, the contactor rolled back and pinched employees right index finger resulting in a laceration.

Employee assigned to housekeeping activities where employee was carrying wooden pallets. Part of the pallet fell from the employee's grip causing a trip and fall landing on both hands. Employee diagnosed with epicondylitis.

An employee reported that their shoulder was still sore the morning after operating a jackhammer in an awkward position the day before inside the 211BC-01 drag. Injury reportable on 2/22/2021 due to medical treatment.

2 production employees were working on unplugging the coal railcar unloading hopper of frozen material. While cleaning the material, a large piece of frozen coal, approximately 0.5m diameter, exited the open door on the hopper and struck one of the two employees in the right forearm resulting in a fracture.

Employee was latching the railcar swing door, while pulling downward on the swing door pry bar, the pry bar came out of the latching knuckle. The employee felt a pull on the left shoulder and left arm briefly went numb. Strain to Left shoulder.

Employee was working on overhead sensor system smoke detector attempting to install a temporary junction box cover to conceal the detector leads until a replacement was received.  During the fabrication, employee used a blade retractable utility knife. Employee inadvertently cut through EE's left mechanic gloves and resulting in single laceration to EE's left forefinger.

Employee was coming down a ladder and bumped left knee on a rung. The employee was on the third 20 foot ladder set from the top of the silo.

Employee was traveling north in bobcat, hit a bump in road, and states their leg tripped the lockout bar which put the machine into lockout mode. The vehicle came to a sudden stop which caused employee to travel inside machine as far as the seat belt would allow. This resulted in a right shoulder strain.

Employee was in the Preheat Tower on the second level removing a thermocouple. While pulling on the thermocouple to remove it employee believes they may have strained left arm. Incident became reportable on 8/14/20.

While walking with ladder on employees shoulder, EE tried to lift the ladder over the railing of a portable stair case causing EE's body to come out of position and straining EE's back. Injury became reportable on 7/27/2020 due to medical treatment.

Employee was replacing buckets on bucket elevator chain and was holding new bucket to bolt into place when EE felt 2 pops in EE's right arm. Employee diagnosed with a detached right biceps tendon.

The employee was using a come-a-long to work on a coal mill roller roll. Employee was tightening down the nut on the top of the tapered journal assembly. At the end of the day employee felt soreness in right shoulder. Employee did not report the incident immediately. The incident became reportable on 5/18/2020 after employee was given restricted duty.

While shoveling in the tunnel to clear material from under a transport belt the employee felt some discomfort in the ribcage on the left side in the area below EE's heart. The incident became reportable on 3/16/2020 due to medical treatment provided.

While setting out Chains to be installed on the 993 loader, employee was pulling on some of the links to spread them out. While doing this, elbow started to hurting.

While being loaded with blasted stone into the haul truck, a stone from the kalkberg bench caused the truck to jolt which then caused the employee to experience a "pop" in the neck. The employee stated that EE initially felt pins and needles but that subsided by the time EE got up to EE's phone to inform the supervisor.

While walking along the boom catwalk at the wharf employee tripped and attempted to catch themselves with left hand on the handrail.

EE was assigned to clean and wash out the inside of the coal mill and slipped while in the mill and fell to EE's left. When falling EE reached EE's left arm out to EE's side to catch self and upon impact EE's left hand and thumb bent backwards when bracing self from the fall.

While removing buckets from the stacker reclaimer with torches and hand tools a bucket fell off and landed on EE's foot. Bucket landed just above the steel toe of EE's right boot.

Employee was at the north coal feeder. EE was shoveling coal from around the base of the feeder. While twisting the shovel to dump it they felt a pain in the right bicep.

Employee was tasked with cleaning out return roller guard of built up material on Mill #1 feed conveyor. While employee was reinstalling the guard they tore left bicep muscle.

River Belt Conveyor system (8A + 8B) caught fire. The fire was due to a malfunction in the tail pulley bearing on 8B which caused excessive heat and tracking issues with the conveyor belt. A fireman, from the Ravena Fire department was transported to Saint Peter's Hospital where fireman was treated for over exhaustion and dehydration.

EE was walking down the east side staircase from the second floor of the pug mill to the ground floor when at the second to last step EE felt EE's right leg give out and EE fell. EE noted pain in the area of EE's right knee and could not bend it.

While working on the stone box floor of the primary crusher on the lube system an EE and IE had the power go out without notice at 07:45. As they were leaving the lighting was very dim as only the emergency lights were on and IE tripped over a pad eye on the floor welded to a steel plate and fell on IE's left shoulder and the left side of IE's face.

EE hit their head on a pipe line for compressed air on the second floor of the packhouse by the inside lane conveyor belt which jerked EE's head back causing EE's neck to pop. At this time EE felt minor discomfort in neck but continued to work next shift without issue (3/10).

Installing steel plates and shoring in open excavation to perform work on plant drainage lines.

Employee working from manlift next to conveyor beltline. As they were leaning on the belt it unexpectedly started up pulling them in their harness away from the manlift.

On 10/31/2018 around 1300 ee was traveling in the PreHeat Tower area on windy day. Dust particular had entered left eye under eye PPE.

While removing concrete from a core drilling operation, the tool used to remove the concrete core slipped and pinched the employee's hand. EE's right hand was caught between a piece of plywood and the rebar causing a laceration to EE's right hand through EE's glove.

The ee was performing the routine task of cleaning the feed shelf in the PHT. EE was using an air lance through a poke hole to move material when some material exited the poke hole and fell into the gauntlet of EE's left glove and burning EE's exposed skin. EE was not wearing all mandatory PPE provided for the task.

The EE was closing a hatch on 332BE01 bucket elevator when EE struck EE's forehead at the left eyebrow level on the hatch keeper located above the hatch. The keeper is normally in the "up" position but fell down as the EE began to stand. The laceration required 5 stitches to close.

While installing new cassette parts in the clinker cooler, a millwright pinched the fleshy pad on the back of rt thumb between the porta-ram shim plate and the bottom of the cassette when the cassette unexpectedly fell into place after initially hanging up.

An apprentice millwright attempted to remove by hand a frozen chunk of bauxite from the feeder. The chunk rolled pinching right ring finger between the chunk and the feeder frame. The wound is a laceration to the right ring finger.

A new production control supervisor (PCS) was assigned the task of changing valve positions in the buffer silos. The PCS used the incorrect tool for the task (too large). When tapping the actuator ee struck/pinched the tip of right ring finger between the tool and a pipe flange. The injury is a laceration and fracture of the finger tip.

Employee was grinding a shim plate on top of the kiln (9' grinder). The grinding wheel flew off of the grinder, struck the Tyre, deflected back and struck employee in the right thigh. First aid was administered. Employee was taken to urgent care where they received 5 stitches to right thigh.

The EE was working with a vacuum truck crew cleaning spillage. EE used a utility knife to cut through duct tape at the joint of one hose. The blade became stuck and EE exerted upward force. The knife came free of the tape and struck EE in the rt leg just above the knee. The injury is a laceration. First aid was applied.

The L8 drill operator was exiting the cab of the drill when EE pinched and cut the pinkie finger of right hand on the door grab bar and latch. The operator had removed the leather utility gloves was wearing a pair of light nitrile gloves as EE was going spray paint markers for drill holes.

While cleaning the pre-heater tower at the level 1A feed shelf, and wearing the proper personal protective equipment, a large plume of dust came up and under EE's face shield and safety glasses and got into EE's right eye. The tower went slightly positive. The injury is a corneal abrasion of the right eye.

EE was assigned clean-up work on Belt #9. The weather was windy with strong gusts. When leaving the MCC after locking out, a strong gust of wind blew material into their right eye. The EE was wearing protective eyewear.

Electrician was stripping cable and using leg as a brace while stripping. Knife slipped off cable and cut leg through the pants making a 3" incision requiring stitches.

EE and another ee were moving a 3" steel pipe. The other ee dropped end causing the pipe to bounce on ee shoulder. EE worked for a few days and was than later seen by a doctor on 11/25/16, which brought no concerns and ee returned to work. On 12/8/16 ee went to a follow-up visit. This doctor placed ee on miner work restrictions.

Employee(EE) was working with a crew to install a platform which consisted of rigging with chain falls and slings. While adjusting the nylon sling the rigging slipped and caught the EE's finger between the choker and beam causing a fracture. EE did not request medical treatment until two days after the incident.

Employee was attempting to cut a zip tie from a flange when knife slipped off and stabbed self in the left wrist. Employee was taken to Albany Med urgent care and received 4 stiches. Return to work after the incident with no restrictions.

Smoke & steam were observed rising from the southernmost coal stockpile in the secondary reclaim area. A dozer operator was dispatched to spread and compress the stockpile. The operator was task trained and provided with a four gas air monitor for the cab. Notice to MSHA was made at 10:15 and 10:30. The hot spot was extinguished. The pile was worked & compressed for cooling.

EE was working with a Journeyman who was working on a lift. EE knew the lift was to be moved shortly and saw a metal pipe in the way. EE decided to move it. It was wet and heavy and it slipped while lifting and hit EE in EE's arm causing a slight fracture in forearm near EE's right wrist.

EE tripped on low step and fell forward. As EE fell, threw arms in front of self to catch self. Rt. hand struck a pipe on the west side of the walkway. The EE tore the triceps muscle in rt. arm.

EE was trimming packing material with a serrated knife for a tail bearing installation on FM3 dust collector. EE stated was not having efficient success, so switched to using a box cutter. During the process of cutting action (towards body in a sitting position) the box cutter slipped off material, then cut a three inch gash in upper thigh.

Employees were manually moving a welding machine. After they rolled the welder into place and set it down, the employee stood up. EE twisted to walk away and felt a pain in lower back. EE went to one knee in pain. EE then stood up and proceeded to find a safe comfortable place to sit until safety arrived.

Crew member was cutting a piece of intermediate diaphragm. As employee was cutting through the piece, the piece broke free, swung and hit the employee on the head and hand.

Employee fractured right pinky finger when it became caught between two pedestals. Employee was terminated on 5/29/16 due to becoming incarcerated.

Slurry was found to be leaking from a crack on the SE quadrant of the north slurry basin (tank no. 2). The crack is located approximately 20 ft above grade. Slurry was spilling into a bermed area east side of the slurry basins.

The injured EE lost footing and fell backward (fall on same level) while pulling a return roller into place. As EE fell attempted to brace self with rt arm and felt a "popping" sensation as EE struck the floor. The injury is a strain to rt shoulder.

Employee was turning a large nut onto an anchor bolt. A co-worker lost his grip on the anchor bolt and it dropped 6 inches. Employees finger was caught between the nut and the equipment base. His right ring finger was lacerated, he received 6 stitches.

EE was using an air lance to clean the grizzly at the discharge of K2 coal belt when the steel tube unthreaded itself from the ball valve installed after the Chicago fitting. EE attempted to pinch the air hose but was struck on the rt. forearm by the air stream receiving a high pressure air wound. EE was transported to a local medical center for treatment and observation.

EE was cutting out damaged re-bar with a burning torch. EE did not realize that the piece he was cutting had stored energy. As he cut through on one end, the re-bar sprung up and struck him on the rt. forearm. The wound is a laceration requiring stitches

Employee was prescribed physical therapy on 8/28/15 after a shoulder strain that occurred on 5/30/15. Strain was treated with first aid until 8/28/15.

EE was pulling heavy cable from an old cable tray that was being replaced when he felt a twinge across his mid-section. EE continued to feel worse as the shift proceeded. EE has an umbilical hernia.

EE was shoveling spilled material from the floor around the shuttle-belt puller motor when he felt a burning sensation in his right groin. Took short break, felt well enough to continue, changed up position and continued at easy pace. Notified supervisor of pain, kept eye on area.

EE was pulling a new section of conveyor in place when he lost his footing and tripped, falling backward.

A construction worker was forming columns on the second floor of a new pre heater structure. He was located inside of the beam form. There was another worker on the beam side of the concrete form, when an iron worker stepped on the form causing it to tip over onto the carpenter causing the injury.

The affected ee has a pre-existing history of heart and seizure disorders. The ee was working in his office when he felt he was about to have a seizure. He reported this to his co-worker who helped prepare for the seizure and then notified the control room. The plant ERT was dispatched and 991 was contacted. EE was treated and transported by ambulance to a local hospital.

Fire and smoke was reported from Belt 1 drive tower to the Control Room at ~1:55PM. The control room called 911. All ee's were evacuated and accounted for. Fire depts arrived ~2:15PM. The MSHA Call Center was notified at 2:19PM. MSHA Inspector made contact at ~2:45PM. The fire was declared "out" at ~5:00PM. There were no injuries.

EE was walking behind hydraulic drive cabinet on kiln 1 drive pier when he began to slip and fall on an oily surface. EE caught himself but the motion strained his lower back.

The EE made a repair to a D9T dozer. After climbing down from the dozer he stepped backwards and tripped in a frozen rut. He landed on both hands spraining both wrists.

EE rigging comealongs when repairing drag conveyor. The comealongs were anchored to a steel pipe whick broke striking the EE in the head. EE was stabilized and transported by ambulance to local ER. He did have a concussion. EE was examined and released later in the evening.

The EE was pushing a hand truck into the Mill room thru the center overhead door and stepped on a mill ball with his left foot. As he stepped on the mill ball he felt a sharp pain in his left knee, as he felt the pain in his knee he shifted his weight to his left side. Note well: the diagnosis of a hernia was made on 01/21/2010.

EE was attempting to clear a jam on Raw Mill No. 2 circ pump with modified 4' pipe wrench and come-along on shaft of locked out pump when the pump housing shattered. The EE was struck on the left shoulder by the pipewrench.

EE using a bench grinder to dress 1/8" thick steel band. Work piece, glove and then right hand was pulled into the grinding wheel. Right pinky finger broken in two places and badly lacerated.

An EE reported a fire at Kiln No. 1 booster heater at 6:01PM. 911 was called and the local VFD responded within 10 minutes. The fire was fully out at 6:35PM. The fire likely started when no. 2 fuel oil leaked from abandoned oil lines onto an electrical coil. MSHA Inspector Richard Moon (Albany Field Office) inspected the site on Tuesday, 01/19/2009.

EE was going from tug deck to barge deck when he stepped on a refueling line and rolled his rt. ankle.

EE was working with team to clear plugged cyclone, when the screw conveyor stopped, caused hot material to spill out onto the floor.

EE was removing oxygen cylinder from high pressure cylinder storage rack when he slipped and fell on ice. Cylinder fell on his knee.

EE was trying to unjam the East basin pumps by pushing on a 4' pipe wrench when the wrench slipped on the shaft and his hands slipped on the wrench (gloves were wet) and he lost his balance.

IRON WORKER WAS DRAGGING AN I-BEAM OUT OF THE KILN AND SLIPPED. THE I-BEAM CAUSED A LACERATION TO LEFT INNER THIGH.

WORKER WAS USING AIR TORQUE WRENCH TO FIT STEEL. HE ACTUATED WRENCH W/OPPOSITE HAND NEAR TORQUE ARM. TORQUE ARM PINCHED UNDERSIDE OF LEFT MIDDLE FINGER.

EMPLOYEE WAS GRINDING THE BEVELS ON THE KILN SHELL WHEN A PIECE OF GRIT GOT INTO HIS EYE. RX GIVEN.

Please note: this injury was not reported by the employee to his supervisor until 08/22/2007. Lower Back strain from lifting cement bags.

Injured EE was tightening bolts on upper section of vertical roller. He was working on a ladder in an awkward position holding the nut with his lt. hand and working a ratchet with his rt. hand. EE leaned over and around conveyor bedframe and roller mount. He felt a ripping sensation in his left shoulder.

EE overextended his right knee while working on a drag conveyor in the plant secondary reclaim. Poor body position and lack of job planning are the root causes.

EE cut middle finger of rt. hand with utility knife, when cutting section of hose from vacuum truck tak-up port.

Supervisor suffered mild concussion, bruises, lacerations and chipped teeth when he opened a tail pulley guard that was full of stone. The pressure behind the guard sprung the guard open and the guard struck the supervisor.

EE injured his right ring finger and pinky when attempting to dislodge a 4" round x 48" long bar stock that came from the 2X belt scale.

EE was releasing a 1.5 tone come-along when he felt pin across his lower back.

Employee got dust in his eye.

When claimant was pushing clinker with industrial shop broom, he felt a shart pain across his mid-back section.

EE fell tripped on crack in concrete slab injuring his left ankle. EE has pre-exiting condition toward severe damage to both ankles. Still under internal investigation.

Operator of International Water Truck states that water truck lost brakes as it descended 9% grade toward CKD Landfill. Truck struck abandoned fan housing for old pelletizer silo and came to rest against the scraper tandem. The truck did not overturn. The operator was wearing a seatbelt.

Employee cut the palm of left hand while cutting shrink wrap. He was using a sheet rock knife.

The miner was injured while racking out a 4,160 V switchgear for a raw mill. The miner went to open the switchgear for the wrong mill. He opened the switchgear still under full load. The mechanical interlock failed to hold. The miner is seriously burned.

While attempting to repair traveler on 6th floor hoist with co-worker, employee was reaching the hoist pendant by extending his arm and felt a sharp pain in the left arm on the inside of his elbow. There is a large lump in the area and EE is not able to straighten out the arm without pain.

The employee has a hernia on the left side of his groin as the result of overextending his left leg to operate the rimpull pedal on a Cat 992G loader.

Employee rolled and sprained right ankle when he stepped on a seam between concrete slabs in the Maintenance Shop

Employee's right index finger caught in fan belt in drill rig motor causing partial amputation.

While jackhammering through an opening on top of a silo, the jackhammer bit droped through the material causing the jackhammer to fall pinching a finger between the silo roof and the jackhammer handle. The right ring finger finger tip required 5 stiches.

The employee tripped over a pile of material he was shoveling from the floor of the pelletizer. He attempted to stop his fall by bracing himself on the pelletizer dust collector fan housing. He cut the back of his right hand. The wound required three stitches. No time was lost. He returned to normal duties.

When claimant was jackhammering in the transport screw dust collector area, his jackhammering action must have caused some clinker dust to fall on him from above his work area.

The employee slipped on ice and fell on the deck of the barge Alexandria. The weather at the time was sleet and freezing rain. There is the potential for a slipped disc.

The employee walked up behind an operating backhoe. He did not make contact with the operator. The backhoe operator made a move with the bucket and machine lurched. The outrigger pad came down on the employee's right foot breaking the bones of the large toe.

Strained muscles of lumbar region of back while picking up a screw section from an awkward height and position.

An oxy acetylene torch backflashed. The employee was burned on the inside of his right forearm.

Employee sprained left ankle while attempting to walk over pile of chunk cement. Employee failed to follow Examination of Workplace SOP.

Employee tripped on chain hanger

USING A JACKHAMMER TO BREAK UP CLINKER, HAMMER GOT STUCK. EMPLOYEE WAS UNABLE TO PULL IT OUT. HE THEN FELT A MINOR TWITCH IN HIS CHEST. HE WAS SENT TO A LOCAL CARE FACILITY.

Employee was using a 6 ft. pry bar to clear chunks of clinker from the outside feeder for cement mill no. 2. As he was prying, the bar slipped and he felt a pull in his left shoulder.

A CONTRACTOR EE WITH 2 YEARS EXPERIENCE FELL 31 FT (9.4 M) TO A STEEL STRUCTURAL BEAM WHEN THE GRATING FAILED IN THE CATWALK ON WHICH HE WAS WORKING. THE INJURED CONTRACTOR WAS RESCUED FROM T HAT POSITION BY THE PLANT'S EMERGENCY RESPONSE TEAM AND MEDEVAC'D TO A LOCAL TRAUMA CENTER.

WHEN WALKING TO THE STAIRS FOR THE BELT NO. 8B SPLICE SHACK, THE EE SLIPPED AND FELL IMMEDIATELY. HE LANDED ON HIS BACK AND RIGHT SHOULDER TWISTING HIS RIGHT KNEE. THE EE HAD SLIPPED ON A PIEC E OF CONVEYOR BELT BURIED IN THE SNOW.

EE WAS OPERATING FRONT END LOADER TO REMOVE SNOW ON THE AGGREGATE BENCH OF THE CENTRAL QUARRY WHEN HE STRUCK A ROCK TOE WITH THE BUCKET IN THE SNOW. WHEN THE BUCKET STRUCK THE TOE, EE PITCHED FORWARD IN THE CAB & STRUCK HIS RT. FOREHEAD ON THE ROPS RESULTING IN A LACERATION. EE WAS WEARING A SEATBELT. THE LOADER HAS A LAP BELT.

THERE WAS NO FIRE.

EE WAS GETTING ON A CATERPILLER FORKLIFT WHE HE STRUCK THE OUTSIDE OF HIS RIGHT KNEE ON THE CORNER OF THE CAB OF THE FORKLIFT.

THE CENTRAL CONTROL ROOM OPERATOR MADE AN ERROR IN PUTTING HOT CLINKER ON THE CONVEYOR BELT RATHER THAN SENDING THE CLINKER DIRECTLY TO THE CLINKER STORAGE HALL. THE BELTING ON THE CONVEYOR WA S NOT A HIGH TEMPERATURE MATERIAL AND NOT SUITED FOR THE APPLICATION.
